Intramedullary spinal cord abscess: case report.

Intramedullary spinal cord abscesses are rare lesions with a poor prognosis unless diagnosed and treated promptly. We report a case of a Nigerian man with tuberculous meningitis that was complicated by an intraspinal cord abscess and was treated surgically. The literature regarding this uncommon clinical entity is reviewed.

Cervical Tuberculosis in the Postmenopausal Period: A Case of Mistaken Identity With Carcinoma.

Female genital tuberculosis (FGTB) is an uncommon manifestation of extrapulmonary tuberculosis that can closely resemble gynaecological malignancies, often resulting in delayed diagnosis and treatment. While the fallopian tubes are the commonest organs to be affected, isolated cervical involvement is exceedingly rare. We report the case of a 60-year-old multiparous woman from a tuberculosis-endemic region in Sudan who presented with severe lower abdominal, pelvic, and perineal pain, along with odourless vaginal discharge persisting for four months. Her condition had rendered her bedridden. Cli

First confirmation of Mycobacterium tuberculosis complex from medieval Ireland by aDNA analysis - palaeopathological and microbial findings.

Eight burials from the multi-period rural settlement site of Ranelagh near Roscommon town, Ireland, with palaeopathological lesions suggestive of skeletal tuberculosis or brucellosis were examined by ancient DNA (aDNA) testing. Tuberculosis infection (MTB complex DNA) was confirmed in five individuals - an 11th-13th CE adolescent female (14.5-17.5 years), two young adults females (18-35 years, 7th-10th CE), one adolescent of unknown sex and one middle-aged adult (35-50 years, medieval in date). In the latter case, the differential diagnosis included brucellosis due to the presence of small mul

Tuberculous spondylitis in Russia and prominent role of multidrug-resistant clone Mycobacterium tuberculosis Beijing B0/W148.

Extrapulmonary and, in particular, spinal tuberculosis (TB) constitutes a minor but significant part of the total TB incidence. In spite of this, almost no studies on the genetic diversity and drug resistance of Mycobacterium tuberculosis isolates from spinal TB patients have been published to date. Here, we report results of the first Russian and globally largest molecular study of M. tuberculosis isolates recovered from patients with tuberculous spondylitis (TBS). The majority of 107 isolates were assigned to the Beijing genotype (n = 80); the other main families were T (n = 11), Ural (n = 7

[Scoliosis secondary to tuberculous spondylitis: A case report and discussion of diagnosis].

Idiopathic scoliosis is the most common form of spinal deformity in adolescence. However, secondary causes of scoliosis should always be considered so as to provide appropriate treatment and avoid complications. Additional explorations, such as MRI, are required in the presence of atypical signs. We report a case of spinal tuberculosis, or Pott's disease, misdiagnosed as idiopathic scoliosis, and then present a discussion to help identify the warning signs during the evaluation of scoliosis that might indicate secondary scoliosis and suggest when to perform further radiologic exploration.

Use of adjunctive treatment with interferon-gamma in an immunocompromised patient who had refractory multidrug-resistant tuberculosis of the brain.

We describe a patient with acute lymphocytic leukemia and multidrug-resistant tuberculosis of the brain and spinal cord. Despite treatment with six antituberculous drugs and a steroid medication for 11 months, there was no appreciable clinical or radiological improvement in the patient's condition. Within 5 months of initiating adjunctive therapy with IFN-gamma and granulocyte colony stimulating factors, substantial neurological and radiological improvement was noted. Therapy with IFN-gamma was continued for 12 months, resulting in complete resolution of the lesions in the brain and spinal cor

[Tuberculosis of the spine in the elderly and its surgical treatment].

Among the contingents with newly diagnosed spinal tuberculosis are mainly subjects aged over 50 years who have many concurrent diseases, basically associated with age, late diagnosis and advanced destructive process attended by cerebrospinal disturbances in 67.5% of cases. The characteristic features and high incidence of complications of the disease course in this category of patients call for early and complete surgical treatment. The results of treatment demonstrate a satisfactory tolerance to spinal operations by elderly patients and their high effectiveness in timely application.

Vertebral osteomyelitis caused by nontuberculous mycobacteria: a case series of 23 patients and a review of 108 cases in the literature.

BACKGROUND: Nontuberculous mycobacteria (NTM) are ubiquitous environmental opportunistic pathogens that rarely cause spinal infections. The clinical presentation, diagnostic process, and treatment strategies for NTM vertebral osteomyelitis often resemble those of spinal tuberculosis, posing significant challenges to clinical management. OBJECTIVE: This study aims to systematically analyze the clinical characteristics, imaging findings, microbial profiles, treatment approaches, and outcomes of NTM vertebral osteomyelitis, with a comparative review of relevant global literature to further elucid

Imaging of musculoskeletal tuberculosis: a new look at an old disease.

There are certain imaging features that help to differentiate tuberculosis from other bone and joint disorders with a similar presentation. The current authors discuss these distinguishing imaging features particularly with respect to ultrasound, computed tomography, and magnetic resonance imaging. The judicious and appropriate use of these newer imaging modalities coupled with aspiration or biopsy can lead to earlier recognition of musculoskeletal tuberculosis before the onset of debilitating disease.

[One-stage combined anterior and posterior strategy in treating active tuberculosis of thoracic and lumbar spine complicated with severe kyphotic deformity].

OBJECTIVE: To explore an effective and reasonable surgical strategy for active spinal tuberculosis with severe kyphotic deformity (kyphotic angle >or= 45 degrees). METHODS: From January 2004 to January 2008, 30 consecutive patients of active spinal tuberculosis complicated with significant angulation were enrolled in this study, including 8 male and 22 female. The average age was 35 years (range, 7 - 60 years), with average angle of kyphosis of 58 degrees (range, 45 degrees - 70 degrees). There were 28 patients complicated with intraspinal abscess, of which 10 patients presented with incomple

Posterior Extensive Circumferential Decompressive Reconstructive Technique in Surgical Treatment of Upper Thoracic Spine Compressive Lesions.

BACKGROUND: Complexity in ventral surgical exposure and presence of scapula and large parascapular musculature during posterior surgery are the main obstacles in operative treatment of upper thoracic spine compressive lesions (UTSCLs), such as trauma, tuberculosis, and neoplasm. Recently, the advantages of combined ventrodorsal surgery could be successfully gained using a 1-stage posterior salvage technique with enough accessibility to all 3 spinal columns. This was a retrospective evaluation of 35 patients with UTSCLs treated by the posterior extensive circumferential decompressive reconstruc

Closing-opening wedge osteotomy for severe, rigid, thoracolumbar post-tubercular kyphosis.

Childhood spinal tuberculosis, especially when associated with severe vertebral destruction of more than two vertebral bodies can end up in severe deformity. These children show progressive deformity throughout the period of growth and can develop severe kyphosis of >100°. Such kyphosis is severely disabling with significant risk of neurological deficit and respiratory compromise. Surgical correction of these deformities by both anterior and posterior approaches has been described but each have serious limitations of approach, correctability and safety. We describe here a technique of posterio
