Immune reconstitution inflammatory syndrome: immune restoration disease 20 years on.

Restoration of immune responses against opportunistic pathogens after commencing antiretroviral therapy (ART) may cause immune restoration disease (IRD) in about 10%-40% of HIV patients with low CD4(+) T-cell counts and usually presents clinically as a type of immune reconstitution inflammatory syndrome (IRIS). IRIS may be associated with many different opportunistic pathogens, but types associated with Mycobacterium tuberculosis, BCG, cryptococci, JC polyomavirus (the cause of progressive multifocal leukoencephalopathy [PML]), hepatitis C virus and hepatitis B virus infection are the most inf

[Severe disseminated constrictive polyserositis in a patient with rheumatoid arthritis].

Constrictive polyserositis (pleuritis, pericarditis) is a syndrome within the underlying disease (tuberculosis, periodic disease, rheumatoid arthritis, systemic lupus erythematosus, asbestos, silicosis, uremia, some genetic diseases), a complication due to chest surgery or radiation or drug therapy, is occasionally idiopathic (fibrosing mediastinitis). There are frequently great difficulties in making its nosological diagnosis. The paper describes a patient in whom the onset of disease was exudative pleurisy with the signs of constriction, arthralgias; pleural punctures provided serous exudate

Prevalence of Pretreatment Testing Recommended for Patients With Chronic Inflammatory Skin Diseases.

IMPORTANCE: Laboratory testing for the presence of tuberculosis, hepatitis, and other conditions before starting most systemic immunomodulatory agents is recommended in patients with chronic inflammatory skin diseases (CISD) but current testing patterns in the US are unclear. OBJECTIVE: To determine the prevalence of pretreatment testing that is recommended for patients with CISD (psoriasis, hidradenitis suppurativa, or atopic dermatitis). DESIGN, SETTING, AND PARTICIPANTS: This descriptive analysis of US commercial insurance claims databases from December 31, 2002, to December 31, 2020, inc

A real-world risk analysis of biological treatment (adalimumab and etanercept) in a country with a high prevalence of tuberculosis and chronic liver disease: a nationwide population-based study.

OBJECTIVES: Few studies on tumour necrosis factor (TNF) inhibitor-associated tuberculosis (TB) and hepatic events have been performed in regions where these risks are elevated. This study aimed to provide a direct comparison between adalimumab and etanercept in a high-risk population and to address the implications for physicians working with patients in such an environment. METHOD: Data collected from the National Health Insurance Research Database (NHIRD) in Taiwan between 2007 and 2011 were analysed retrospectively for incidences of eight adverse events associated with TNF-α inhibitors. Immunoallergic Hepatitis Induced by Antitubercular Drug: A Case Report.

Antitubercular treatment is associated with numerous adverse effects, among which immunoallergic reactions present a significant challenge in management. These reactions may sometimes lead to the discontinuation of one or more antitubercular drugs, potentially compromising the patient's recovery. We report the case of a 42-year-old patient, followed for pleuropulmonary tuberculosis, diagnosed clinically, and put on anti-bacillary treatment. The evolution was marked by the onset of hepatic cytolysis, revealed by vomiting that occurred five days later. The diagnosis of immuno-allergic hepatitis

[Infection associated cerebral vasculitis].

Infections are a frequent cause of cerebral vasculitis, important to diagnose because a specific treatment may be required. Infection-associated vasculitis can be caused by angiotropic pathogens (varicella zoster virus, syphilis, aspergillus). They can be associated with subarachnoidal meningitis (tuberculosis, pyogenic meningitis, cysticercosis). They can appear contiguously to sinuses or orbital infection (aspergillosis, mucormycosis). Finally, they also may be due to an immune mechanism in the context of chronic infections (hepatitis B virus, hepatitis C virus, human immunodeficiency virus)

Designing vaccines based on biology of human dendritic cell subsets.

The effective vaccines developed against a variety of infectious agents, including polio, measles, and hepatitis B, represent major achievements in medicine. These vaccines, usually composed of microbial antigens, are often associated with an adjuvant that activates dendritic cells (DCs). Many infectious diseases are still in need of an effective vaccine including HIV, malaria, hepatitis C, and tuberculosis. In some cases, the induction of cellular rather than humoral responses may be more important because the goal is to control and eliminate the existing infection rather than to prevent it.

The spectrum of liver diseases in HIV infected individuals at an HIV treatment clinic in Kampala, Uganda.

BACKGROUND: Liver diseases are common in patients with HIV due to viral hepatitis B and C co-infections, opportunistic infections or malignancies, antiretroviral drugs and drugs for opportunistic infections. OBJECTIVE: To describe the spectrum of liver diseases in HIV-infected patients attending an HIV clinic in Kampala, Uganda. METHOD: Consecutive patients presenting with jaundice, right upper quadrant pain with fever or malaise, ascites and/or tender hepatomegaly were recruited and underwent investigations to evaluate the cause of their liver disease. RESULTS: Seventy-seven consecutive pa

Elevated AST-to-platelet ratio index is associated with increased all-cause mortality among HIV-infected adults in Zambia.

BACKGROUND & AIMS: We investigated the association between significant liver fibrosis, determined by AST-to-platelet ratio index (APRI), and all-cause mortality among HIV-infected patients prescribed antiretroviral therapy (ART) in Zambia. METHODS: Among HIV-infected adults who initiated ART, we categorized baseline APRI scores according to established thresholds for significant hepatic fibrosis (APRI ≥1.5) and cirrhosis (APRI ≥2.0). Using multivariable logistic regression we identified risk factors for elevated APRI including demographic characteristics, body mass index (BMI), HIV clinical a

Infectious disease risks from dead bodies following natural disasters.

OBJECTIVE: To review existing literature to assess the risks of infection from dead bodies after a natural disaster occurs, including who is most at risk, what precautions should be taken, and how to safely dispose of the bodies. METHODS: Disease transmission requires the presence of an infectious agent, exposure to that agent, and a susceptible host. These elements were considered to characterize the infectious disease risk from dead bodies. Using the PubMed on-line databases of the National Library of Medicine of the United States of America, searching was done for relevant literature on th

Differential diagnosis of acute liver failure in India.

BACKGROUND: Acute liver failure (ALF) is a condition with rapid deterioration of liver function resulting in hepatic encephalopathy and/or coagulopathy in patients with previously normal liver. Complicated forms of certain infectious diseases like falciparum malaria, leptospirosis, dengue fever, ricketsial fever, typhoid fever, haemophagocytosis, herpes simplex virus, cytomegalovirus, tuberculosis or amoebic liver abscess can present with altered mentation and/or bleeding manifestations in presence of jaundice and mimic ALF due to acute viral hepatitis (AVH). METHODS: We describe our experien

Anal tuberculosis complicating anti-TNFα therapy.

A 42-year-old man receiving anti-tumour necrosis factor α (anti-TNFα) therapy with adalimumab due to psoriatic arthritis presented with a 2-month-old anal ulcer. An apical right lung infiltrate was found in his chest X-ray, although he had no pulmonary symptoms. Two biopsies of the ulcer were taken and reported as granulomatous, necrotising, with chronic inflammation (first), and as hyperplasic epidermis with linfocitary infiltrate and the presence of plenty of plasmatic cells (second). Histochemical techniques, including Ziehl-Neelsen, Grocott and periodic acid-Schiff stains, and PCR for Myco

Comparing timelines and evidence available to support new TB, HIV, and HCV drug approvals: The same, only different.

BACKGROUND: Tuberculosis (TB), human immunodeficiency virus (HIV), and hepatitis C virus (HCV) share a global presence and propensity to disproportionately affect marginalized populations. However, over recent decades, many fewer drugs have been brought to market for TB than for the others. Although three new anti-TB drugs have been approved in the US or Europe in the last 10 years, uptake of these drugs has been limited. Using case examples of drugs developed recently for TB, HIV, and HCV, we explore possible reasons. We examine the use and effect of regulatory pathways intended to address we

Loss to follow-up: Understand its determinants among tuberculosis patients, in Coimbra District (2005-2017).

INTRODUCTION: Tuberculosis patients who abscond from treatment are more likely to contribute to ongoing transmission. This growing concern has led to the development of this study in order to assess risk factors among Tuberculosis patients lost to follow up (LTFU) notified in the Coimbra District between 2005 and 2017. MATERIAL AND METHODS: A case-control study was designed to compare Tuberculosis patients LTFU (cases) to those successfully treated (controls). Forty-two LTFU cases were compared to 712 controls. Social and demographic data, co-morbidities, disease location, previous treatment

HIV and co-infections.

Despite significant reductions in morbidity and mortality secondary to availability of effective combination anti-retroviral therapy (cART), human immunodeficiency virus (HIV) infection still accounts for 1.5 million deaths annually. The majority of deaths occur in sub-Saharan Africa where rates of opportunistic co-infections are disproportionately high. In this review, we discuss the immunopathogenesis of five common infections that cause significant morbidity in HIV-infected patients globally. These include co-infection with Mycobacterium tuberculosis, Cryptococcus neoformans, hepatitis B vi

Diagnosis of avian tuberculosis in a horse by use of liver biopsy.

A 15-month-old Appaloosa colt had pyrexia, weight loss, diarrhea, and dependent edema. Abnormal laboratory findings included anemia, hypoalbuminemia, and hyperglobulinemia. Activities of liver-specific enzymes in this colt were high. Analysis of a liver biopsy specimen resulted in a presumptive diagnosis of tuberculosis. We recommend that liver biopsy be used as a diagnostic procedure if the differential diagnosis in a horse includes avian tuberculosis or granulomatous enteritis, especially if serum activities of liver-specific enzymes are high.
