Laparoscopy is useful in both the diagnosis and the management of impalpable testes. Intra-abdominal testicles can be removed laparoscopically if atrophic or can be partly devascularized by spermatic vessel clipping if apparently normal. Twenty three patients with impalpable tests underwent diagnostic laparoscopy in our hospital between 1988 and 1991, in an attempt to evaluate this modality of investigation. Four of them had bilateral undescended, i.e. total of 27 tests, 12 on the right side and 15 on the left. Eight patients were adults, age between 23 and 38, average 30 years, 15 were children, age between 1 and 9, average 5 years. We report a rare case of vasitis nodosa. A 54-year-old man was found to have a right spermatic cord mass during the herniorrhaphy operation. There was no history of surgical or traumatic causes. An indurated mass in the right spermatic cord was suspected to be spermatic cord tumor, and a right high orchiectomy was performed. Histological examination revealed disruption of the ductal lumen, granulomas with the sperm-like necrotic tissue and proliferation of the lymphocytes and plasma cells in the smooth muscle bundles. Pathological diagnosis was vasitis nodosa.

We have to date installed artificial spermatoceles made of silicone in a total of 33 patients. Artificial insemination of husband (AIH) was performed in 1-9 times using semen collected from 11 of the 33 patients in whom spermatocele was installed, and pregnancy was achieved in 2 cases. We reviewed the medical records of 65 boys with surgically and pathologically proved monorchism. A striking laterality to the left side was identified. Free L-carnitine was assayed in semen from patients with various andrologic diseases by an enzymatic-spectrophotometric method. Extremely low concentrations were found in semen from patients with agenesis of the vas deferens (31.8 +/- 18.8 nm/ml). Herein is presented a new technique to construct an artificial spermatocele, using monofilament knitted polypropylene wrapped with tunica vaginalis and anastomosed end-to-end to the head or the body of the epididymis. The material was tested in 10 dogs. It did not harm the spermatozoa, the function of the reservoir or the tolerance of the surrounding tissues. Therefore, the material was used in a patient with bilateral congenital absence of the vasa deferentia, while wife became pregnant following cervical insemination. The spermatocele is still functional after 15 months.

The cytologic appearance of epithelioid sarcoma in fine-needle aspiration biopsy cytology (FNABC) smears has not been extensively described. We report a case of epithelioid sarcoma in a 55-year-old male who presented with nodular swellings in the abdominal wall and scrotum. The scrotal swelling was subsequently demonstrated to arise from the vas deferens. We highlight the cytomorphologic, immunocytochemical, and histopathologic features of this rare lesion in an unusual site, with particular emphasis on the dilemma encountered in the cytodiagnosis of this lesion.
