Granulomatosis after autologous stem cell transplantation in nonHodgkin lymphoma - experience of single institution and a review of literature.

BACKGROUND: Sarcoidosis before and after treatment of malignancy is an important differential diagnosis that has to be distinguished from lymphoma. PATIENTS AND METHODS: Hodgkin lymphoma, diffuse large B-cell lymphoma and aggressive follicular lymphoma are being staged and treatment effect is evaluated with PET-CT. We report three cases of aggressive lymphoma after high dose therapy and autologous stem cell transplantation with positive lymph nodes on PET-CT, which were histologically diagnosed as sarcoidosis/granulomatosis. In the literature, we found that false positive lymph nodes were mor

Chondro-osseous respiratory epithelial adenomatoid hamartoma of the nasal cavity.

Chondro-osseous respiratory epithelial adenomatoid hamartoma is a rare hamartomatous lesion of the nasal cavity, with only five cases reported in the literature to date. We report the case of a 3-year-old boy who presented with nasal obstruction and a mass in the left nasal cavity. The mass was completely resected on endoscopy. On microscopy, hamartomatous proliferation of respiratory-type glands admixed with islands of immature hyaline cartilage, characteristic of chondro-osseous respiratory epithelial adenomatoid hamartoma, was seen. Neither local recurrence nor distant metastasis was observ

A new subtype of pachydermodactyly: unilateral pachydermodactyly transgrediens.

Pachydermodactyly describes a rare condition of localized fibromatosis, usually symmetrically affecting the interphalangeal joints of both hands. We describe a case of a new subtype of pachydermodactyly in a 14-year-old boy, which we term 'unilateral pachydermodactyly transgrediens'. This atypical pattern is caused by specific localized mechanical manipulation of the hands. This condition contributes to the completely indolent spectrum of pachydermodactyly, and usually does not need therapy. Therefore it is essential not to misinterpret it as an inflammatory state such as juvenile idiopathic a

Elevated Lactate Secondary to Gastrointestinal Beriberi.

Thiamine deficiency usually occurs with prolonged nutritional deficiency and is almost universally identified with Wernicke's encephalopathy or beriberi. It is also, however, a rare cause of elevated lactate and gastroenterological symptoms. This case report describes a 30-year-old man with 2 weeks of gastroenterological symptoms and intermittent oral intake, who was found to have an elevated lactate level. Neurological exam was normal and an extensive workup was negative, but after being treated with thiamine, his lactate level improved overnight and all of his symptoms resolved. Thiamine lev

Asymmetrical Ovarian Enlargement: Caught Timely Before the Cut!

BACKGROUND: Van Wyk-Grumbach syndrome is a rare complication of prolonged untreated primary hypothyroidism characterized by precocious puberty and enlarged multicystic ovaries. High index of suspicion and timely diagnosis can prevent unnecessary surgical procedures because this condition can be reversed with thyroid hormone supplementation. CASE: A 12-year-old girl underwent abdominal ultrasonography for recurrent abdominal pain, which showed bilateral asymmetrical enlargement of ovaries. Ovarian tumor markers were negative. Subsequent evaluation revealed severe hypothyroidism. Patient was st

Intestinal angioedema induced by angiotensin-converting enzyme inhibitors: an underrecognized cause of abdominal pain?

Intestinal angioedema caused by angiotensin-converting enzyme inhibitors such as lisinopril is rare but well documented in the literature. Patients with this condition typically present with common symptoms such as diffuse abdominal pain, cramping, nausea, and emesis. Imaging is needed to reveal segmental edema of the small intestine, often associated with free fluid in the abdomen. The authors report 2 cases of intestinal angioedema caused by angiotensin-converting enzyme inhibitors. Awareness of this allergic reaction and careful history taking--noting temporal relationship to occurrence of

A diagnostic challenge: dilated pupil.

PURPOSE OF REVIEW: Dilated pupil is a diagnostic challenge encountered by neurologists and ophthalmologists. The aim of this review is to provide an overview of the current data and guidelines concerning dilated pupils. RECENT FINDINGS: The majority of recent reports on dilated pupils are indicative of several medical conditions that require evaluation. The topical synthetic parasympatholytic agents; local contamination of antihistamines with their antimuscarinic effects; atropine, scopolamine, and tropane alkaloids in all species of Datura plants may produce mydriasis. SUMMARY: The causes o

Tolosa-Hunt syndrome in children.

We report a case of Tolosa-Hunt syndrome in a 4-year-old girl. Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) revealed a left cavernous sinus involvement with internal carotid artery occlusion. Clinical signs included left ophthalmoplegia, convulsions and right hemiplegia. Etiopathogenesis of this syndrome is unclear but as in our case, response to corticosteroid therapy is spectacular and avoids unnecessary invasive diagnostic procedures. We underline the importance of cross-sectional imaging in the diagnostic approach. Several other diseases including tumors, vascular lesions

Nonmotor presentations of multiple system atrophy.

Multiple system atrophy (MSA) is a neurodegenerative disease characterized clinically by a combination of parkinsonian, cerebellar, autonomic and pyramidal features. Recent data show that the MSA clinical spectrum is wider than was originally thought, and may include various nonmotor symptoms that arise at disease onset. These symptoms include urinary disorders, erectile dysfunction in men, orthostatic hypotension, sleep disruption, stridor, and severe constipation. Neurologists and other specialists must be made aware that this relatively common neurodegenerative disease can manifest with non

Ultrasound soft-tissue applications in the pediatric emergency department: to drain or not to drain?

Soft tissue infections frequently prompt visits to the pediatric emergency department. The incidence of these infections has increased markedly in recent years. The emergence of community-acquired methicillin-resistant Staphylococcus aureus is associated with an increasing morbidity, mortality, and frequency of abscess formation. Bedside ultrasound may have a significant impact in the management of patients that present to the pediatric emergency department with soft tissue infections, including cellulitis, cutaneous abscess, peritonsillar abscess, and necrotizing fasciitis. Ultrasound is an e

Acute acalculous cholecystitis during the course of primary Epstein-Barr virus infection: a new case and a review of the literature.

OBJECTIVE: The aim of this study was to describe a case of acute acalculous cholecystitis occurring in the course of primary Epstein-Barr virus (EBV) infection. METHODS: The clinical features of the case were analyzed and compared to those of three other similar cases reported in the international literature. RESULTS: All cases occurred in European females with cholestatic hepatitis, presented with gallbladder wall thickening, and recovered uneventfully without the need for surgical intervention. CONCLUSIONS: Acute acalculous cholecystitis may occur during the course of acute EBV infection,

Anthrax: is there a risk of cross-infection during endoscopy?

Bacillus anthracis is a large, gram-positive, spore-forming bacterium responsible for human anthrax. Review of the medical literature indicates specific instrument reprocessing instructions addressing the prevention of B. anthracis transmission are limited. Although nosocomial anthrax infection has not been reported, strict adherence to well-established instrument reprocessing guidelines for cleaning and high-level disinfection is recommended to prevent patient-to-patient transmission of B. anthracis (or any other pathogen) via gastrointestinal endoscopes or bronchoscopes. All liquid chemical

Perioperative management of rheumatoid arthritis. Areas of concern for primary care physicians.

The presence of rheumatoid arthritis can impose increased risks on patients undergoing surgical procedures. Affected patients often have peripheral joint inflammation, temporomandibular joint disease, laryngeal involvement, or cervical spine disease. Preoperative evaluation should also include assessment of cardiovascular disease, pulmonary disorders, and Sjögren's syndrome. Perioperative control of infection and prophylactic use of antibiotics are particularly important. To avoid wound-healing problems, preoperative discontinuation of therapy with aspirin, nonsteroidal anti-inflammatory drugs

Anterior lingual mandibular salivary gland defect--a dilemma in diagnosis.

Anterior lingual mandibular salivary gland defects are very rare. These lesions present as non-definitive bony radiolucencies. There are several other radiolucent entities which present in the anterior mandible with greater frequency and this creates a diagnostic dilemma for the practitioner. The ultimate and proper treatment of non-treatment depends on a correct diagnosis of these lesions. The correctness of the diagnosis and avoidance of unnecessary traumatic procedures may be aided by more sophisticated radiographic studies, such as a CAT scan and/or Magnetic Resonance Imaging. A case of a

[Laparoscopy as an auxiliary aid in abdominal emergencies].

The authors present a prospective study of 159 diagnostic laparoscopy performed in patients about whom doubts existed about the actual visceral involvement after initial clinical and laboratory examination by the Surgical Staff on duty. Patients whose initial diagnosis was easily made were immediately submitted to surgery and were not examined by laparoscopy. The laparoscopic exam was shown to be able to diagnose a large number of acute intra-abdominal disturbances, whether or not traumatic, and to be efficient, with accuracy values of 97.4%. The exam also allowed to avoid 44.6% to unnecessary

Ectopic pregnancy: common and some uncommon misdiagnoses.

Several diseases of the peritoneal cavity may present in manners that are difficult to distinguish from ectopic pregnancy. A careful history and a thorough physical examination are paramount to making the correct diagnosis. Diseases of the gastrointestinal and urinary tracts usually present in specific manner. The difficulty arises when the clinical presentations of these disease processes are atypical. Under such circumstances, other diagnostic modalities, including serial beta-hCG testing and pelvic ultrasound, are extremely useful in distinguishing ectopic pregnancy from other diseases that
