[Bone and calcium update; diagnosis and therapy of metabolic bone disease update. Advances in treatment of CKD-MBD].

Secondary hyperparathyroidism and vascular calcification are major clinical states in CKD-MBD (chronic kidney disease-mineral and bone disorder) . Recently fibroblast growth factor 23-klotho axis has been shown to be implicated in the pathogenesis of secondary hyperparathyroidism. Vascular calcification has been identified to be a highly regulated process. Novel therapeutic agents for CKD-MBD have been developed. The guidelines of CKD-MBD have been announced from many countries including Japan from the point of view of patient's survival, and it is expected that the development of practice for…

Intracranial calcifications in childhood: Part 1.

This article is the first of a two-part series on intracranial calcification in childhood. Intracranial calcification can be either physiological or pathological. Physiological intracranial calcification is not an expected neuroimaging finding in the neonatal or infantile period but occurs, as children grow older, in the pineal gland, habenula, choroid plexus and occasionally the dura mater. Pathological intracranial calcification can be broadly divided into infectious, congenital, endocrine/metabolic, vascular and neoplastic. The main goals in Part 1 are to discuss the chief differences betwe

Local delivery of osteopontin attenuates vascular remodeling by altering matrix metalloproteinase-2 in a rabbit model of aortic injury.

OBJECTIVE: Vascular remodeling, often accelerated after cardiovascular procedures, may result in stenosis or aneurysm formation. The bone-associated protein osteopontin has been suggested to be involved in vascular remodeling, yet the effect of locally applied osteopontin in an acute vascular injury model of aortic calcification has not been examined. METHODS: Vascular healing of rabbit thoracic aortas treated locally with recombinant osteopontin (dose: 1 microg; n = 16) or albumin (control, n = 16) after acute injury created by end-to-end anastomosis was evaluated. Matrix metalloproteinase-2

Automated assessment of breast arterial calcification: a new frontier in cardiovascular risk prediction.

Breast Arterial Calcification (BAC) is a common incidental finding on mammograms, traditionally regarded as a benign degenerative change. However, a growing body of evidence indicates that BAC is associated with coronary artery disease (CAD), particularly in women with hypertension. Hypertension may contribute to BAC through persistent hemodynamic stress and vascular endothelial dysfunction, which promote the osteoblast-like transformation of medial vascular smooth muscle cells and calcium deposition in the extracellular matrix. The resulting increase in arterial stiffness is consistent with M

Massive left atrial calcification and devastating systemic emboli in a patient with chronic renal failure.

Valvular calcification is a common consequence of systemic calcium deposition resulting from chronic renal failure. Thrombi can form on such vascular calcifications and embolize to the cerebral, myocardial, and mesenteric vasculature with devastating consequences. We report the unique case of a patient with myocardial and cerebrovascular ischemia resulting from emboli arising from a massive mitral annular calcification.

[Calcific uremic arteriolopathy in hemodialysis patient, review of literature through five cases reports].

Calcific uremic arteriolopathy, also called calciphylaxis, is a rare and severe disorder that presents with skin ischemia and necrosis, sometimes it presents with systemic necrosis, the process is secondary to the obliteration of the arterioles first by sub-intimal calcium deposits and then by thrombosis. These lesions can often lead to death due to infectious complications and comorbidities such as diabetes, obesity, arteritis, diffuse vascular calcifications, heart disease and undernutrition. The diagnosis is suggested by the characteristic ischemic skin lesions and their distribution, often…

Association between soluble Klotho levels and calcification propensity T50 in patients with hemodialysis: a cross-sectional study.

Soluble Klotho is an anti-aging hormone implicated in vascular calcification. The calcification propensity score (T50) is a method used to evaluate the calcification propensity. However, no study has evaluated the association between soluble Klotho and T50. Therefore, this study aimed to investigate the association between soluble Klotho and T50 in patients undergoing hemodialysis. This cross-sectional study recruited patients undergoing hemodialysis. We measured T50 using the turbidimetric method and soluble Klotho using an enzyme-linked immunosorbent assay. Multiple regression analysis was u…

Diffuse hepatic calcifications in a transfusion-dependent patient with Beta-thalassemia: a case report.

Hepatic calcification is usually associated with infectious, vascular, or neoplastic processes in the liver. We report the first case of beta-thalassemia major with isolated diffuse hepatic calcification in a 23 year old woman, who had been transfusion-dependent since the age of 6 months. She was referred to our center with a chief complaint of abdominal pain. Computed tomography scan of the abdomen revealed diffuse hepatic calcification in the right, left, and caudate lobes of the liver. Her medical history disclosed hypoparathyroidism as well as chronic hepatitis C virus infection, which was

Spironolactone ameliorates PIT1-dependent vascular osteoinduction in klotho-hypomorphic mice.

Klotho is a potent regulator of 1,25-hydroxyvitamin D3 [1,25(OH)2D3] formation and calcium-phosphate metabolism. Klotho-hypomorphic mice (kl/kl mice) suffer from severe growth deficits, rapid aging, hyperphosphatemia, hyperaldosteronism, and extensive vascular and soft tissue calcification. Sequelae of klotho deficiency are similar to those of end-stage renal disease. We show here that the mineralocorticoid receptor antagonist spironolactone reduced vascular and soft tissue calcification and increased the life span of kl/kl mice, without significant effects on 1,25(OH)2D3, FGF23, calcium, and

Endogenous α-Klotho is a common target in atherosclerosis and calcific aortic valve disease.

Although atherosclerotic vascular disease (ASVD) and calcific aortic stenosis (AS) are distinct clinical entities, inflammation is a common driving force. The contribution of endogenous α-Klotho, an anti-aging protein, in the vasculature and aortic valve (AV) as an inflammatory mediator of such diseases remains unclear. We investigated α-Klotho expression using qPCR, ELISA, and immunohistochemistry in vascular (aorta, carotid, and femoral) and AV tissue samples from patients with ASVD (n = 112) and calcific AS (n = 172), respectively. We also determined circulating levels of soluble α-Klotho.

Management of phosphorus load in CKD patients.

Disturbances in mineral and bone metabolism play a critical role in the pathogenesis of cardiovascular complications in patients with chronic kidney disease (CKD). The term "renal osteodystrophy" has recently been replaced with "CKD-mineral and bone disorder (CKD-MBD)", which includes vascular calcification as well as bone abnormalities. In Japan, proportions of the aged and long-term dialysis patients are increasing which makes management of vascular calcification and parathyroid function increasingly more important. There are three main strategies to manage phosphate load: phosphorus dietary…

[New Developments in CKD-MBD. New aspects in phosphate binders].

In patients with end-stage renal disease (ESRD) , hyperphosphatemia contributes to secondary hyperparathyroidism and can lead to skeletal complications including fracture, and vascular calcification. Currently employed phosphate binders include calcium carbonate, sevelamer hydrochloride, and lanthanum carbonate. While these agents are effective at binding orally ingested phosphates, they vary in safety, potency, and off-target effects, some of which may be salutary (e.g., low-density lipoprotein and uric acid lowering with sevelamer) and others adverse (low turnover bone disease and progressiv…

Correlation of pre-existing vascular pathology with arteriovenous graft outcomes in hemodialysis patients.

BACKGROUND: Arteriovenous grafts (AVGs) are prone to neointimal hyperplasia leading to AVG failure. We hypothesized that pre-existing pathologic abnormalities of the vessels used to create AVGs (including venous intimal hyperplasia, arterial intimal hyperplasia, arterial medial fibrosis, and arterial calcification) are associated with inferior AVG survival. STUDY DESIGN: Prospective observational study. SETTING & PARTICIPANTS: Patients with chronic kidney disease undergoing placement of a new AVG at a large medical center who had vascular specimens obtained at the time of surgery (n = 76).

Necrotizing funisitis and calcification of umbilical vein: case report and review.

BACKGROUND: Necrotising funisitis (NF) is a rare, chronic stage of funisitis, a severe inflammation of the umbilical cord and an important risk factor for fetal adverse outcomes. NF is characterized by yellow-white bands running parallel to the umbilical blood vessels. These bands consist of inflammatory cells, necrotic debris, and calcium deposits. Calcification is visible in ultrasonography, which makes it possible to suspect NF when umbilical vascular wall calcification is detected by prenatal ultrasonography. CASE PRESENTATION: Ultrasonography revealed calcification of the umbilical venou

Recent advances in the crosstalk between bone and vascular system: from mechanism to therapy.

Osteoporosis and cardiovascular disease are prevalent health concerns, particularly among the elderly. Recent studies have increasingly demonstrated that osteoporosis is consistently associated with vascular calcification, drawing attention to the interactions between bones and blood vessels, and giving rise to the concept of the bone-vascular axis. The bone vascular axis involves several factors, including osteokines such as FABP3, PDGF-BB, MYGDF, and Aging Bone-Derived Extracellular Vesicles (AB-EVs), which influence vascular calcification. Simultaneously, calcified blood vessels secrete scl…
