A total of 100 female patients with putative urinary stress incontinence (USI) were surveyed by us. Of these, 27 patients with genuine USI underwent urologic, uroradiologic and urodynamic evaluations before and after their corrective surgery by the technique of Burch. The results of the surgery were assessed clinically and subjectively and compared to the data obtained from postoperative urodynamic analysis. The operation affected significantly the urethral closure pressure and the posterior angle between the urethra and the base of the bladder, bringing about marked clinical improvement in mo

The different types of urinary incontinence in the female are defined and described. A meticulous differential diagnosis is a prerequisite for adequate treatment. The pre-treatment examination includes a detailed history, a residual urine, a gynaecological examination, a neurological examination, an intraveinous pyelogram, the incontinence test, the Bonney and the Bethoux tests, and a urethro-cystoscopy. In difficult cases urodynamic tests and X-rays of the bladder and the pelvic organs are added to the workup. Among 54 patients with urinary incontinence, 31 had stress incontinence and 23 had …

The usual cause of urinary incontinence secondary to pelvic trauma is damage to the sphincter mechanism in the prostatic urethra. A case is presented of an unusual and correctable cause of incontinence, vesicourethral fistula. This complication can probably be prevented by adequate debridement, drainage, and diversion.

Thirty-six patients with recurrent stress incontinence had undergone a total of 109 previous procedures, at least one of which had been a Marshall-Marchetti-Krantz (MMK) operation. At the time of operative dissection, it was apparent that most of these patients had had an "alleged" MMK operation with no evidence of adhesion or scarring in the periurethral and vesical neck regions. With the transvesical approach, accurate assessment of the tone and quality of the bladder neck and the proximal urethra is possible. This approach also provides an opportunity to "see and feel" the effect of the pro

INTRODUCTION: The failure rate for anti-incontinence surgery ranges from 5% to 80%. There is not actual consensus on the use of artificial urinary sphincter (AUS) as treatment for recurrent urinary incontinence in women. Several authors have shown that AUS can be useful, if the intrinsic sphincteric deficiency is checked. We present the first case in Spain, to our knowledge, of laparoscopic implantation of AUS as treatment for female recurrent urinary incontinence.
MATERIAL AND METHODS: Under general anaesthesia, patient was placed in supine decubitus with slight Trendelenburg, access to the …

INTRODUCTION: To evaluate, feasibility, efficacy and morbidity of laparoscopic artificial urinary sphincter (AUS) implantation in women with severe stress urinary incontinence.
PATIENTS AND METHODS: Twenty-six women with severe stress urinary incontinence were treated between October 2007 and January 2012 by laparoscopic implantation of an AUS AMS 800 (American medical Systems, Inc., Minnetonka, Minnesota). For 18 patients AUS was primary implanted and, for eight, AUS was revised for a mechanical failure. Three patients had a concomitant laparoscopic vaginal prolapse repair. Mean value was fo…

PURPOSE OF REVIEW: Stress urinary incontinence (SUI), defined as involuntary leakage with effort, exertion, sneezing, or coughing, remains a significant cause of morbidity. While well characterized in the general population, SUI in the setting of neurogenic lower urinary tract dysfunction (NLUTD) poses unique diagnostic and management challenges. This review outlines sex-specific epidemiology, diagnostic strategies, and evolving management approaches for SUI in NLUTD, emphasizing individualized and mechanism-based care.
RECENT FINDINGS: Incontinence prevalence varies by neurologic diagnosis, …

OBJECTIVE: The aim of this study was to evaluate the association between gestational diabetes mellitus and the severity and impact of lower urinary tract symptoms and urinary incontinence on quality of life in pregnant women.
METHODS: This case-control study enrolled 500 pregnant women (251 with gestational diabetes mellitus; 249 healthy controls) between 30 and 38 weeks' gestation. Women completed the International Consultation on Incontinence Questionnaire-Short Form, Urogenital Distress Inventory-Short Form, Incontinence Impact Questionnaire-Short Form, and Incontinence Severity Index. OBJECTIVE: To describe for the first time the technique of robot-assisted artificial urinary sphincter (R-AUS) insertion in male patients with neurogenic incontinence.
MATERIALS AND METHODS: From January 2011 to the present date, six patients with spinal cord injury have undergone R-AUS insertion at our academic institution and we have prospectively collected data on pre-, peri- and early postoperative outcomes. A transperitoneal five-port approach was used using a three-arm standard da Vinci® robot (Intuitive Surgical, Sunnyvale, CA, USA) in a 30° reverse Trendelenburg position. The artifici

The majority of nursing home residents experience some type of urinary incontinence. Other bladder-related disorders (eg, urinary retention and urinary tract infection) also are common in long-term care facilities. Efforts to manage urological conditions such as the use of indwelling catheters and absorbent products, perineal hygiene and care, toileting, and bladder rehabilitation are areas of concern and have become the subject of revised regulations. The intent of recent changes to the Centers for Medicare and Medicaid Services surveyor guidance for incontinence and urinary catheters is to e…

INTRODUCTION: The artificial urinary sphincter (AUS) is a rare indication for stress urinary incontinence secondary to spinal cord injury.
CLINICAL PICTURE: A 48-year-old male sustained spinal cord injury, resulting in detrusor acontractility and urinary stress incontinence.
TREATMENT AND OUTCOME: An AUS was implanted and the patient achieved complete urinary continence.
CONCLUSION: The AUS is an appropriate and durable device for management of patients with neurogenic stress incontinence.

Background/Objectives: This study examined whether adding relaxation-oriented immersive virtual reality (VR) to pelvic floor muscle electrostimulation with electromyographic (EMG) biofeedback was associated with additional improvement in incontinence-related quality of life among postmenopausal women. Methods: In this exploratory non-randomized controlled pilot trial, 40 postmenopausal women received electrostimulation with EMG biofeedback alone (ES; n = 20) or the same intervention preceded by VR (VR + ES; n = 20). Participants completed eight sessions over 2 weeks and were assessed at baseli

PURPOSE: Previous studies suggested better functional outcomes and longer device survival for female artificial urinary sphincter (AUS) implantation compared to male AUS implantation. We hypothesized that the adoption of robotic approaches for female implantation might have influenced these comparisons. This study aimed to compare the outcomes of robotic female AUS and male AUS implantation for non-neurogenic stress urinary incontinence (SUI).
METHODS: We retrospectively reviewed charts of male patients who had AUS implantation and female patients who underwent robotic AUS implantation for no…

INTRODUCTION: To spread de concept of a new artificial urinary sphincter with conditional occlusion for stress incontinence. The new prototype was conceived and designed in The Institute of Urology and Nephrology of London by Professor Craggs M. and Professor Mundy A.R.
METHODS: The FlowSecure sphincter consists of an adjustable pressure-regulating balloon, a stress relief reservoir, a control pump and valve assembly unit with self-sealing port and a urethral cuff. The pressure regulating balloon determinates de operating pressure of the device; the pressure is adjustable in the range 0-80 cm
