Primary peritoneal carcinoma presenting on routine papanicolaou smear.

BACKGROUND: Primary peritoneal carcinoma is an uncommon disease, characterized by peritoneal carcinomatosis without other identifiable primary tumor. It typically presents resembling ovarian cancer, with abdominal pain and distention and in an advanced stage. We report a unique presentation of this disease. CASE: A 76-year-old woman had severe glandular dysplasia on a screening Papanicolaou smear. An ectocervical lesion was biopsied, revealing moderately differentiated adenocarcinoma. Cervical stenosis prevented sampling of the endocervix and endometrium. Colonoscopy and mammography did not r

Leiomyosarcoma of the vagina.

Vaginal leiomyoma is uncommon and borderline leiomyoma of the vagina is an extreme rarity. Leiomyoma of the vagina may undergo malignant change to become borderline malignancy or leiomyosarcoma. A 38-year-old woman, complaining of foul vaginal discharge and dyspareunia with 2 months duration, underwent total excision of tumor. The removed tumor was diagnosed as borderline case of leiomyoma histologically. Unfortunately, she had tumor recurrence within 4 months. Thus, it was more likely that the primary diagnosis was incorrect and that the diagnosis leiomyosarcoma has been missed. She underwent

Vaginal stump recurrence in carcinoma of the corpus uteri.

Among 403 women who had an operation for a cancer of the corpus uteri, and a subsequent radiation (3400 R HD) a total of 10 (2.48%) suffered from a recurrence of the cancer at the vaginal stump. For none of these ten women a 5-year-cure can be reported. To prevent a recurrence of the cancer at the vaginal stump, it is recommended that a vaginal collar should be removed during the operation. Moreover, an increased dose of irradiation of the small pelvis (6000 R HD), a postoperative vaginal Caesium-137-filling, and a preoperative filling of the uterus with radiation carriers is recommended.

Malignant Peripheral Nerve Sheath Tumor of the Rectum Characterized by Focal S100 Protein Expression.

This case describes a malignant peripheral nerve sheath tumor (MPNST) arising in the ischiorectal fossa, an anatomical location that is infrequently reported in the scientific literature. The report provides new anatomical and clinical insights into a rare and aggressive neoplasm that is frequently misdiagnosed due to nonspecific symptoms and overlapping histopathological features with other rectal or perianal tumors. A 52-year-old woman with no significant medical or oncologic history presented with mild rectal bleeding and a painful perianal mass, initially presumed to be thrombosed hemorrho

Comparison of angiomyofibroblastoma and aggressive angiomyxoma in both sexes: four cases composed of bimodal CD34 and factor XIIIa positive dendritic cell subsets.

Aggressive angiomyxoma (AA) is a distinctive, locally aggressive, fibromyxoid tumor of the pelvic and genital soft tissues. AA is of unknown histogenesis but the cytologically bland spindled tumor cells, which surround characteristic variegated blood vessels, show fibroblastic or myofibroblastic features. AA may be related to angiomyofibroblastoma (AMF), another cytologically bland fibromyxoid genital spindle cell tumor with variable myoid differentiation that does not, as a rule, recur. Recently, CD34+ primitive fibroblasts and factor XIIIa+ dendritic histiocytes have been found in varying co

Luteoma of Pregnancy Presenting as Ruptured Ectopic Pregnancy: A Case Report.

Luteoma of pregnancy is a rare, benign neoplasm arising from the ovary, which occurs due to pregnancy-induced hormones. This rare ovarian lesion was first described by Sternberg and Barclay in 1966. Our case is unusual as the ovarian mass was misdiagnosed as ruptured ectopic pregnancy. Only three such cases have been previously reported in the literature. A 28-year-old multigravida with three months of amenorrhea presented with vaginal bleeding, abdominal pain, and gradually increasing vertigo for six days with increased intensity in the last four hours. On examination, she was conscious and or

Uterine Neurotrophic Tyrosine Receptor Kinase Rearranged Spindle Cell Neoplasms: Three Cases of an Emerging Entity.

Uterine sarcomas are rare; most are either smooth muscle or endometrial stromal in origin. Recent molecular advances have identified several, genetically defined entities with specific morphologic, clinicopathological associations, and therapeutic options. We report 3 cases of uterine neurotrophic tyrosine receptor kinase ( NTRK )-rearranged spindle cell neoplasms," currently classified as "emerging entities" in the WHO Classification of Female Genital Tract Tumors, 2020, Fifth Edition. The affected patients were 32, 34, and 42 years of age. Two patients presented with vaginal bleeding; the th

[Understanding lower gynaecological cancer consultation delay and help-seeking behavior in patients over 65].

INTRODUCTION: Delays for consultation of more than six months exist for uterine cancer. Delays in diagnosis of more than five years exist for vulvar cancer. The peak incidence of these neoplasms appear after the age of 65 years. Patient's symptoms are characteristically swelling, vaginal bleeding or itching. This study aims to understand what is happening during this period for women over 65 years old. It also tries to identify triggers during the help-seeking period. METHODS: Qualitative studies using semi-structured interviews with dual analyses (semio-pragmatic and psychodynamic) have been

Female genital tract cancer.

BACKGROUND: Site-specific cancer frequencies and incidence rates are reported regularly by the National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) program, but not by histologic type within site. This report reviews data for 160,977 histologically or cytologically confirmed invasive and in situ cancers of the female genital tract. METHODS: Data were supplied by the SEER program for histologically confirmed cases of uterine corpus, uterine cervix, ovary, vulva, vagina, fallopian tube, and placental cancers diagnosed between 1973 and 1987. Histologic types were review

Radiation-induced Osteosarcoma of the vaginal vault following cervical cancer treatment.

A woman in her 60s with a prior history of cervical carcinoma treated with chemoradiation presented with a polypoidal vaginal lesion nearly 7 years after initial therapy. Histopathology confirmed osteosarcoma confined to the posterior vaginal wall, with imaging excluding metastases. Total vaginectomy was performed, and final diagnosis was radiation-induced vaginal osteosarcoma. Although adjuvant chemotherapy was recommended, the patient declined. 6 months later, local recurrence and lung metastases developed. This case highlights the rare occurrence of radiation-induced osteosarcoma at the vag

Diagnostic Performance of Diffusion-Weighted MRI and ADC Values in Differentiating Atypical Leiomyomas from Uterine Sarcomas.

BACKGROUND: Leiomyomas are the most common benign uterine tumors, whereas uterine sarcomas are rare malignant neoplasms associated with high morbidity and mortality. Differentiation using imaging remains challenging because of overlapping radiologic features. PURPOSE: To evaluate magnetic resonance imaging (MRI) characteristics of atypical leiomyomas and uterine sarcomas, with emphasis on diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) values. MATERIALS AND METHODS: In this retrospective study, 22 patients with histopathologically confirmed uterine leiomyoma or sarc

Clinical management of atypical polypoid adenomyoma of the uterus. A clinicopathological review of 29 cases.

OBJECTIVE: The clinical management of atypical polypoid adenomyoma (APAM) of the uterus remains to be established. We collected APAM cases, reviewed the clinicopathological features, and discussed the clinical management. METHODS: Twenty-nine patients with APAM were identified by searching the tumor registry of the Japan Clinical Oncology Group (JCOG). Clinical information and histological specimens were obtained from 13 institutional members of the JCOG, and a central pathological review was performed. RESULTS: The mean age of the patients was 38 years (range, 22-58). Squamous metaplasia wa

Conventional endometrioid adenocarcinomas of the endometrium recurring as clear cell tumors: comparative immunohistochemical analyses.

Endometrial carcinomas are known to have the potential for recurrences that are distinctly discordant at the morphologic and immunophenotypic levels from their antecedent primary tumors. This report describes 3 patients with stage I, low or intermediate grade, conventional endometrioid carcinomas that recurred at the vaginal apex as notably clear cell-rich, higher grade, histotypically ambiguous neoplasms. Comparative immunohistochemical analyses were performed on all cases on both the original and the recurrent tumors using a panel of 8 biomarkers, including estrogen receptor, progesterone re

Unusual presentation of aggressive angiomyxoma arising from the posterior vaginal wall.

A multiparous woman presented with a large bulge protruding from the posterior wall of the vagina, initially diagnosed as a rectocele. Imaging revealed a cystic lesion measuring 9 cm × 6 cm × 3.5 cm suspected to be a developmental cyst. The lesion underwent surgical excision with significant improvement in symptoms. Histological analysis reported an aggressive angiomyxoma (AAM); postoperative hormonal therapies were considered and rigorous follow-up initiated. The authors discuss challenges in investigating and managing AAM with proposals made for improving preoperative diagnosis and optimisin
