The warning bell of Dolutegravir resistance: rethinking HIV service models in Africa.

The emergence of resistance to Dolutegravir (DTG)-the cornerstone of first-line antiretroviral therapy-marks a critical turning point in the fight against HIV in Africa. Once considered unlikely due to DTG's high genetic barrier, resistance is now increasingly reported, raising urgent questions about current HIV service models. This Perspective argues that the spread of resistance must be seen not only as a clinical threat but as a warning signal for the broader health system, especially in the wake of post-COVID transformations that have accelerated decentralization, task-shifting, and self-m

Tryptophan hydroxylase 2 deficiency alters autism-related behavioural phenotypes in rats.

Serotonin is a crucial neurotransmitter implicated in various physiological processes. Dysregulation of the serotonergic system, particularly in early life, may be associated with neuropsychiatric disorders, including autism spectrum disorder. This study utilises a rat model lacking the central serotonin-synthesising enzyme tryptophan hydroxylase 2 (TPH2) to investigate the effects of brain serotonin depletion on core autism-related behaviours. The animals' social behaviour was assessed using social interaction and resident-intruder tests, along with ultrasonic vocalisation analysis to evaluat

From implementation to sustainability? A 5-year follow-up on a physical activity counseling program in Uganda for people with HIV and mental health conditions.

Physical activity (PA) interventions benefit people living with HIV and co-morbid mental health conditions, but sustaining them in low-resource settings remains difficult. This study explored the long-term sustainability of a multi-stakeholder PA counseling program in a rural Ugandan fishing community, five years post-implementation. The program integrated a PA counseling into routine HIV care, combining motivational interviewing and self-determination theory. It began with sessions led by health care providers and later transitioned to lay-led group sessions that focused on goal-setting, iden

"O for KP": an evidence review on recentering HIV prevention and PrEP delivery for key populations and the underserved.

PURPOSE OF REVIEW: Despite the proven efficacy of preexposure prophylaxis (PrEP), global coverage remains far below targets, with the greatest gaps among key and underserved populations. This review is timely as it highlights how inequitable access undermines progress toward ending AIDS by 2030 and introduces the "O for KP" framework - a preferential option for key populations and the underserved . This equity-focused principle redefines program success by how well services reach and sustain those most structurally excluded. RECENT FINDINGS: We synthesized evidence on persistent barriers face

Acute rheumatic fever and rheumatic heart disease: updates in diagnosis and treatment.

PURPOSE OF REVIEW: To summarize the latest developments in rheumatic fever and rheumatic heart disease (RHD) prevention, (early) diagnosis, and treatment. RECENT FINDINGS: The revised Jones criteria have demonstrated increased sensitivity and specificity for rheumatic fever diagnosis in high-risk populations. The management of rheumatic fever remains symptom-based, with no treatment options proven to alter the disease course or prevent chronic RHD. The revised World Heart Federation (WHF) guidelines for the echocardiographic diagnosis of RHD encourage task-shifting of RHD screening, extending

Executive function predictors of science achievement in middle-school students.

Cognitive flexibility as measured by the Wisconsin Card Sort Task (WCST) has long been associated with frontal lobe function. More recently, this construct has been associated with executive function (EF), which shares overlapping neural correlates. Here, we investigate the relationship between EF, cognitive flexibility, and science achievement in adolescents. This is important because there are fewer educational neuroscience studies of scientific reasoning than of other academically relevant forms of cognition (i.e., mathematical thinking and language understanding). Eighth grade students at

High inter-rater reliability between physicians and nurses utilising modified Downes' scores in preterm respiratory distress.

AIM: To assess the inter-rater reliability of modified Downes' scores assigned by physicians and nurses in the Ethiopian Neonatal Network and to calculate the concordance of score-based treatment for preterm infants with respiratory distress. METHODS: We included preterm infants admitted from June 2020 to July 2021 to four tertiary neonatal intensive care units (NICUs) of the Ethiopian Neonatal Network that presented with respiratory distress. We calculated the kappa statistic to determine the nurse and physician correlation for each component of the modified Downes' score and total score on

Auditory Beat Stimulation Modulates Memory-Related Single-Neuron Activity in the Human Medial Temporal Lobe.

Auditory beats are amplitude-modulated signals (monaural beats) or signals that subjectively cause the perception of an amplitude modulation (binaural beats). We investigated the effects of monaural and binaural 5 Hz beat stimulation on neural activity and memory performance in neurosurgical patients performing an associative recognition task. Previously, we had reported that these beat stimulation conditions modulated memory performance in opposite directions. Here, we analyzed data from a patient subgroup, in which microwires were implanted in the amygdala, hippocampus, entorhinal cortex and

Behavioral alterations are associated with vitamin B12 deficiency in the transcobalamin receptor/CD320 KO mouse.

Vitamin B12 (cobalamin) deficiency is prevalent worldwide and causes megaloblastic anemia and neurologic deficits. While the anemia can be treated, the neurologic deficits can become refractive to treatment as the disease progresses. Therefore, timely intervention is critical for a favorable outcome. Moreover, the metabolic basis for the neuro-pathologic changes and the role of cobalamin deficiency in the pathology still remains unexplained. Using a transcobalamin receptor / CD320 knockout mouse that lacks the receptor for cellular uptake of transcobalamin bound cobalamin, we aimed to determin

Revisiting attentional processing of non-emotional cues in social anxiety: A specific impairment for the orienting network of attention.

People with social anxiety disorder (SAD) exhibit an attentional bias for threat (AB). Nevertheless, the focus on AB for emotional stimuli has led to neglect the exploration of basic attention deficits for non-emotional material among SAD patients. This study aimed to investigate the integrity of the attentional system in SAD. The Attention Network Test was used to precisely explore attentional deficits, and centrally the differential deficit across the three attentional networks, namely alerting (allowing to achieve and maintain a state of alertness), orienting (allowing to select information

Decision-making and cognitive abilities: A review of associations between Iowa Gambling Task performance, executive functions, and intelligence.

The Iowa Gambling Task (IGT) has been used to study decision-making differences in many different clinical and developmental samples. It has been suggested that IGT performance captures abilities that are separable from cognitive abilities, including executive functions and intelligence. The purpose of the current review was to examine studies that have explicitly examined the relationship between IGT performance and these cognitive abilities. We included 43 studies that reported correlational analyses with IGT performance, including measures of inhibition, working memory, and set-shifting as

Subtle cognitive deficits after cerebellar infarcts.

The role of the cerebellum in cognitive functions has been under debate. We investigated the neuropsychological functioning of patients with cerebellar lesions (infarcts) and evaluated the significance of laterality in cognitive symptoms. Twenty-six patients with exclusive cerebellar lesions as verified by clinical and neuroradiological findings underwent a neuropsychological assessment at the acute stage and at 3 months. Their performance was compared with 14 controls, also assessed twice. The focus was on four domains: visuospatial/motor functions, episodic memory, working memory and attenti

Computational Characterization of Decision Making During Trans-saccadic Visual Perception.

When viewing the environment, humans execute fast sequential saccadic eye movements that disrupt the flow of visual information, yet still preserve stability in their visual percept. At the time of the saccade subjects often experience saccadic suppression of displacement-a failure to notice changes in the location of visual objects slightly shifted during the eye movement. Although well studied, the weighing of sensory information and the extent to which this integration influences visual perceptual judgments is still poorly characterized and understood. Here we used a drift diffusion modelin

Internally-represented space and its mirror-reversed image of the visuospatial representation: A possible association.

The cognitive capacity for number representation is thought to be a functional isomorphism of space representation. Numbers are represented in a left-to-right-oriented mental number line and hemispatial neglect patients consistently demonstrate rightward midline shift of visuospace, the internal space and number representation. However, patients with pathologic pain in one limb showed a negative correlation between midline shift of the visuospace and number representation. The purpose of the present study is to ascertain whether such dissociation in accessing space and number representation is

Management of Complicated Appendicitis in the Pediatric Population: When Surgery Doesn't Cut It.

The management of complicated appendicitis in children has evolved significantly over the last century. What initially was a surgeon's dilemma is becoming the interventional radiologist's task because image-guided percutaneous drainage of abscesses from a ruptured appendix obviates the need for urgent surgery and allows for selective interval appendectomy at the surgeon's discretion (versus conservative nonoperative management in selected cases). This paradigm shift places the onus on the interventional radiologist to recognize when the procedure is emergently indicated and to be cognizant of

[Neuropsychologic changes in Parkinson syndrome].

In Parkinson's disease various cognitive deficits occur of which impairments of perception, memory, speech, vigilance, reaction-time, and shifting aptitude in cognitive tasks can be distinguished. Also depression is frequently observed. Personality changes are discussed originating from organic mental factors, the experience of the disease, and possible premorbid personality traits. Mental processing is probably slowed down in Parkinson's disease (bradyphrenia). However, it is not yet clarified in which cognitive categories bradyphrenia may play a role.

Assessment of Occupational Exposure to Airborne Phenol and Biological Monitoring of Accumulation Trends.

Introduction: This study investigates the relationship between occupational exposure to airborne phenol and its biological accumulation in chemical plant workers, with a particular focus on urinary phenol levels. It also explores the influence of job roles, employment duration, protective equipment use, and personal lifestyle habits on internal exposure. Methods: A cohort of bisphenol A (BPA) manufacturing workers was classified based on job tasks and exposure characteristics. Airborne phenol concentrations were measured using standard occupational hygiene methods, and urinary phenol levels we

Effects of passive ankle exoskeletons on neuromuscular function during exaggerated standing sway.

Wearable robotic exoskeletons designed to assist human movement should integrate with the neuromusculoskeletal system. This means assisting movement while not perturbing motor control. We sought to test if passive ankle exoskeletons, which have been shown to successfully assist human gait, affect neuromuscular control of an exaggerated anterior-posterior standing sway task. Participants actively swayed while wearing an ankle exoskeleton that provided 0, 42 or 85 Nm rad-1 of additional stiffness to the ankle joint in resistance to dorsiflexion. Sway amplitude was controlled via biofeedback to e
