Treatment strategies for complex behavioral insomnia in children with neurodevelopmental disorders.

PURPOSE OF REVIEW: This review describes recent research in pediatric behavioral insomnias in neurodevelopmental disorders and their treatment. RECENT FINDINGS: Insomnia in children with autism spectrum disorder (ASD) and other neurodevelopmental disorders (NDDs) is typically complex, chronic, and difficult to adequately control. Abnormalities in genetic and/or epigenetic regulation of sleep/wakefulness and its timing predispose patients with NDD to insomnia, although poor sleep hygiene, maladaptive associations, and limit-setting are likely to contribute. Parents are agents for change in pro

Sleep Symptoms in Migraine.

PURPOSE OF REVIEW: To review replicated and highlight novel studies of sleep in children and adults with episodic and chronic migraine. RECENT FINDINGS: Attack-related sleep symptoms are most common in the prodrome and may represent early activation of the hypothalamus rather than migraine triggers. Interictally, patients with migraine report poor sleep quality and high rates of insomnia symptoms. Cognitive behavioral therapy for insomnia in adults and adolescents with chronic migraine and comorbid insomnia results in significant improvement on their headache burden. Thus far, objective studi

Nightmares in borderline personality disorder - narrative review.

BACKGROUND: Borderline personality disorder (BPD) patients commonly suffer from nightmares. Still, the prevalence of this issue does not match the little clinical attention it usually receives. Nightmares impact sleep and daily functioning and may play a role in BPD symptomatology, including suicidality. Since BPD has been linked with high rates of suicide, the potential connection with suicidality is crucial to address. AIMS: To create an up-to-date review of current knowledge on nightmares in BPD and to explore the links between nightmares, insomnia, and suicidality or self-harm in BPD pati

Subjective and objective sleep in young people with borderline personality disorder features.

Characterising sleep in young people (aged 15-25 years) with borderline personality disorder (BPD) features is crucial given the association between BPD features and sleep disturbance, negative consequences of poor sleep, and normative developmental sleep changes that occur in this age group. The present study aimed to characterise the sleep profile of young people with BPD to determine whether this profile is non-normative and specific to BPD. Participants were 96 young people (40 with BPD features, 38 healthy individuals, and 18 young people seeking help for mental health difficulties withou

The Many Faces of Sleep Disorders in Post-Traumatic Stress Disorder: An Update on Clinical Features and Treatment.

Sleep disorders and nightmares are core symptoms of post-traumatic stress disorder (PTSD). The relationship seems to be bidirectional, and persistent disturbed sleep may influence the course of the disorder. With regard to sleep quality, insomnia and nocturnal anxiety symptoms, as well as nightmares and stressful dreams, are the most prominent sleep symptoms. Polysomnographic measurements reveal alterations of the sleep architecture and fragmentation of rapid eye movement sleep. In addition, sleep disorders, such as sleep-related breathing disorders and parasomnias are frequent comorbid condit

Association of neurocognitive functioning with sleep stage dissociation and REM sleep instability in medicated patients with schizophrenia.

Many patients with schizophrenia present with impaired cognitive functioning and sleep disturbances. Dissociated stages of sleep represent instability within distinct sleep regulatory cerebral networks. Previous studies found increased rates of rapid eye movement (REM) sleep abnormalities in patients with schizophrenia and a positive association with psychopathology. In this study, we examined if sleep stage dissociation and REM sleep instability was associated with neurocognitive functioning in a sample of medicated patients with schizophrenia. The analyses were performed on 31 baseline polys

[Sleep disorders].

There has been a great development in sleep medicine during the past 20-25 years and the International Classification of Sleep Disorders now lists about 90 relatively well-defined conditions related to sleep. Several of them are severely underdiagnosed. This review considers frequent and/or disabling sleep disorders, in particular disorders for which treatment is available.

SleepPPG-Net2: deep learning generalization for sleep staging from photoplethysmography.

Objective. sleep staging is essential for diagnosing sleep disorders and managing sleep health. Traditional methods require time-consuming manual scoring. Recent photoplethysmography (PPG)-based deep learning models perform well on local datasets but struggle with external generalization due to data drift.Approach. this study evaluates multi-source domain training for improving out-of-distribution generalization in four-class sleep staging (wake, light, deep, rapid eye movement) from raw PPG time-series. The trained deep learning model is denoted SleepPPG-Net2. Additionally, we examined the im

Sex Differences in Children with Uncomplicated Attention Deficit/Hyperactivity Disorder and Sleep Problems.

BACKGROUND: Approximately 7.6% of children are diagnosed with attention deficit/hyperactivity disorder (ADHD), and sleep impairments affect 25-85%. There is a noticeable lack of research on girls and sex differences. The aim of this study was to examine sex differences in children with uncomplicated ADHD and sleep problems. METHODS: Cross-sectional baseline data were retrieved from a randomized controlled trial with weighted blankets (55 boys and 41 girls, 6-14 years) on a cohort recently diagnosed with uncomplicated ADHD and sleep problems. Differences between boys and girls in ADHD symptoms

Are sleep disturbances causally linked to the presence and severity of psychotic-like, dissociative and hypomanic experiences in non-clinical populations? A systematic review.

The present review aimed to 1) identify what sleep disturbances co-occur alongside psychotic-like, dissociative and hypomanic experiences; 2) assess the strength of potential associations between the severity of sleep disturbances and of the experiences studied; and 3) appraise evidence for a causal link. MedLine and PsycInfo were searched and 44 studies were deemed eligible. Results showed that insomnia was associated with all individual psychotic-like, dissociative and hypomanic experiences reviewed (effect size range: small-to-large). Parasomnias were associated with all psychotic-like expe

Fractal dynamics in spontaneous motor activity associate with effective antidepressant chronotherapeutic response, cerebellar volumes, and white matter microstructure in bipolar disorder.

AIM: Bipolar disorder (BD) is characterized by circadian dysregulation, altered sleep-wake behaviors, and heterogeneous antidepressant response. Chronotherapeutic interventions such as total sleep deprivation combined with light therapy (TSD + LT) produce rapid antidepressant effects, but predictors of outcome are lacking. We investigated whether fractal dynamics of motor activity, indexed by the short-term scaling exponent α₁, are associated with antidepressant response, sleep-wake regulation, and structural brain measures and may serve as a biomarker of treatment responsiveness. METHODS: Si

Effects of light on biological functions and human sleep.

The nonvisual effects of light in humans are mainly conveyed by a subset of retinal ganglion cells that contain the pigment melanopsin which renders them intrinsically photosensitive (= intrinsically photosensitive retinal ganglion cells, ipRGCs). They have direct connections to the main circadian clock in the suprachiasmatic nuclei (SCN) of the hypothalamus and modulate a variety of physiological processes, pineal melatonin secretion, autonomic functions, cognitive processes such as attention, and behavior, including sleep and wakefulness. This is because efferent projections from the SCN rea

Sleep and psychosis: An integrative review of conceptual and neurophysiological associations.

The association between sleep-related phenomena and psychosis has received increasing attention in recent years, as sleep disturbances (e.g., insomnia) have shown robust associations with psychotic symptoms. However, the role of other, more specific sleep processes in psychosis remains unclear, despite their potential importance as symptom-related correlates. Particularly, phenomena such as dreaming and hypnagogic experiences may share certain phenomenological features with hallucination-like experiences. Given that both sleep and psychosis involve complex neural systems, examining their neuro

The sleep gap in deep brain stimulation in psychiatry: A systematic review.

INTRODUCTION: Deep brain stimulation (DBS) has emerged as a promising intervention for individuals with treatment-resistant psychiatric disease. While sleep plays a significant role in psychiatric health, the extent to which it impacts DBS patients is not known. OBJECTIVE: To quantify the number and type of studies that report sleep effects in DBS for psychiatric indications, and to report how sleep is impacted after DBS. METHODS: A systematic review was conducted using PubMed. We performed a search using the MeSH terms "deep brain stimulation" OR "DBS" plus each disease of interest. Inclusi

A theoretical perspective on the role of sleep in borderline personality disorder: From causative factor to treatment target.

Sleep plays a crucial role in emotion regulation. Disturbed sleep is therefore increasingly seen as a potential causal factor for the development and maintenance of affective psychiatric disorders. This may hold especially for borderline personality disorder (BPD), a core emotion dysregulation disorder. Although BPD is strongly associated with sleep disturbances such as insomnia, nightmares and circadian dysrhythmia, research into the role of sleep in BPD remains sparse. In this narrative review, we outline a putative vicious cycle of reciprocal exacerbation of disturbed sleep and emotion dysr

Harriet Tubman's Hypersomnia: Insights from Historical and Medical Perspectives.

Harriet Tubman, a hero of the abolitionist movement and early civil rights advocate, suffered a head injury in childhood and subsequently developed sleep attacks associated with visions that were extensively documented in historical accounts. Her contemporaries perceived these visions together with unpredictable and unavoidable urges to sleep as manifestations of her deep faith, rather than as symptoms of an illness. While religious perspectives remain crucial to understanding Tubman's sleep-related experiences, some may consider them insufficient in view of modern medical advances. We propose
