These results are in accord with the findings of others regarding an increase in whole-body VO2 with hypocapnic alkalosis, but do not support a contributory role of skeletal muscle to the overall increase.

BACKGROUND: Suxamethonium is hydrolysed by butyrylcholinesterase (BChE) and a low BChE activity can result in a prolonged duration of action of suxamethonium. The BChE activity is reduced during pregnancy and postpartum period by up to 33%. However, it can also be reduced by mutations in the BChE gene. In this study, we assessed BChE activity and mutations in the BChE gene in pregnant and postpartum patients with prolonged duration of action of suxamethonium. It was hypothesised that at least 30% of patients with a low BChE activity did not have a mutation in the BChE gene.

The myotonic response of the masseter muscles following various doses of suxamethonium was measured using a unique myotonometer in 50 apparently healthy patients, the majority of whom showed a myotonic response which lasted less than 100 s. A maximum increase in masseter tone was found to be greater than 1 kg in five patients, and 12 patients developed a maximum tone of greater than 500 g. A careful preoperative history can be helpful in detecting problems with previous anesthetic experiences. However, it has been found that in the patients who have developed a complete syndrome of malignant hyperthermia (MH), 30% had prior uneventful general anesthesia. It is therefore of extreme importance when delivering anesthesia to recognize the signs of MH early in order to initiate correct procedures to prevent the development of a fulminating crisis of MH.
