The management of the extensive malignancy of the hypopharynx generally combines with total laryngectomy. The authors report a case of scanty differentiated papillary thyroid carcinoma, extended to the hypopharynx, with partial involvement of thyroid, cricoid and first tracheal ring cartilage and unilateral vocal fold palsy treated with an "atypical" partial tracheo-laringectomy combined with total circular pharyngectomy. Familial bilateral abductor vocal cord paralysis is a rare entity with few prior descriptions in the literature. Modes of inheritance include X-linked, autosomal recessive, and autosomal dominant. A case of this condition in a father and son is presented. Signs and symptoms at presentation, diagnosis, therapeutic considerations, and modes of inheritance are discussed.

Polysomnography and blood gas measurements during sleep in a young man with craniofacial dysostosis, who presented with an extremely severe sleep apnea syndrome, are reported. Tracheostomy relieved all his complaints permanently, namely hypersomnia, deteriorated intellectual performance, automatic behaviour with hypnagogic hallucinations and snoring. A case of acute epiglottitis in a 79-year-old man is presented describing the airway and anaesthetic management. Early reports of adult epiglottitis advocated observation or tracheostomy for airway management. Recent reports stress early endotracheal intubation. Unlike the paediatric age group adults may present with differing degrees of airway obstruction, epiglottic abscess, and with a more diverse set of medical problems. This can make airway management more variable than in children. A discussion of airway management in adult epiglottitis is presented.

We report the case of a 41-year-old female patient with relapsing polychondritis and severe respiratory involvement.
