The effect of the die material used in the indirect technique on the fracture resistance of a restored molar.

The aim of this study was to evaluate the effect of different die materials used in the indirect resin composite restorative technique on the fracture resistance and failure mode of restored molars and compare it with the direct resin composite restorative technique. Two flexible die silicone materials for dental models (Die Silicone - Voco and Scan die - Yller) and a type IV dental stone material (Fujirock EP - GC) were evaluated. Sixty third molars were selected and divided into four groups: indirect resin composite restoration - Die silicone (IRCR-DS); indirect resin composite restoration -

Tooth extractions in patients with cancer receiving high-dose antiresorptive medication: a randomized clinical feasibility trial of drug holiday versus drug continuation.

OBJECTIVE: Medication-related osteonecrosis of the jaw (MRONJ) is a serious adverse reaction to high-dose antiresorptive medication (AR) in patients with cancer. A temporary discontinuation of AR (drug holiday) has been suggested to potentially reduce the risk of MRONJ after oral surgery. However, no consensus exists. The aim of the present feasibility trial was to evaluate the impact of a high-dose AR drug holiday in connection with surgical tooth extraction on the development of MRONJ and patient-reported health state. STUDY DESIGN: Patients with cancer receiving high-dose AR were randomize

Effects of different cusp coverage restorations on the fracture resistance of endodontically treated maxillary premolars.

STATEMENT OF PROBLEM: Cusp coverage restorations for the restoration of endodontically treated teeth represent a conservative approach in terms of function and esthetics. However, limited scientific data are available regarding the optimum reduction design and thicknesses. PURPOSE: The purpose of this in vitro study was to evaluate the fracture resistance and fracture patterns of cusp coverage restorations with different cusp reduction designs and reduction thicknesses on endodontically treated maxillary premolars (ETMPs) with mesio-occluso-distal (MOD) cavities. MATERIAL AND METHODS: One hu

Aesthetic resin onlay restorations: 'rationale and methods'.

UNLABELLED: Resin composite restorations have gained increasing popularity over the past two decades. This has been largely driven by a patient-orientated demand for the use of aesthetic restorative materials. It has occurred concomitantly with an improvement in the mechanical properties of available materials, and advances in our knowledge of resin bonding. Onlay restorations are advocated for a plethora of clinical applications. This paper considers the role of adhesive onlay restorations fabricated in resin composite in contemporary restorative practice, including the presentation of two ca

Direct access to equivocal approximal carious lesions.

Approximal carious lesions provide unique diagnostic and restorative challenges for clinicians interested in conserving tooth structure. Direct access reduces diagnostic doubt and facilitates optimal restorative management. The direct access afforded to newly erupted posterior teeth provides an incomparable window of opportunity to the proximal surfaces. Optimal management is limited to the interval between exfoliation and eruption, because access is lost once the erupting adjacent permanent teeth come into contact. Diagnostic and restorative strategies accomplished after creation of direct ac

Comparison of fluoride profiles by SIMS with mineral density of subsurface enamel lesions treated intra-orally with a fluoride-releasing device.

A variety of intra-oral model systems has evolved which allows for the study of remineralization of coronal and/or root-surface lesions following application of topical fluoride (F) agents. The problem of interpretation of the results has led to a variety of analytical methods (i.e., microhardness, F biopsy, microradiography, and polarizing light microscopy), each of which provides important but limited information related to the overall understanding of remineralization. Microhardness measures change in mineral content which is more precisely localized by microradiography and polarized light

Influence of a parafunctional oral habit on root fracture development after trauma to an immature tooth.

Root fractures in immature teeth are rare because the resilience of the alveolar bone is more favorable to the occurrence of luxation. This article reports a case of traumatic injury in an immature permanent tooth that progressed to root fracture, having a parafunctional oral habit as the possible modifying factor of case evolution. A 12-year-old boy presented for treatment complaining of a defective restoration and mild pain on the maxillary right central incisor. The patient had a history of crown fracture in this tooth due to trauma 2 years before. The clinical examination showed healthy gi

Esthetic Smile Design: Limited Orthodontic Therapy to Position Teeth for Minimally Invasive Veneer Preparation.

The standards of dentistry are being elevated, with a greater emphasis being placed on esthetics along with functionality. Minimally invasive dentistry has become an essential component in creating restorations that are functional and have increased longevity. In the case discussed in this article, the patient underwent 9 months of orthodontic therapy to correct her improper overbite and overjet, and the spacing of her dentition so the teeth could be positioned for future minimally invasive restorations. Orthodontic therapy was paramount in positioning the teeth so that the future restorations

Treatment of inflammatory external root resorption resulting from dental avulsion and pulp necrosis: clinical case report.

The aim of this case report was to present a treatment for severe inflammatory external root resorption. The condition developed due to the patient's neglect to seek adequate treatment following replantation of an avulsed maxillary left central incisor. Following diagnosis, treatment consisted of conventional endodontic therapy with calcium hydroxide dressings and definitive filling of the root canal after the resorption was controlled radiographically. A 24-month follow-up showed that the resorption process had stabilized and the patient was free of symptoms. Successful tooth replantation req

Resolution of a titanium implant fracture after a recurrent trauma.

Post-traumatic complications occasionally lead to tooth loss as well as the need for future implants. However, rehabilitation with endosseous osseointegrated implants does not protect the patient from the risk of suffering a new trauma. Implant fracture and the damage of the hexagon are post-traumatic complications that guide the clinician to preparing a more intricate treatment plan. The authors present a clinical case of a recurrent trauma of maxillary implant fracture. The treatment plan was to remove the implants followed by autogenous bone grafting to correct the defect. Two titanium impl

RME-induced root resorption and repair: a computerised 3-D reconstruction.

This study was designed to evaluate the 3-D relation of epithelial rests of Malassez (ERM) to orthodontically induced resorption lacunae in human tooth roots. First premolars were extracted following rapid maxillary expansion, then they were prepared for transmission electron microscopy and sectioned. Serial 1 micron sections were used for the 3-D reconstructions and ultra thin sections were utilized to verify the identification of epithelial cells and blood vessel types. Each serial section was photographed and profiles of the resorption bays, epithelial cells and blood vessels were digitized

Comparative Evaluation of the Anesthetic Efficacy of Needle-Free Anesthesia and Conventional Anesthesia in Patients With Symptomatic Irreversible Pulpitis: A Randomized Clinical Trial.

Introduction Pain is the primary reason for which most of the patients seek endodontic treatment. Local anesthesia is considered to be the most important step in the procedure to reduce the pain. However, the majority of the patients do not cooperate due to the fear of syringe anesthesia. The aim of this clinical trial was to compare the anesthetic efficacy of needle-free anesthesia and conventional anesthesia in patients with symptomatic irreversible pulpitis undergoing root canal therapy. Materials and methods A total of 54 patients were enrolled in the study, and the treatment was administe

Etching Efficacy of Self-Etching Functional Monomers.

Besides chemically interacting with hard tooth tissue, acidic functional monomers of self-etch adhesives should etch the prepared tooth surface to dissolve the smear layer and to provide surface micro-retention. Although the etching efficacy of functional monomers is commonly determined in terms of pH, the pH of adhesives cannot accurately be measured. Better is to measure the hydroxyapatite (HAp)-dissolving capacity, also considering that functional monomers may form monomer-Ca salts. Here, the etching efficacy of 6 functional monomers (GPDM, phenyl-P, MTEGP, 4-META, 6-MHP and 10-MDP) was inv

[Geometric position relationship between gingival, preparation, restoration margins and the cementation (adhesive) layer in the marginal area: new classification and scheme for margin position].

The current low quality of fixed restoration margins is highly correlated with the high incidence of margin-related complications. It is als … between the successive P or R line and free G line and can be measured in the clinical procedure of tooth preparation. The optimal solution is 0-200 µm, that is, RPG200: the P and R are located on the free G line and the distance between these margins is less than 200 µm. This distance not only has the aesthetic effect of invisibility to the naked eye, it also has …

Evaluation of stress generation in core build up-material of mutilated primary teeth: a comparative finite element analysis between BioFlx, stainless steel and zirconia crowns.

Stainless-steel crowns (SSCs) are the most durable restorative option for deciduous teeth, although they are unsightly. However, prefabricated zirconia crowns (ZCs) look more pleasant but require substantial dental preparation. Recently, BioFlx crowns have been introduced as a white-colored alternative to SSCs, providing both flexibility and aesthetics. However, clinical trials have not assessed their oral cavity load-bearing capacity and suitability for repairing severely decayed deciduous teeth. To address this gap, the present study compared the shear stresses generated by BioFlx crowns, ZC
