The Transitions Clinic Network: Post Incarceration Addiction Treatment, Healthcare, and Social Support (TCN-PATHS): A hybrid type-1 effectiveness trial of enhanced primary care to improve opioid use disorder treatment outcomes following release from jail.

BACKGROUND: In 2016, at least 20% of people with opioid use disorder (OUD) were involved in the criminal justice system, with the majority of individuals cycling through jails. Opioid overdose is the leading cause of death and a common cause of morbidity after release from incarceration. Medications for OUD (MOUD) are effective at reducing overdoses, but few interventions have successfully engaged and retained individuals after release from incarceration in treatment. OBJECTIVE: To assess whether follow-up care in the Transitions Clinic Network (TCN), which provides OUD treatment and enhanced

['Let Them Deal With It' - Interview Results From Follow-Up Care Provider Surveys In The 'Überpflege' Project].

Discharge management and transitional care are key interfaces within the German healthcare system. Nevertheless, there remain significant gaps in the transition of patients from hospital to post-discharge care. In the project "ÜberPflege - Care-centred transitional care in hospital to reduce post-discharge bottlenecks following inpatient treatment within the framework of discharge management", eleven guideline-based interviews were conducted with representatives of post-discharge care facilities and analysed using Kuckartz's content analysis method. The results reveal clear shortcomings in the…

Inpatient mortality in transition-aged youth with rheumatic disease: an analysis of the National Inpatient Sample.

BACKGROUND: Transition from pediatric to adult care is a vulnerable time for youth with chronic diseases. In youth with rheumatic disease, studies show high rates of loss to follow up and increased disease activity. However, mortality data are lacking. In this study, we assessed whether transitional age is a risk factor for inpatient mortality. METHODS: We analyzed the 2012-2014 National Inpatient Sample database, a representative sample of discharges in the United States. Individuals with rheumatic diseases were identified by International Statistical Classification of Disease - 9 (ICD-9) co

Reducing hospital readmissions in patients with end-stage kidney disease.

ESKD patients have a large burden of disease, with high rates of readmission to hospital compared with the general population. A readmission after an acute index hospital discharge is either planned or unplanned. A proportion of unplanned readmissions are potentially avoidable, and could have been prevented with optimized transitional care. Readmissions pose financial cost to the health care system and emotional cost to patients and caregivers. In other chronic diseases with high readmission risk, such as congestive heart failure, interventions have improved transitional care and reduced readm…

Bridging the Gap: Developing a Co-Designed Resource to Support the Hospital-to-Home Transition Following Limb Loss.

BACKGROUND: During the hospital-to-home transition, people with limb loss and their caregivers can face several practical, physical, social and psychological challenges. These challenges are amplified by gaps in transitional care support, ultimately leading to readmissions and increased healthcare costs. OBJECTIVE: This co-design research project aimed to help bridge the gap between hospital to home by working collaboratively with 10 healthcare providers, 14 people with limb loss, and 15 caregivers to (a) understand the transition from hospital to home and identify gaps in transitional care; …

Transition to adulthood and transfer to adult care of adolescents with congenital heart disease: a global consensus statement of the ESC Association of Cardiovascular Nursing and Allied Professions (ACNAP), the ESC Working Group on Adult Congenital Heart Disease (WG ACHD), the Association for European Paediatric and Congenital Cardiology (AEPC), the Pan-African Society of Cardiology (PASCAR), the Asia-Pacific Pediatric Cardiac Society (APPCS), the Inter-American Society of Cardiology (IASC), the Cardiac Society of Australia and New Zealand (CSANZ), the International Society for Adult Congenital Heart Disease (ISACHD), the World Heart Federation (WHF), the European Congenital Heart Disease Organisation (ECHDO), and the Global Alliance for Rheumatic and Congenital Hearts (Global ARCH).

The vast majority of children with congenital heart disease (CHD) in high-income countries survive into adulthood. Further, paediatric cardiac services have expanded in middle-income countries. Both evolutions have resulted in an increasing number of CHD survivors. Expert care across the life span is necessitated. In adolescence, patients transition from being a dependent child to an independent adult. They are also advised to transfer from paediatrics to adult care. There is no universal consensus regarding how transitional care should be provided and how the transfer should be organized. Thi…

The transitional circulation: physiology and anesthetic implications.

Over the past decade, several aspects of the physiology of the transitional circulation have been elucidated. The transitional circulation may be viewed as a process divided into four phases. The concept of these phases underscores the fact that the normal transitional circulation should be viewed as an orderly process and not a single event. Within these phases, several mechanisms seem to be involved in the control of pulmonary vascular resistance (PVR). Yet even these mechanisms do not completely explain the process of the normal transition, and very little is understood about why the transi

Visit-to-Visit Systolic Blood Pressure Variability Predicts Long-Term Cardiovascular Events After Coronary Artery Bypass Grafting.

This prospective observational study investigated whether systolic blood pressure variability (SBPV) during the transitional care period predicts long-term outcomes after coronary artery bypass grafting (CABG). We enrolled 120 consecutive patients undergoing isolated CABG between 2020 and 2024 in a structured Transitional Care Service System. Office blood pressure was measured at scheduled visits. SBPV was quantified using the coefficient of variation (CV), and patients were stratified by median SBP-CV (8.4%) into low- and high-variability groups. The primary endpoint was major adverse cardiac…

Understanding of the transition to adult healthcare services among individuals with VACTERL association in Sweden: A qualitative study.

Current knowledge of transitional care from the perspective of individuals with congenital malformations is scarce. Their viewpoints are required for the development of follow-up programs and transitional care corresponding to patients' needs. The study aimed to describe expectations, concerns, and experiences in conjunction with transfer to adult health care among adolescents, young adults, and adults with VACTERL association, (i.e. vertebral defects, anorectal malformations (ARM), cardiac defects (CHD), esophageal atresia (EA), renal, and limb abnormalities). Patient understanding of two commonly used patient reported outcome measures for primary care: a cognitive interview study.

BACKGROUND: Standardised generic patient-reported outcome measures (PROMs) which measure health status are often unresponsive to change in primary care. Alternative formats, which have been used to increase responsiveness, include individualised PROMs (in which respondents specify the outcomes of interest in their own words) and transitional PROMs (in which respondents directly rate change over a period). The objective of this study was to test qualitatively, through cognitive interviews, two PROMs, one using each respective format. METHODS: The individualised PROM selected was the Measure Yo
