High-dose intra-articular versus weight-based peri-articular tranexamic acid in primary total knee arthroplasty: a randomized controlled trial.

BACKGROUND: Intra-articular (IA) and peri-articular (PA) tranexamic acid (TXA) are used to reduce blood loss after total knee arthroplasty (TKA), but comparative evidence is limited because dosing and administration protocols vary. This study compared weight-based PA TXA at 10 mg/kg with fixed-dose IA TXA at 3 g. METHODS: In this prospective, randomized, assessor-blinded trial, 80 patients undergoing primary unilateral TKA were allocated 1:1 to PA TXA at 10 mg/kg or IA TXA at 3 g. The co-primary outcomes were transfusion-adjusted calculated blood loss at 72 h and allogeneic packed red blood c…

Recombinant Activated Factor VIIa Combined With Tranexamic Acid Control Haemostasis in Patients With Severe Factor XI Deficiency Undergoing Urological Procedures.

INTRODUCTION: Patients with severe Factor XI deficiency face unique challenges during urological procedures due to an elevated risk of bleeding, balanced against the use of tranexamic acid (TXA), which may form blood clots. Given the limitations associated with plasma-based treatment, there have been reports of successful and safe off-label use of recombinant activated Factor VII (rFVIIa) in combination with TXA in general surgery. AIM: Report our experience with this approach in urological surgeries involving tissues with heightened fibrinolytic activity. METHODS: A retrospective case serie…

Efficacy and Safety of Topical Tranexamic Acid in Elderly Hip Fractures Undergoing Surgical Treatment: Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Hip fractures are a major health concern, especially among older adults. The conventional treatment for this condition involves surgery, but it carries the risk of excessive blood loss and complications. Tranexamic acid (TXA) has emerged as a possible solution for reducing bleeding during surgery. This study aims to evaluate the safety and efficacy of topical TXA compared to systemic TXA and a placebo in adult patients undergoing surgical treatment for hip fractures. METHODS: The literature was reviewed using 3 databases (PubMed, Scopus, and Google Scholar) for studies published u…

The Efficacy of Tranexamic Acid for Reducing Blood Transfusion Rates in Extracapsular Hip Fractures: A Single-Center Randomized Controlled Trial.

Perioperative allogenic blood transfusions for patients with hip fractures are associated with increased costs and complications. This prospective randomized controlled trial evaluated whether tranexamic acid (TXA) reduces blood transfusion rates and blood loss in extracapsular hip fractures, when administered at the time of hospital admission. Patients with closed intertrochanteric or subtrochanteric femur fractures undergoing intramedullary nailing (n=100) were enrolled from October 2015 to January 2019. On arrival to the hospital, patients were randomized to receive either 1 g of intravenou…

Effect of tranexamic acid in arthroscopic anterior cruciate ligament repair: A systematic review and meta-analysis of randomised clinical trials.

PURPOSE: Perioperative blood loss remains a major challenge to surgeons in anterior cruciate ligament reconstruction (ACLR) surgery, despite of the introduction of minimally invasive approach. Tranexamic acid (TXA) is believed to reduce blood loss, which may minimise the complication of postoperative haemarthrosis with insufficient evidence on its effectiveness in ACLR. The primary aim of this study was to examine the effect of TXA on postoperative blood loss and other secondary outcomes in patients undergoing arthroscopic ACLR surgery. METHOD: PUBMED, EMBASE, MEDLINE and CENTRAL database wer…

Prophylactic use of tranexamic acid for prevention of bleeding during transbronchial lung biopsies - A randomized, double-blind, placebo-controlled trial.

BACKGROUND: Although massive bleeding following transbronchial lung biopsies (TBLB) is rare, even minor hemorrhage may prolong the procedure and result in inadequate sampling. Tranexamic acid (TXA) is an antifibrinolytic agent, which reduces bleeding in numerous scenarios, however, its prophylactic use in mitigating post-TBLB bleeding has not been investigated. We conducted a prospective, randomized, double-blind, placebo-controlled trial to determine whether topical infusion of TXA prior to TBLB would reduce bleeding, shorten procedure duration and increase the number of biopsies obtained. M…
