Artificial Intelligence in Videofluoroscopy Swallow Study Analysis: A Comprehensive Review.

Videofluoroscopic Swallowing Study (VFSS) is considered the gold standard for diagnosing swallowing disorders, or dysphagia. However, the interpretation of VFSS is susceptible to human bias and subjectivity, resulting in significant inter- and intra-patient variability. In this context, artificial intelligence (AI) has emerged as a potentially valuable tool for physicians. This study reviews state-of-the-art research utilizing AI to analyze VFSS for the assessment of swallowing disorders and to support clinical decision-making. Our comprehensive analysis highlights substantial progress in area

The Efficacy of Outpatient Swallowing Therapy: A Retrospective Longitudinal Cohort Study.

Oropharyngeal dysphagia is an independent predictor of poor outcomes in many health conditions and can be targeted directly through swallowing therapy. This study aims to explore the outcomes of outpatient swallowing therapy in clinical practice across a diverse cohort of patients. This was a retrospective, single-site longitudinal cohort study. Patients 18 years or older with dysphagia who completed 7-8 weeks of outpatient swallowing therapy with a pre- and post-treatment videofluoroscopy were included. Therapy employed a progressive swallowing exercise regimen based on the Systematic Exercis

Swallowing and ventilation patterns in stable COPD patients: An observational study.

Our study aimed to investigate swallowing coordination by analyzing ventilatory patterns during of solids and liquids food intakes. Twenty-one patients with severe to very severe stable COPD (GOLD III and IV) underwent ventilation and swallowing recordings while performing standardized swallowing tasks. The results revealed that the expiratory-expiratory (EE) swallowing pattern was predominant, accounting for 80 % of swallows, with no significant differences between solid and liquid swallows. Non-EE patterns occurred in an average of 20.68 % of swallows per patient. Our results demonstrated an…

Swallowing Evaluation in Post-COVID-19 Patients with Oropharyngeal Dysphagia.

Oropharyngeal dysphagia (OD) is a prevalent issue in hospitalized COVID-19 patients. This study aimed to determine swallowing abnormalities in post-COVID-19 patients with OD and to determine the potential risk factors of aspiration in patients who have recovered from COVID-19. Screening for OD was done for 310 patients who were discharged from the main university isolation hospital during the study period. A longitudinal descriptive study was carried out on 127 adult post-COVID-19 patients between the ages of 24 and 65 years who failed OD screening at the time of discharge. Instrumental swallo

"A hard pill to swallow": a case series of chlorpromazine-induced priapism.

Priapism is a rare but potentially serious adverse effect of several medications including chlorpromazine, which is commonly used in the treatment of refractory migraine. We describe three cases of ischaemic priapism occurring in men following intravenous chlorpromazine administration for migraine relief. These cases highlight an important but under-recognised complication that can result in long-term erectile dysfunction if not promptly managed. Clinicians should maintain a high index of suspicion for this adverse effect and ensure patients are appropriately counselled regarding the need for

Faciobrachial dystonic seizure can be triggered by swallowing in LGI1 encephalitis.

BACKGROUND: Leucine-rich glioma inactivated 1 (LGI1) antibodies are associated with a limbic encephalitis syndrome characterized by faciobrachial dystonic seizures (FBDSs) that is responsive to immunotherapy. The precise pathophysiology, neural origins, and underlying mechanisms of FBDS remain incompletely understood. CASE REPORT: We present a 54-year-old Chinese female patient with LGI1-related isolated FBDS triggered by swallowing. DISCUSSION: FBDS in LGI1-antibody encephalitis exhibit overlapping features with both paroxysmal kinesigenic dyskinesia (PKD) and epileptic motor seizures, both

Swallowing and feeding of young children on high-flow oxygen therapy.

BACKGROUND: Oral feeding practices of young patients on high-flow oxygen (HFO2) have been controversial. Limited literature exists on this topic, but new studies suggest introducing oral feeds. OBJECTIVE: This study aims to describe the changes in swallowing and feeding of a group of young children on HFO2. METHOD: Twelve participants (mean age 34.17 months [s.d. = 3.97]) on HFO2 were assessed clinically at the bedside using the Schedule of Oral Motor Assessment. Assessments were conducted twice to determine the change in characteristics: upon approval from the managing doctor when respirato

A Variable Stiffness Bioinspired Swallowing Gripper Based on Particle Jamming.

As the chameleon tongue swallows the food, it wraps the entrapped meat around the food, ensuring that it is completely enclosed and preventing it from falling off. Inspired by swallow behavior, this article introduces the design, manufacture, modeling, and experimentation of a variable stiffness swallowing gripper (VSSG). The VSSG is comprised of an intimal membrane, an adventitial membrane, and an internal medium of particles and liquid water. This gripper integrates swallowing behavior with a particle jamming mechanism, exhibiting both soft and rigid state. In the soft state, it gently swall…

Functional involvement of the sternohyoid muscle during breathing and swallowing in rats.

The sternohyoid muscle depresses the hyoid bone, but it is unclear whether the muscle contributes to respiratory and swallowing mechanisms. This study aimed to clarify whether the sternohyoid muscle participates in the respiration and swallowing reflex and how the activity is modulated in two conditions: with airway stenosis and with a fixed sternohyoid muscle length. Electromyographic activity in the sternohyoid, digastric, thyrohyoid, and diaphragm muscles was recorded in anesthetized rats. The sternohyoid muscle activity was observed in the inspiratory phase and during swallowing, and was w

Revealing Goal-Directed Neural Control of the Pharyngeal Phase of Swallowing.

Swallowing is considered a three-phase mechanism involving the oral, pharyngeal, and esophageal phases. The pharyngeal phase relies on highly coordinated movements in the pharynx and larynx to move food through the aerodigestive crossing. While the brainstem has been identified as the primary control center for the pharyngeal phase of swallowing, existing evidence suggests that the higher brain regions can contribute to controlling the pharyngeal phase of swallowing to match the motor response to the current context and task at hand. This suggests that the pharyngeal phase of swallowing cannot

Diagnostic Validity of Clinical Observations for Detecting Physiologic Swallowing Impairment.

A clinical swallow evaluation (CSE) is a noninvasive and indirect assessment of the anatomical and physiological integrity of the swallowing mechanism in a natural setting. A CSE goes beyond a screening, a dichotomized indicator of dysphagia risk, by comprehensively examining the patient's swallowing through gaining sensory and motor information of oral and pharyngeal function. Information obtained from CSEs in combination with medical, social, and environmental patient related factors allow clinicians to make critical decisions about patients' health and quality of life. It is essential that

The Effects of Swallowing Exercises in Healthy Older Adults: A Systematic Review.

Dysphagia, common among older adults, leads to significant morbidity and potential mortality. The role of preventative exercises in healthy, community-dwelling elderly people remains to be understood. In this systematic review, we aimed to understand whether preemptive exercise programs can improve swallowing in healthy older adults. Articles published from 1990 to 2023 were included and identified through searches of MEDLINE, EMBASE and Cochrane databases by a medical librarian using the keywords "presbyphagia" and" aging" and "dysphagia" and "management", as well as "therapy" and "treatment"
