Mandibular movement restoration through bilateral coronoidectomy by intraoral approach.

The present study aimed at reporting a clinical and surgical case of bilateral coronoidectomy, using an intraoral approach. The patient is a 26-year-old man, who sought attendance complaining of a gradual reduction of his oral opening in the past 3 years; however, he had an aggravation in the last 2 months. After clinical examination and imaging evaluation, the diagnosis of coronoid process hyperplasia was confirmed, and the surgical treatment was proposed. Under general anesthesia, with nasotracheal intubation guided by a nasofiberendoscope, using an intraoral approach, the bilateral coronoid

Conservative treatment of severe limited mouth opening after transtemporal craniotomy.

Limited mouth opening (LMO) is a common complication of the transtemporal craniotomy that is often involved in the temporalis and temporomandibular joint. The occurrence of prolonged LMO was rare. Risk factors for such limitation after these neurosurgical procedures are defined, and the strategies of treatment and prevention are further discussed. This article presents 12 patients with severe LMO after transtemporal neurosurgical procedures. Four patients received myofunctional training combined with the neuromuscular blocking in the temporalis, which methods may be more effective for LMO withi

Management of pain in peritonsillar abscess.

A prospective study was undertaken in 75 patients with peritonsillar abscess to determine the treatment that was most effective in relieving the excruciating pain associated with the condition. The patients were divided into three treatment groups: intravenous antibiotic, aspiration, and incision and drainage. The effect of treatment on pain was objectively assessed by serially measuring the upper to lower incisor distance and by giving the patient water to drink at regular intervals to determine the point at which swallowing was pain-free. The improvement of the mean upper to lower incisor di

Management of temporomandibular disorders in a restorative practice.

Various disorders related to the masticatory muscles or to the temporomandibular joints may be encountered in a restorative dental practice. Most of them can be managed by the restorative dentist whose primary interest may not lie in the area of temporomandibular disorders. The term temporomandibular disorders is used in this discussion to denote the group of diseases of the masticatory system that involves primarily the temporomandibular joints, the masticatory muscles, and the occlusion of the teeth. The learning objective of this article is to describe temporomandibular disorders encountere

Malignant hyperpyrexia in oral surgery--case report and literature review.

A case of malignant hyperpyrexia (MH) occurring in a patient undergoing orthognathic surgery is reported which resulted in the procedure being abandoned prior to completion. The oral surgeon's role in recognising the early clinical signs of masseteric spasm and central cyanosis is highlighted. The case shows that with proper investigation and management the MH susceptible patient was able to have a second anaesthetic with safety to complete the surgery.

Clinical symptoms of open lock position of the condyle. Relation to anterior dislocation of the temporomandibular joint.

Nine cases of open lock position of the condyle of the temporomandibular joint (TMJ) are reported. In two patients recurrent dislocation of the TMJ was diagnosed clinically, and four had previous episodes of anterior dislocation. An arthrotomographic examination revealed that the condyles of the affected TMJs were located anterior to the anterior bands of the disks at an open-mouth position. An arthrographic fluoroscopic examination showed that the anterior bands mechanically obstructed the anteriorly displaced condyles from posterior movement into the articular fossae to various degrees at op

Mandibular coronoid process locking: a prospective study of frequency and association with internal derangement of the temporomandibular joint.

Coronoid process locking has been regarded a rare condition. The literature reveals 59 reports each of one or two cases and an additional two papers reporting on four and six patients, respectively, the latter including hereditary cases. Coronoid process elongation tends to be overlooked when limitation of mouth-opening ability is investigated, since most interest is focused on the joint. To study the frequency of coronoid process hyperplasia and its possible association with TMJ disk displacement, 163 patients with limited mouth-opening ability were examined. All patients were referred for ra

Evaluation of Zoledronate for Pain Suppression in Diffuse Sclerosing Osteomyelitis of the Mandible: Protocol for a Prospective, Single-Arm Interventional Study.

INTRODUCTION: Diffuse sclerosing osteomyelitis (DSO) is a non-bacterial osteomyelitis that occurs infrequently and is characterized by recurrent severe pain, swelling of the mandible, and trismus. To date, no effective treatment has been established. Bisphosphonates have been suggested as an efficacious treatment for DSO; however, no prospective studies have examined the effect of bisphosphonate treatment in adults with DSO. This study aims to evaluate the effect of zoledronate on pain suppression in patients with DSO. METHODS: This is a single-center, exploratory, single-arm, prospective int…

Levofloxacin-Induced Oromandibular Dystonia in a 9-Year-Old Patient.

Extrapyramidal symptoms (EPS) that include akathisia, dystonia, pseudoparkinsonism, and dyskinesia are abnormal movements commonly induced by antipsychotic medications. These symptoms are also associated with specific non-antipsychotic agents. This case report describes a case of a 9-year-old boy on antibiotics treatment that developed EPS. A 9-year-old boy presented to the emergency department of Imam Hossein Children›s Hospital with chief complaints of trismus, difficulty speaking, and tongue protrusion. One week before these presentations, he had been prescribed Tavanex® (levofloxacin) and …

Bronchial Visualization of Tetanic Contractions: A Case Report.

BACKGROUND Tetanus is a potentially fatal infectious disease which, during its evolution, creates multiple complications, usually requiring intensive management and care. CASE REPORT We present a clinical case of a 59-year-old male patient with generalized tetanus admitted to the intensive care unit. Flexible bronchoscopy revealed contraction of the bronchial demonstrating that tetany existed at the respiratory level, which rarely becomes evident. The clinical manifestations included trismus, facial paralysis, neck stiffness, and compromised respiratory function. The patient presented a state …

[Cervicofacial actinomycosis: report of a case].

Actinomycotic infection involving the head and neck is not commonly diagnosed and is often a diagnostic enigma to the otolaryngologist. The reasons for this are due to the infrequency of this disease itself, the current general lack of familiarity with this disease and the pronounced sensitivity of actinomyces organisms to most of the common antibiotics in current use. The case reported here is fairly typical of cervicofacial actinomycosis. A 26-year-old man suffering from trismus and a noncharacteristic mass over the right parotidomasseteric region for one month after a lower molar tooth was …

Needle breakage during dental anesthesia: Management strategies and its associated preventive measures.

Dental anesthesia is useful and important for the procedures of tooth extraction, oral tissue biopsy, periapical and periodontal surgery and more others. However, needle breakage during dental anesthesia, presents a considerable challenge in its clinical management. Retained fragments can lead to pain, trismus, dysphagia, and infections, and may migrate into deeper spaces, increasing the complexity and risk upon its retrieval. To avoid such consequences, prompt removal of the broken fragment is often advocated. However, no standard guidelines were developed. This review provides a comprehensiv…

Biomaterial-based therapeutic strategies for oral submucous fibrosis.

Oral submucous fibrosis (OSF) is a chronic, potentially malignant oral mucosal disease driven by areca nut chewing. It is characterized by progressive collagen deposition and debilitating symptoms such as trismus and burning sensation, which severely impair quality of life and confer a risk of malignant transformation. Given the limited efficacy of current therapies, this review aims to provide a comprehensive and critical appraisal of the evolving therapeutic landscape for OSF. We summarize conventional strategies, including pharmacological, surgical and physical approaches, alongside emergin…
