Fifty cadaver hands were dissected to better delineate the extensor tendon anatomy to the little finger. The extensor digitorum communis was present in 35. Of 15 hands without an extensor digitorum communis, 12 had a junctura present. Three hands lacked both extensor digitorum communis and juncturae. Transfer of the extensor digiti minimi tendon in these hands could cause loss of extension to the little finger. Ten hands had a direct attachment of the extensor digiti minimi tendon on the abductor tubercle. Twenty-two hands had either an attachment of the extensor digiti minimi on the abductor

Open ruptures of the Achilles tendon with loss of the tendon and the overlying skin are very difficult to treat. They pose the problems of (1) combating infection, (2) providing soft-tissue cover, and (3) bridging the gap in the tendon. They are generally managed as multi-staged procedures. Once the infection is brought under control with debridement and antibiotics, skin cover is provided by local transposition flaps, distant pedicle flaps, or free-tissue transfer with microvascular anastomoses. The tendon itself is repaired later by one of the conventional techniques. More recently, composit

Extensor tenosynovitis is a common presenting upper extremity problem and, unless it resolves with medical management, preventative tenosynovectomy is indicated to prevent tendon rupture. When a rupture has occurred, tendon reconstruction with either a transfer or a graft has a reasonable chance of restoring function as long as the number of tendons involved is limited. Rupture of a single extensor tendon requires surgical treatment to eliminate the cause and prevent further damage, as well as repairing the injured tendon. It is important to give consideration to associated wrist and MP joint

Ten cases of closed extensor tendon rupture at the musculotendinous junction are reported. In five patients the rupture occurred at work, and in five during athletic activities. In each instance, indirect tendon injury resulted from a strong stretching force applied to a contracting muscle. In one case, rupture of the extensor pollicis longus tendon occurred proximal to the extensor retinaculum. In eight cases of complete tendon rupture direct repair was impossible. Of these eight patients, five were treated by side-to-side juncture and three by tendon transfer. Two patients with incomplete ru…

The further development of minimally invasive techniques in recent years, enables adequate intraoperative visualization, mobilization and stable reconstruction even of larger subscapularis tears resulting in good to excellent clinical and structural results. Chronic tears of the subscapularis tendon with high grade muscle atrophy and advanced fatty infiltration can be treated with a muscle tendon transfer (e.g. pectoralis major or latissimus dorsi transfer). If pseudoparalysis and/or signs of anterosuperior decentration of the humeral head or defect arthropathy are present, in most cases a rev…

BACKGROUND: Tendon avulsion at the musculotendinous junction caused by digit avulsion amputation is still a challenging problem. We aimed to compare the tendon functional outcomes of two repair methods for tendon avulsion rupture at the musculotendinous junction in patients with complete thumb avulsion amputations.
METHODS: A retrospective study was performed to evaluate patients with complete thumb avulsion amputations whose tendons were repaired through reattachment to muscle (group I) or tendon transfer (group II) between July 2008 and October 2019. Outcomes of total range of motion, pinch…

PURPOSE: Although flatfoot is a widespread human condition, historical medical texts and ancient illustrations on this deformity are extremely rare. Nowadays, doubts regarding its management remain unsolved. This historical review aims to identify the presence of pes planus since the prehistoric era and examine the treatments proposed over the centuries up to the present.
METHOD: For this propose, we performed an extensive electronic search of the relevant literature, complemented by a manual search of additional sources from archaeological to artistic, literary, historical, and scientific ac

The long Z-osteotomy (scarf) has proven to be an effective procedure for correction of the metatarsus primus varus component of the hallux abducto valgus deformity. An historical review and technical considerations of the procedure are described. Although technically demanding, it offers the advantages of superior strength with internal fixation, early range of motion, and early weightbearing. An uncommon but challenging intraoperative complication of the procedure is troughing (channeling). The author describes a technique that involves transfer of the adductor hallucis tendon through the hor

Twelve consecutive patients with long thoracic nerve palsy, who underwent transfer of the pectoralis major muscle for dynamic stabilization of the scapula, were reviewed. Direct transfer of the tendon to the lateral margin of the inferior scapular angle was performed with a bony chip from the tendon's insertion. All patients were followed up clinically by evaluation of the Constant score and radiographically with magnetic resonance imaging for an average of 92.5 months (range, 60-136 months). The mean Constant score improved from 41 to 85.4 points. Mean active flexion increased from 89 degrees

BACKGROUND: Surgery of the inferior oblique muscle (IO) has undergone significant changes in the past 160 years. Many investigators have contributed to our understanding of the action of this muscle and to the surgical options that have developed. This article reviews the history of IO surgery with particular emphasis on the anterior transposition procedure.
METHODS: Anatomic and physiologic studies on the neurofibrovascular bundle of the IO are presented.
RESULTS: The ligamentous structure of the neurofibrovascular bundle of the IO provides the ancillary origin for the posterior temporal fi

The theoretic disadvantage of dynamic tendon transfers is the perception that they are "more complex" than static procedures. The latter may provide a simple solution to claw deformity in a subset of patients; however, they completely disregard the disability associated with loss of the intrinsic musculature. Dynamic procedures reconstruct in part the deficient intrinsic forces and are thus capable of correcting the deformity and some disabilities associated with ulnar nerve palsy. In our practice, we have consistently achieved reasonable correction of claw deformity and improvement in tendon …

CASE: While working in a factory, a thirty-five-year-old man injured his right forearm and was then diagnosed with acute compartment syndrome. Ruptures of the flexor and extensor tendons were revealed via single volar and dorsal incisions. End-to-side transfer with use of the remaining extensor tendons was performed; the end of the extensor indicis proprius tendon was sutured to the side of the extensor digitorum communis tendon of the middle finger, and the extensor pollicis longus tendon was sutured to the extensor pollicis brevis tendon.
CONCLUSION: In cases of high-energy injuries, associ

BACKGROUND: Tendon transfers are established techniques to regain external rotation mobility in patients with an irreparable, posterosuperior massive rotator cuff tear (MRCT). Posterosuperior MRCT with intact teres minor (type D MRCT) can lead to excessive teres minor loading to maintain external rotation. We hypothesize that tendon transfers are effective in relieving teres minor loading in type D MRCTs. Our aim was to biomechanically assess muscle synergism with latissimus dorsi (LD transfer) and lower trapezius (LT transfer) tendon transfer during external rotation at different abduction he…

Nerve grafting, tendon transfer and joint fusion are routinely used to improve the upper limb function in patients with brachial plexus palsies. Newer techniques have been developed that provide additional options for reconstruction. Nerve transfer is a tool for restoring upper limb function in total root avulsions where nerve grafting is not possible. In partial brachial plexus injuries, nerve transfers can greatly improve shoulder, elbow, wrist and hand function. Intraoperative electrical stimulation can be used to diagnose precisely which nerve is injured and to choose which nerve fascicles…

Muscle and tendon variations in the upper and lower extremities are common but often go unnoticed due to their asymptomatic nature, typicall … limb. From a clinical perspective, recognizing these variations is essential for procedures such as tendon transfers, where an accessory muscle or a tendon could either complicate or facilitate the tendon transfers within the anatomical landscape.
