Contact between the coracoacromial arch and the rotator cuff tendons in nonpathologic situations: a cadaveric study.

HYPOTHESIS: A recent ultrasound study has shown that impingement phenomenon was observed in healthy shoulders. We hypothesized that nonpathologic contact beneath the coracoacromial arch occurs in normal shoulders. MATERIALS AND METHODS: Seven fresh-frozen cadaveric shoulders were studied. Each specimen was attached to a custom-designed shoulder-positioning device. A 22-N force was applied to the humeral head to keep it centered in the glenoid fossa. Contact pressure beneath the coracoacromial arch was measured by a flexible force sensor during flexion, abduction, internal and external rotatio

Anterior and nasal transposition of the inferior oblique muscles in patients with missing superior oblique tendons.

INTRODUCTION: Patients with missing superior oblique (SO) tendons present with overelevation/underdepression in adduction. Unilateral cases often exhibit abnormal head postures, whereas in bilateral cases, there may be a marked V-pattern with upgaze exotropia. These patients may have craniosynostosis. METHODS: Nine children with unilateral (n = 2) or bilateral (n = 7) absent SO tendons underwent anterior and nasal transposition of the inferior oblique (IO) muscles, some in combination with horizontal rectus recession for horizontal strabismus. They were evaluated 6 to 46 months postoperativel…

Ruptures of flexor tendons at the wrist as a complication of fracture of the distal radius.

We report two cases of rupture of flexor tendons after fracture of the distal radius. The first case was a rupture of the flexor digitorum profundus and superficialis tendon to the index finger that happened 20 years after the fracture. The second was a rupture of the flexor pollicis longus tendon that occurred two years after, and the flexor profundus tendon to the index finger that occurred four years after the fracture. In the first case, the ruptures were caused by the bony protuberance of the radius after long interval without interference of the ulnar head.

Autonomic innervation of tendons, ligaments and joint capsules. A morphologic and quantitative study in the rat.

We analyzed the neuronal occurrence of autonomic transmitters; noradrenaline (NA), neuropeptide Y (NPY) and vasoactive intestinal polypeptid … that the concentration of NPY compared to VIP was 15-times higher in ligaments and twice as high in tendons and capsules. The differences noted may reflect a difference in vulnerability to degenerative conditions. In pathological conditions, dysregulation of autonomic transmitters in hypovascularized tissues subjected to repetitive mechanical load may con …

Involvement of multinucleated giant cells synthesizing cathepsin K in calcified tendinitis of the rotator cuff tendons.

OBJECTIVES: Calcified tendinitis of the shoulder joint is a common painful condition. Resorption of the calcium deposits is one of the key events in the pathogenesis of this disease. The aim of this study was to examine whether the multinucleated giant cells that appear in this condition have osteoclast phenotypes. METHODS: Immunohistochemical and RNA in situ hybridization analysis of cathepsin K, a marker for osteoclasts, was performed in human surgical samples. RESULTS: The multinucleated cells located near the calcium deposits were positive for cathepsin K protein and mRNA. Reverse transc

Delayed sequential ruptures of the index and thumb flexor tendons due to an occult scaphoid nonunion.

A case of sequential spontaneous loss of flexion in the thumb and index finger over a 10 year period is reported. Previously unrecognized carpal collapse as a consequence of a longstanding scaphoid nonunion was identified as the cause. Fusion of the interphalangeal joint of the thumb and suture of the index profundus tendon to the middle profundus tendon resulted in a good outcome.

Rheumatoid arthritis: evaluation of wrist extensor tendons with clinical examination versus MR imaging--a preliminary report.

To determine whether magnetic resonance (MR) imaging can improve assessment of the extensor tendons of the wrist and their propensity to rupture, the dorsal tendons in 23 wrists in 20 patients with rheumatoid arthritis were clinically assessed and then examined with MR imaging. Clinical and MR imaging evaluations were performed blindly and independently. The six extensor tendon compartments were assessed with MR imaging for presence of pannus, synovial effusion, and tendon involvement or rupture. Clinical scores of systemic disease, dorsal tendon sheath swelling, and risk of tendon rupture ran…

Simultaneous rupture of the tibialis posterior and flexor digitorum longus tendons in a closed tibial fracture.

An interposition of ruptured tendons of the tibialis posterior and flexor digitorum longus occurred between the lower third tibial fracture fragments in a closed tibial fracture in a 26-year-old man. The tendon ruptures were not diagnosed preoperatively but were recognized at the time of open reduction. The tendons were repaired and the fracture internally fixed. Six months postoperatively, the patient had a 10 degree dorsal extension deficit in the ankle joint, the motion was painless, and the strength of the posterior tibial compartment muscles was grade 5.

Effect of antigen-induced synovitis on the extensor tendons within the retinacular compartment of the dog.

Antigen-induced proliferative synovitis was produced within the extensor retinacular compartment of one forepaw of young littermate dogs. The extensor tendons within the synovitis compartment were compared with the tendons in control compartments of the opposite forepaw. At 7 days following challenge, the tendons from the proliferative synovitis compartment showed increased uptake of [3H]proline at 5 min after pulse labeling. At 7, 14, and 28 days following challenge, the tendons within the proliferative synovitis compartment showed a significant increase in metabolic activity compared with co…

Pathology of the skeleton and tendons of broiler chickens reared to roaster weights. I. Crippled chickens.

The incidence of cripples among broiler chickens kept until 74 days of age was greatest in chickens fed ad libitum and kept in batteries. The incidence was lower in chickens kept in floor pens and was reduced by feed restriction. Crippling was due to long-bone distortion, rupture of the gastrocnemius tendon, and spondylolisthesis. Long-bone distortion was the most common defect and could be divided into valgus and varus deformation. The former was more important in chickens less than 7 weeks of age, and the latter was more important in older chickens. Tibial dyschondroplasia was found in some

Two-step reconstruction of the flexor tendons (Hunter's technique) in the treatment of fingers "en crochet".

The authors used Hunter's technique to treat 16 cases of fixed-flexion deformity of the finger (with irreducible flexion of the joint greater then 70 degrees), secondary to lesions of the flexor tendons. The flexion deformity was corrected in 11 of the 16 cases, and active movement greater than 70 degrees was obtained in only 7 cases. Technical difficulties (particularly cutaneous problems), frequent complications, prolonged reeducation are the reasons that the indications for its use are exceptional, and why there is need for excellent cooperation by the patient.

"Reconstruction of segmental defect of flexor tendons of the wrist and hand using extensor digitorum longus".

Plastic and hand surgeons rely on traditional tendon grafts, but severe bilateral multi-level trauma may render these unusable. We report a 30-year-old man with extensive forearm loss reconstructed using the entire extensor digitorum longus tendon, nerve and vein grafts, and an ALT flap, demonstrating EDL's versatility and minimal donor morbidity.

Functional Reconstruction of Thumb with Tendons from an Amputated Finger in Median Nerve Territory-Oriented Macrodactyly.

Nerve territory-oriented macrodactyly (NTOM) of the hand, particularly involving the median nerve, results in severe enlargement and dysfunction of the thumb and index finger, compromising grasp and pinch functions. We present two paediatric cases managed by ray amputation of the index finger and debulking of the thumb, with a novel approach utilizing tendons from the amputated finger to restore thumb flexion, abduction, and opposition in a single-stage procedure. This technique avoids additional donor-site morbidity and optimizes the use of available structures. Both patients achieved improve…

[Selective in vivo force measurement of superficial and deep finger flexor tendons depending on wrist position].

There are different options for further treatment following flexor tendon injuries of the hand. These can be divided into passive and active dynamic treatment regimens. The selection of the optimal further treatment depends primarily on the biomechanically acting forces during postoperative finger exercises. Quantitative biomechanical studies are therefore required. The present experiment examined which forces act on the superficial and deep finger flexor tendons in different wrist positions. A measuring device was developed with the help of which both the total forces of the fingers and the i…

Ossification of the posterior longitudinal ligament is linked to heterotopic ossification of the ankle/foot tendons.

INTRODUCTION: Systemic osteogenesis has been speculated to be involved in the pathogenesis of ossification of the posterior longitudinal ligament (OPLL). Our purpose was to compare the radiologic prevalence and severity of heterotopic ossification in foot tendons of Japanese patients with OPLL and to determine their association with systemic heterotopic ossification. MATERIALS AND METHODS: Clinical and radiographic data of 114 patients with OPLL were collected from 2020 to 2022. Control data were extracted from a medical database of 362 patients with ankle radiographs. Achilles and plantar te…

Multiaxial 3D MRI of the Ankle: Advanced High-Resolution Visualization of Ligaments, Tendons, and Articular Cartilage.

MRI is a valuable tool for diagnosing a broad spectrum of acute and chronic ankle disorders, including ligament tears, tendinopathy, and osteochondral lesions. Traditional two-dimensional (2D) MRI provides a high image signal and contrast of anatomic structures for accurately characterizing articular cartilage, bone marrow, synovium, ligaments, tendons, and nerves. However, 2D MRI limitations are thick slices and fixed slice orientations. In clinical practice, 2D MRI is limited to 2 to 3 mm slice thickness, which can cause blurred contours of oblique structures due to volume averaging effects …

Mapping Origins of Tendons on the Medial Epicondyle to Improve Treatment of Medial Epicondylitis: Anatomical Study.

PURPOSE: Medial epicondylitis is a tendinosis found commonly in throwing and golfing athletes. Although there are choices for nonsurgical treatments, when these fail, surgical intervention can be considered. When surgical treatment is performed, the objective is to debride the diseased tissue from the epicondyle. The purpose of this study was to clarify the locations and size of the common flexor tendons and medial collateral ligament (MCL) relative to each other and to the posterior ridge of the medial epicondyle. METHODS: The common flexor tendons and MCL were dissected and reflected their …
