Coping, drug use, and religiosity/spirituality in relation to HIV serostatus among gay and bisexual men.

Cross-sectional data were collected on a sample of 259 gay and bisexual, male-identified individuals as part of a larger study of the psychosocial functioning of lesbian, gay, bisexual, and transgender persons. Analyses considered differences between HIV-positive and HIV-negative men in relation to active and religious coping strategies; avoidant coping strategies (specifically, illicit drug use); and the psychosocial states of anxiety, hostility, and depression in relation to self-reported HIV-status of the participants. As compared with HIV-negative men, the HIV positive participants indicat…

Gender-Affirming Voice Training Delivered by a Novel Mobile App: Mixed Methods Study.

BACKGROUND: Gender-affirming voice training (GAVT) has reduced gender incongruence for transgender and gender-nonconforming (TGNC) individuals, but remains limited by cost and scarcity of specialized providers. Mobile health (mHealth) is a bridge to these access gaps; yet, few high-quality, clinically informed mHealth apps exist within voice clinical sciences. OBJECTIVE: We previously created a free mobile app (Attuned; Weill Cornell Medicine and Ithaca College) to deliver clinically informed GAVT through structured instructional videos narrated by a speech-language pathologist and a laryngol

Prevalence of mental health outcomes among trans and gender diverse adults: a global systematic review and meta-analysis.

Existing reviews of mental health outcomes among trans and gender diverse (trans)people have been limited by incomplete coverage, restricted or selected samples, afocus on youth populations, and insufficient exploration of heterogeneity. We aimed togenerate pooled prevalence estimates, and examine variation in mental disorders, suicidality, and non-suicidal selfinjury (NSSI) among trans adults globally. Weconducted a systematic review and meta-analysis of studies reporting current orlifetime prevalence of mental disorders, assessed using symptom screeninginstruments with validated general-popul

Interrogating the Effects of Technologies of Pregnancy Surveillance on Queer, Trans and Nonbinary Midwifery Service-Users.

Queer, transgender and nonbinary (QTNB) people access midwifery services, yet little is known about their experiences of midwifery care. Our qualitative study used critical narrative inquiry to examine how QTNB midwifery service-users in Ontario, Canada, experience cisheteronormativity in healthcare. Drawing on in-depth interviews with 14 participants, our analysis examines the effects of biomedicalisation on QTNB people who navigate these systems. Our findings show that QTNB people access and participate in technologies of pregnancy surveillance, which deploy information-seeking practices tha

Structural threat as a theoretical driver of trans health inequities.

Transgender and nonbinary (trans) people in the United States are increasingly exposed to structural conditions that undermine their safety, belonging, and inclusion. Existing theoretical frameworks focus on how anti-trans stigma and discrimination impact health, but structural drivers of trans health inequities remain undertheorized. This paper introduces Structural Threat to explain how cultural, legal, administrative, and institutional systems destabilize and withdraw social safety and protection for trans communities. Structural threat places trans people in a constant state of uncertainty

Assessing and Addressing Cardiovascular Health in LGBTQ Adults: A Scientific Statement From the American Heart Association.

There is mounting evidence that lesbian, gay, bisexual, transgender, and queer or questioning (LGBTQ) adults experience disparities across several cardiovascular risk factors compared with their cisgender heterosexual peers. These disparities are posited to be driven primarily by exposure to psychosocial stressors across the life span. This American Heart Association scientific statement reviews the extant literature on the cardiovascular health of LGBTQ adults. Informed by the minority stress and social ecological models, the objectives of this statement were (1) to present a conceptual model

Transgender people and occupational diving: a new challenge for diving physicians?

The gender identity of transgender people is not fully aligned with their sex assigned at birth. It has been estimated that approximately 355 of 100,000 people in the general population consider themselves transgender. Transgender people are increasingly choosing to transition through gender-affirming hormone therapy, including treatment with testosterone or oestrogens and gender-affirming surgeries. Occupational diving is performed in a unique, highly hostile physiological environment. An occupational diver should be free of pulmonary, cardiovascular, neurological, and psychological risk fact

Entrapped Dilator in a Male-to-Female Transgender Patient: Tips and Tricks for Device Removal.

INTRODUCTION AND HYPOTHESIS: Medical cases involving foreign bodies can present unique challenges, especially when they occur in sensitive areas, such as the neovagina of a male-to-female transgender. METHODS: A 32-year-old transgender woman attended a tertiary, academic-affiliated hospital and reported to insert a vaginal dilator into the neovagina after removing its base and after mild abdominal pain and difficulty in removing the dilator after multiple successful attempts, she looked for help in the emergency room. RESULTS: A video article demonstrates a step-by-step approach to remove th

Future directions in transfeminine genital gender-affirming surgery: tissue substitutes and transplant.

PURPOSE OF REVIEW: The present study reviews contemporary literature focused on uterine transplantation and tissue substitutes for the purposes of transfeminine genital gender affirming surgery (GAS). Additional background is provided for both topics to provide a more comprehensive understanding of the modern applications. RECENT FINDINGS: Uterine transplant remains an experimental procedure in cisgender patients who wish to avoid surrogacy. A natural extension might be to apply these techniques to GAS. Technical barriers include anatomic differences and alterations to gender-affirming hormon

Transgender Vulvar and Clitoral Anatomy: A Guide for Gynecologists.

BACKGROUND: It is important for gynecologists to understand postoperative anatomy to provide inclusive and competent care to patients who have undergone gender-affirming vaginoplasty. The neoclitoris and neovula differ from cisgender anatomy. This article and associated video address these knowledge gaps to enhance gynecologic care for transgender patients. TECHNIQUE: We review the steps of penile inversion vaginoplasty, focusing on the creation of the neoclitoris and neovulva. The key differences and similarities between cisgender and transgender clitoral anatomy are highlighted. A detailed

The vaginal microbiome of transgender men receiving gender-affirming hormonal therapy in comparison to that of cisgender women.

The vaginal microbiome of trans men and menopausal women is suspected to be similar due to a lack of estrogen leading to the absence of lactobacilli. However, data are scarce. We performed an analysis of the vaginal microbiome of trans men (n = 25) in comparison to that of menopausal (n = 25) and premenopausal women (n = 25). The vaginal microbiome of trans men and menopausal women showed a higher alpha diversity than that of premenopausal women. Various beta diversity indices (e.g., Bray‒Curtis (Un-)Weigthed Unifrac), showed significant differences in community composition between trans men a

Ethical considerations for non-procreative uterus transplantation.

The growing demand for uterus transplantation (UTx) invites continued philosophical evaluation of the function of UTx (and what constitutes its 'success'), as well as the recipient eligibility for UTx. Currently, UTx caters to partnered, cisgender women of childbearing age looking to get pregnant and give birth to a biogenetically related child. The medical justification for this-the treatment of uterine infertility-explains the primacy of this practice. However, this dominant conceptualization of UTx does not necessarily capture the diverse needs for which both cis- and transgender women migh

Guidelines for research with transgender, gender diverse, and intersex individuals with eating disorders: recommendations from trans and intersex researchers.

Further research is urgently needed to address the disproportionately high rates of eating disorders (EDs) among transgender, gender diverse, and intersex (TGDI) individuals in comparison to cisgender, endosex (non-intersex) populations. As TGDI advocates, academics, and clinicians with lived/living experience with EDs, we propose a set of recommendations to guide ethical research specifically about EDs and disordered eating behaviors in TGDI populations. The guidelines included here aim to educate non-TGDI researchers and support TGDI researchers seeking to carry out such research. Considerat

Peri-surgical imaging of intersex and gender diverse youths.

This publication provides an overview of current imaging indications and practices for patients undergoing gender-affirming surgery, with an emphasis on the importance of tailored, patient-specific care. Gender-affirming surgeries are performed with personalized approaches at various stages of life for those with intersex traits or differences in sex development (I/DSD) and transgender and gender diverse (TGD) individuals. For I/DSD patients, ultrasound, genitography, or MRI occurs during infancy and puberty to evaluate genital and gonadal anatomy. Facial harmonization involves bony and soft t

Adverse childhood experiences among black sexually minoritized men and Black transgender women in Chicago.

BACKGROUND: Adverse childhood experiences (ACE) are important predictors of mental health outcomes in adulthood. However, commonly used ACE measures such as the Behavioural Risk Factor Surveillance System (BRFSS) have not been validated among Black sexually minoritized men (SMM) nor transgender women (TW), whom are known to have higher rates of ACE and poorer mental health outcomes. Assessing the psychometric properties of the measure is important for health equity research, as measurements that are not valid for some populations will render uninterpretable results. METHODS: Data are drawn fr

Overview of gender-affirming surgery.

This publication will discuss the diversity of procedures that are common to those with intersex traits,/differences of sex development (I/DSD), and transgender and gender diverse (TGD) patients. It will address how these procedures are performed, and common after care and long-term considerations which are relevant to the pediatric radiologist. Some surgeries offered to these patient populations have shared surgical approaches and radiographic considerations. With the growth of TGD procedures as a field, more pediatric subspecialties are performing these procedures, and the approaches are inf
