Alternobaric vertigo in sport SCUBA divers and the risk factors.

We investigated the eustachian tube function and the incidence of alternobaric vertigo (AV) in 29 sport self-contained underwater breathing apparatus (SCUBA) divers with, or without, some possible risk factors for AV. The divers had normal audiological and otoscopic findings at the pre-dive examination. We used the nine-step inflation/deflation tympanometric test and Toynbee test for evaluation of eustachian tube function, and the Valsalva manoeuvre for patency. Information on divers, their history, and their otolaryngologic examination were obtained in the pre-dive examination. Divers perform…

Symptoms of vertigo in general practice: a prospective study of diagnosis.

BACKGROUND: There is little published evidence of the general practice experience of the diagnostic outcomes when symptoms of vertigo present. What research there is has been dominated by specialist centres. This gives a skewed view of the prevalence of the causes of such symptoms. AIM: To describe the likely diagnosis of symptoms of vertigo. DESIGN OF STUDY: Prospective cohort study METHODS: Thirteen GPs were recruited and trained to clinically assess and follow up all patients presenting with symptoms of vertigo over a six-month period Age-sex data were simultaneously gathered on those wh…

An autosomal dominant disorder with episodic ataxia, vertigo, and tinnitus.

The authors report an autosomal dominant episodic ataxia that is clinically distinct from the other episodic ataxias. Vestibular ataxia, vertigo, tinnitus, and interictal myokymia are prominent; attacks are diminished by acetazolamide. Linkage analyses of markers flanking the EA1 and EA2 loci demonstrate genetic exclusion from the other autosomal dominant episodic ataxias. The authors suggest EA3 for periodic vestibulocerebellar ataxia and EA4 for the disorder described here.

The canalith repositioning maneuver in patients with benign positional vertigo.

The canalith repositioning maneuver (CRM), as defined by Epley, can be an effective treatment for benign paroxysmal positional vertigo (BPPV). The staff at Başkent University's Ear Nose and Throat Clinic performed CRM on 68 cases of canalithiasis in 64 BPPV patients from June 1996 to August 1997. Symptoms resolved after the first session in 49 patients (72%) and after the second session in 11 cases (16.2%). It was necessary to repeat the maneuver three times in two cases (2.9%) and four times in one patient (1.5%). Discounting three patients who were lost to follow-up, only two patients in our…

The importance of magnetic resonance imaging in the evaluation of vertigo and imbalance.

Vertigo and imbalance are believed to be rare manifestations of skull base neoplasms. Patients with skull base neoplasms can present with vague otolaryngological complaints, including diplopia, facial numbness, facial weakness, hearing loss, tinnitus, hoarseness, headache, and otalgia. Physical examination of these patients can sometimes reveal paralysis or paresis of cranial nerves. Magnetic resonance imaging (MRI) is the gold standard for evaluation of cranial nerve involvement in skull base diseases. Vertigo and imbalance can be manifestations of a neuropathy or lesion within the vestibular…

Surgical treatment of vertigo by utriculostomy: an experimental study in sheep.

The objective of this study was to develop and test an endolabyrinthic microsurgical procedure for treatment of vertigo, the utriculostomy. This involves the application of local heat for obtaining a fistula in the membranous labyrinth, so as to establish communication between the endolymphatic and perilymphatic spaces at the utricle level. Before the procedure, an experimental model using quail eggs was built for pre-evaluation, and macroscopic and histological studies were performed in the temporal bones of three healthy sheep. Following this, the utriculostomy was performed through the oval…

The Epley maneuver for the treatment of benign paroxysmal positional vertigo.

In recent years, different forms of physical therapy have been proposed for the treatment of benign paroxysmal positional vertigo (BPPV). These mainly consist of maneuvers aiming to reposition and disperse free-floating endolymph particles in the posterior semicircular canal. We report our experience with one of these procedures, the Epley maneuver (EM), in treating 30 cases of BPPV. Twenty-six patients (87%) were cured, while four (13%) did not respond to treatment. These results are similar to those reported in the literature for the EM and the Semont maneuver.

Isolated acute vertigo in the elderly; vestibular or vascular disease?

INTRODUCTION: Elderly patients with isolated acute vertigo are commonly encountered in clinical practice, but little is known about the underlying cause of the symptoms. MATERIAL & METHODS: We prospectively studied 24 patients aged 50-75 years with the acute onset of isolated vertigo lasting > 48 h and no abnormality on neurological examination other than nystagmus. The study protocol included neuro-imaging (MRI 22 patients, CT 2 patients), Doppler sonography, and electro-oculography. RESULTS: MRI/CT showed the presence of an infarction of the caudal cerebellum in six patients (25%), 3 of wh…

Rotational vertigo in embolic stroke of the vestibular and auditory cortices.

We describe the unique case of a woman with embolic infarction within the right middle cerebral artery territory who had nonepileptic rotational vertigo, nausea, and unsteady gait that gradually resolved within a week. A well-demarcated lesion of the posterior insula (homologous to parieto-insular vestibular cortex in monkeys) was probably the cause. This is supported by a contraversive tilt of perceived vertical and by involvement of the adjacent auditory (Heschl's) cortex. Dipole source analysis of late auditory evoked potentials revealed a decreased amplitude of a dipole source in the right…

The clinical efficacy of Vertigoheel in the treatment of vertigo of various etiology.

In this paper the authors describe the clinical efficacy in treatment of vertigo of various etiology. A group of 31 patients were treated with Vertigoheel medication: 14 patients suffered from vertebrobasilar arterial insufficiency, 8 patients were diagnosed as Meniere's disease, 5 patients complained of vertigo of traumatic origin and 4 patients suffered from neuronitis vestibularis. The authors found regression of clinical symptoms in the majority of cases in the investigated group who were treated with Vertigoheel.

An expert system for the computer-aided diagnosis of dizziness and vertigo.

We have developed an expert system to assist in the diagnostic work-up of otoneurological cases. Our otoneurological expert system ONE takes advantage of both patient history and clinical measurement data in order to supply all possible information about the patient's symptoms and other findings. This paper presents ONE after its initial stage of development, which included tests with numerous patients.

[Cough headache and vertigo as symptoms of the Arnold-Chiari syndrome].

Headache with paroxysmal vertigo were induced by cough in a case of the Arnold-Chiari type I malformation. The authors suggest that exertional headache and posterior cranial fossa symptoms of unclear aetiology may indicate presence of the syndrome, and the availability of magnetic resonance imaging made possible the detection of the malformation.

Investigations into the cause of vertigo in sudden sensorineural hearing loss.

In this light microscopic study of the temporal bone, an attempt has been made to find a morphologic correlate of vertigo associated with idiopathic sudden sensori-neural hearing loss (ISSHL). Hair cell densities of the three cristae and both maculae, as well as vestibular ganglion cell (neuronal) count estimation, was done in nine ears that had documented histories of ISSHL. There were five ears with vertigo and four without. These quantitative data--i.e., hair cell densities and neuronal counts, of the vertiginous ears (group I) and nonvertiginous ears (group II)--was compared by histograms …

[Use of SPECT in the diagnosis of vertigo syndromes of vascular nature].

Dizziness is doubtlessly one of the most common symptoms to arise in ischemia of the brainstem. In such cases the circulatory deficit can not only cause a direct lesion of the vestibular structures but it may also block the compensatory process. There are, however, significant difficulties in establishing whether such dizziness can be attributed to a brainstem insufficiency (BI). In fact, both CAT and NMR provide data only in the case of permanent CNS tissue lesions and tests such as the Doppler examination of neck blood vessels are unable to establish the true state of cerebral blood flow. In

Benign paroxysmal positional vertigo as a complication of postoperative bedrest.

Seven cases of benign paroxysmal positional vertigo (BPPV) after operation were reported, occurring between the fourth and 14th postoperative day. In three of the seven cases, the condition occurred when the patients first arose from bed after 5 to 6 days of postoperative bedrest. In two cases, BPPV occurred when patients moved their head while maintaining strict bedrest. In the other two cases, it occurred 1 to 4 days after patients had been released from bedrest. The cause of BPPV was considered to be postoperative bedrest which might have facilitated the deposition of precipitate on the cup…

The use of vestibular tests for the selection of anti-vertigo drugs.

Vertigo is not a disease, but a symptom. Therefore, the therapy of vertigo is primarily directed at eliminating the underlying cause. In about half of the cases the cause will not become apparent, however, and the therapy is doomed to be symptomatic. Several types of drugs have been found to have an anti-vertigo activity, e.g. antihistamines, calcium antagonists, anti-emetics. For the selection of vestibular depressants, the vestibular caloric test and the rotation test can be used. The duration of the nystagmus induced by rotation is the most useful parameter. The lag-period, the extent and d…

Astemizole. Its use in the treatment of patients with chronic vertigo.

Thirty-eight patients with chronic vertigo and positional and/or spontaneous nystagmus finished a double-blind study to determine the efficacy of astemizole on nystagmus and its symptoms. Patients with Meniere's disease were excluded. The study took 13 weeks with a follow-up examination. Six to eight noncaloric electronystagmograms were recorded at determined intervals. Three doses of the drug, 5, 10, and 20 mg/d, and placebo were tested. A patient was considered a responder if the number of nystagmic beats occurring in eight head and body positions was reduced by 50% or more and the symptoms …

[Therapy in neurology. Part 8: Diagnosis and treatment of vertigo].

Acute vertigo episodes in patients suffering from Menière's disease, vestibular neuropathy or ischaemic attacks in the vertebrobasilar system are reason for hospitalization of these patients, if possible in intensive care units. Improvement of microperfusion, administration of antivertigenous drugs and digitalization are most important. Usual clinical symptoms are spontaneous nystagmus and ataxia, seen only if the patient is able to be out of bed. Cranial computed tomography, x-ray examinations of the skull and an internal check-up are basis for differential diagnosis and treatment is necessar…
