Superficial infections include paronychia, cellulitis and lymphangitis. Grave infections include felon, purulent tenosynovitis, thenar infections, septic arthritis and human bites. Treatment by incision and drainage, moist heat, elevation, splinting and specific antibiotics or other agents applies to most hand infections. When pus is found, adequate drainage usually gives good results. Without drainage, pressure necrosis and gangrene often follow. Rest for the infected part and for the patient is as important as any other facet of treatment.

OBJECTIVE: To investigate the agreement between clinical signs (tenderness and/or swelling) in ankles and feet joints and ultrasound findings in patients with rheumatoid arthritis (RA).
METHODS: RA patients with at least 1 tender and/or swollen joint in bilateral ankles and metatarsophalangeal (MTP) joints detected by physical examination were enrolled and underwent ultrasound examination by greyscale (GS) and power Doppler (PD) mode. The agreement between clinical signs and ultrasound-detected inflammation (joint effusion, synovitis, or tenosynovitis) was analyzed.
RESULTS: In the study, 11…

BACKGROUND: Musculoskeletal Ultrasonography (MSUS) is an important tool for the clinical assessment in Juvenile Idiopathic Arthritis (JIA). The objective of this study was to evaluate the reliability of MSUS to detect elementary lesions: synovitis, tenosynovitis, cartilage damage and bone erosions in the wrist and metacarpal (MCP) joints of patients with JIA.
METHODS: Thirty children in various subgroups of JIA according to ILAR criteria, were included in this cross-sectional study. Clinical data including painful, swollen and limited joints were recorded. Five rheumatologist ultrasonographer…

Third-degree atrioventricular (AV) block is characterised by complete dissociation between atrial and ventricular activity. It is commonly caused by degenerative conduction disease, ischaemia, medications, or in this case - an infective process. We report a rare case of third-degree AV block caused by disseminated gonococcal infection (DGI). Rates of Neisseria gonorrhoeae are increasing, and DGI remains an important but often under-recognised complication. While DGI typically presents with tenosynovitis, polyarthralgia, and skin lesions - cardiac involvement is uncommon. A 36-year-old cis-male…

OBJECTIVE: This article reviews the normal anatomy of the extensor tendons of the wrist as well as the clinical presentation and MRI appearances of common tendon abnormalities, such as tears, tenosynovitis, intersection syndromes, and associated or predisposing osseous findings. Treatment options are also discussed.
CONCLUSION: We review the anatomy and normal MRI appearance of the clinically important dorsal extensor tendons of the wrist, in addition to the spectrum of abnormalities associated with these tendons.

A 30 year old woman presented with pain in her right wrist a year after operation for de Quervain's tenosynovitis. Magnetic resonance imaging showed avascular necrosis of the scaphoid without loss of volume. She was instructed to wear a brace on her wrist while working. A year later she was free of symptoms but the scan was unchanged. Avascular necrosis of the scaphoid not associated with injury is extremely rare.

Tendinopathies are some of the most common diagnoses treated by hand surgeons. Diagnoses such as trigger digit, de Quervain tenosynovitis, extensor carpi ulnaris tendinitis, and epicondylitis often resolve with nonoperative treatment and/or a single ambulatory procedure. When symptoms persist or worsen after surgery, patients are disappointed and treatment can be challenging. This article reviews practical points in evaluation of such cases, and surgical options that work in revision scenarios.

Tendinopathies involving the hand and wrist are common. Many are diagnosed easily, and in many cases, the management is straightforward, provided the pathology and principles are understood. Common conditions involving the tendons of the hand and wrist include trigger finger, tenosynovitis of the first through sixth dorsal extensor compartments, and flexor carpi radialis tendonitis. Management strategies include nonsurgical treatments, such as splinting, injection, or therapy, and surgical techniques such as tendon release.

A carpal tunnel release was performed on a patient with recurrent carpal tunnel syndrome and asymptomatic hyperuricaemia with no prior history of gouty arthritis. Intraoperatively, the patient was found to have tenosynovitis without crystals or tophaceous deposits in the carpal tunnel. Postoperatively, the patient developed an acutely inflamed hand, which responded dramatically to anti-gout medications. We report this patient as an initial attack of gout after a carpal tunnel release.

Ultrasound machines with 7.5 to 5.0MHz linear transducers are well suited for rapid and straightforward differentiation of soft tissue swelling in the musculoskeletal system of cattle; in proximal limb regions 3.5MHz convex scanners allow better imaging. The main indications for ultrasonography of the musculoskeletal system in cattle are suspected arthritis, tenosynovitis, bursitis, abscesses, haematomas, diagnosis of muscle and tendon lesions, and generally the evaluation of soft tissue swellings everywhere that cannot be diagnosed based on clinical examination. The examiner starts by obtaini…
