Dimensions, septation, and pattern of pneumatization of the sphenoidal sinus.

The endoscopic endonasal transsphenoidal approach (EEA) to treat sellar, parasellar, and suprasellar tumours continues to gain increased significance. Due to the close proximity of the sphenoid sinus to the carotid artery and the optic canal, it is very important for surgeons to know the anatomical features and variations of the sphenoid sinus as relevant to EEA. A prospective study of the sphenoid sinus morphology was carried out on the cranial tomographic (CT) scan images of 60 Nigerian adult patients. The CTs were reviewed regarding the different anatomical variations of the sphenoid sinus:

Predictive value of suprasellar extension for intracranial infection after endoscopic transsphenoidal pituitary adenoma resection.

OBJECTIVE: To investigate the relationship between suprasellar extension (SSE) and intracranial infection after endoscopic endonasal transsphenoidal approach (EETA) for pituitary adenoma resection. METHODS: We retrospectively analyzed 94 patients with suprasellar extended pituitary adenoma admitted to the Department of Neurosurgery of the Affiliated Hospital of Guilin Medical College from January 2018 to December 2021. We measured the preoperative magnetic resonance sagittal SSE and collected clinical data and divided the patients into groups according to the presence of postoperative intracr

Endoscopic Anatomical Study of the Glabellar Keyhole Approach.

AIM: To observe the exposure range of a neuroendoscope through the glabellar approach and measure the anatomical parameters to provide a basis for clinical application. MATERIAL AND METHODS: A total of 10 adult cadaveric heads fixed with formalin were dissected by stratified local anatomy and simulated operation. The length of each point was measured from the corresponding anatomical mark of the anterior fossa on the bone window plate and analysed to clarify relevant surgical indications and feasibility to provide an anatomical basis for clinical application. RESULTS: The distance from the l

Spheno-Orbital Meningiomas.

Meningiomas are the most common intracranial brain tumor. Spheno-orbital meningiomas are a rare subtype that originate at the sphenoid wing and characteristically extend to the orbit and surrounding neurovascular structures via bony hyperostosis and soft tissue invasion. This review summarizes early characterizations of spheno-orbital meningiomas, presently understood tumor characteristics, and current management strategies.

Morphometric evaluation and classification of the superior orbital fissure on 3D MDCT images.

The anatomy of the superior orbital fissure is very important because of the spaces it connects, the regions it is adjacent to, and the structures it contains. We aimed to study the width, length and types of the structure, their change according to gender, body sides, and age. The fissure shape has many variations that are not easy to systematize. Therefore, it was aimed to classify them with various reference points determined for the first time, identify their types and determine their incidence. An objective and comprehensive classification was used on 3-dimensional images using multidetec

[Surgical Anatomy of the Cavernous Sinus, Parasellar Region, and Orbit].

The cavernous sinus, para-sellar region, and orbit have intricately intertwined cranial nerves, blood vessels, and dura mater. In surgery, anatomical understanding is very important. Recognizing the location(depth)of the cranial nerves running on the lateral and upper wall of the cavernous sinus is vital and is directly linked to postoperative complications. In addition, understanding the dural ring in the clinoid segment of the internal carotid artery is important. The periosteum on the upper surface of the anterior clinoid is the distal dural ring of the internal carotid artery, and the peri

The endoscopic diving technique in endonasal transsphenoidal surgery: how I do it.

BACKGROUND: The endoscopic diving technique (EDT) is a procedure utilized in endonasal endoscopy, which consists of irrigating the surgical cavity during tumor removal, with a continuous flow of pressurized saline solution. METHODS: The authors provide a detailed step-by-step description of the technique, shedding light on surgical tips and pitfalls. Advantages, disadvantages, and the most useful application of the technique are clarified and showed with a high-quality endoscopic video. CONCLUSION: EDT represents an effective and safe procedure. It allows a wide exploration of the surgical c

Endoscopic Endonasal Versus Microscopic Transsphenoidal Surgery for Recurrent and/or Residual Pituitary Adenomas.

OBJECTIVE: Surgery for recurrent/residual pituitary adenomas is increasingly being performed through endoscopic surgery. Whether this new technology has altered the indications and outcomes of surgery is unknown. We conducted a systematic review and meta-analysis of published studies to compare the indications and outcomes between microscopic and endoscopic approaches. METHODS: A PubMed search was conducted (1985-2015) to identify surgical series of endoscopic endonasal and microscopic transsphenoidal resection of residual or recurrent pituitary adenomas. Data were extracted regarding tumor c

Relations of the pterygoid hamulus and hard palate in children and adults: anatomical implications for the function of the soft palate.

We investigated spatial relations of the pterygoid hamuli to the hard palate on 65 skull bases: 31 disarticulated sphenoidal bones from the newborn up to 9 years of age, 19 skulls of adult skeletons (21-59 age group), and 15 skulls aged 60-100 years. We measured: (a) width of the hard palate in the choanal region, (b) length of the hamulus, (c) inclination of the hamulus from the perpendicular line, and (d) distance between the tips of the contralateral hamuli. The width of the hard palate in the choanal region was smallest in children (mean +/- standard deviation, 21.5 +/- 2.6 mm) compared wi

Anatomical study of the orbitozygomatic transsellar-transcavernous-transclinoidal approach to the basilar artery bifurcation.

OBJECT: An anatomical study in which measurements were obtained was undertaken to demonstrate that the orbitozygomatic transcavernous-transclinoidal approach provides excellent exposure of the trunk of the basilar artery (BA) and its bifurcation. METHODS: Bilateral stepwise dissections were performed on 10 fixed cadaver heads with the aid of x 3 to x 40 magnifications. A frontotemporal craniotomy was made, followed by an orbitozygomatic osteotomy. After the dura mater had been opened, the sylvian fissure was widely separated. The anteromedial triangle of the cavernous sinus was opened to mobi

Hydatid cyst of the pterygopalatine-infratemporal fossa.

Hydatid disease is caused by the parasitic tapeworm. Echinococcus. This parasite in larval stage can thrive in many parts of the body, most commonly in the liver and the lung. Hydatid disease in the head and neck region is rare. An unusual location for hydatid disease in the pterygopalatine fossa-infratemporal fossa is presented. The patient did not have evidence of any other cyst on a ten-year follow-up.
