Superior Canal Dehiscence Syndrome: Relating Clinical Findings With Vestibular Neural Responses From a Guinea Pig Model.

HYPOTHESIS: In superior canal dehiscence (SCD), fluid displacement of the endolymph activates type I vestibular hair cells in the crista of the affected canal and thus irregular superior canal (SC) neurons in Scarpa's ganglion, which provides the neurophysiological basis for the clinical presentation of SCD. BACKGROUND: Patients with SCD display sound- and vibration-induced vertigo/nystagmus and increased amplitudes of vestibular evoked myogenic potentials. METHODS: Extracellular recordings from n = 25 primary vestibular neurons of 16 female guinea pigs were analyzed. We recorded from the sa

Utricular paresis and semicircular canal hyperactivity: a distinct otolith syndrome.

CONCLUSIONS: Although combined utricular and canal paresis has been described previously, this is the first report of canal hyperactivity associated with utricular hypofunction. Unsteadiness and swaying were the most common symptoms, and patients with shorter duration of symptoms also had positional vertigo. We propose that this syndrome is a variant of utricular dysfunction and should be considered in the differential diagnosis of peripheral vestibular disorders. OBJECTIVE: To describe a syndrome of instability associated with utricular dysfunction and hyperactive caloric responses. METHODS

Why and when to refer patients for vestibular evoked myogenic potentials: A critical review.

Cervical and ocular vestibular evoked myogenic potentials (cVEMPs and oVEMPs respectively) are now used by an increasing number of laboratories to evaluate otolith inner ear function and their pathways through the central nervous system. However, the literature is incomplete or unclear as to what information both c- and oVEMPs can add beyond what a good clinical examination can provide, and what other paramedical tests can provide also, and the present review aims to clarify what is known so far. The following review will describe what is known with regards to both c- and oVEMPs and their use.

Retinoic acid deficiency impairs the vestibular function.

The retinaldehyde dehydrogenase 3 (Raldh3) gene encodes a major retinoic acid synthesizing enzyme and is highly expressed in the inner ear during embryogenesis. We found that mice deficient in Raldh3 bear severe impairment in vestibular functions. These mutant mice exhibited spontaneous circling/tilted behaviors and performed poorly in several vestibular-motor function tests. In addition, video-oculography revealed a complete loss of the maculo-ocular reflex and a significant reduction in the horizontal angular vestibulo-ocular reflex, indicating that detection of both linear acceleration and

Effects of visual input on galvanic body sway test of unilateral vestibular deficiency in patients with unilateral vestibular schwannoma.

OBJECTIVE: Galvanic body sway tests (GBSTs) are performed with eyes fixed and closed. However, quantitative effect of fixation on GBST has been unknown. The purpose of this study is to address this question. METHODS: We evaluated GBSTs of patients with unilateral vestibular schwannomas and normal controls, while their eyes were open and closed. We evaluated three GBST parameters: maximum amplitude of the response, velocity and latency of the onset of response. RESULTS: Closing the eyes diminished stability, resulting in increased amplitude and velocity of the responses. However, apparent con

Evaluation of Semicircular Canal Function Using the Video Head Impulse Test in a Case of Relapsing Polychondritis and Otitis Media With Effusion.

Relapsing polychondritis (RP) is a rare, immune-mediated disorder characterized by recurrent inflammation of cartilaginous structures, often affecting the ears, nose, and respiratory tract. Vestibular dysfunction in RP is uncommon but clinically significant, resulting from inner ear involvement. We describe a case of an 81-year-old man with RP who presented with fever, bilateral hearing loss, vertigo, and ocular inflammation. Bilateral otitis media with effusion (OME) was identified on imaging and otoscopy, precluding the use of caloric testing. Video head impulse test (vHIT) revealed bilatera

A 1-year follow-up study with C-VEMPs, O-VEMPs and video head impulse testing in vestibular neuritis.

The aim of this paper was to evaluate prospectively, in a group of patients affected by VN, a diagnostic protocol employing C-VEMPs, O-VEMPs and vHIT together. The diagnosis of vestibular neurolabyrinthitis was based on the clinical history, absence of associated auditory or neurological symptoms, and a neuro-otological examination with an evaluation of lateral semicircular canal function using the Fitzgerald-Hallpike caloric vestibular test and ice test. Our series revealed an incidence of 55% of superior and inferior vestibular neurolabyrinthitis, 40% of superior vestibular neurolabyrinthiti

Vestibular information is necessary for maintaining metric properties of representational space: evidence from mental imagery.

The vestibular system contributes to a wide range of functions, from postural and oculomotor reflexes to spatial representation and cognition. Vestibular signals are important to maintain an internal, updated representation of the body position and movement in space. However, it is not clear to what extent they are also necessary to mentally simulate movement in situations that do not involve displacements of the body, as in mental imagery. The present study assessed how vestibular loss can affect object-based mental transformations (OMTs), i.e., imagined rotations or translations of objects r

Brainstem auditory evoked response in the diagnosis of inner ear injury in the horse.

Brainstem auditory evoked response (BAER) testing was done to evaluate inner ear/VIIIth cranial nerve (CN8) function in the horse. The BAER test consisted of stimulating the auditory system with clicks and recording far-field responses of the brainstem auditory components via cutaneous electrodes and a signal averaging system. The normal response was shown to be a series of waves occurring within the first 10 msec after the stimulus click. Functional loss of the auditory receptor organ (cochlea) or CN8 results in loss of the entire response on the side of the injury. Because of the anatomic re

Investigation of vestibular damage by antituberculous drugs.

Vestibular function testing was performed regularly on patients who were administered streptomycin, kanamycin, or enviomycin, and vestibular damage was detected at an early stage, and quantitatively. We investigated the point in time at which the therapy should be discontinued. Subjects consisted of 204 cases of tuberculosis treated with streptomycin, kanamycin, enviomycin. They were admitted to the hospital between December 1984 and August 1989. Twenty-eight cases of vestibular dysfunction due to streptomycin, kanamycin, and enviomycin could easily be detected at an early stage by performing

Semicircular Canals Input Can Modify the Fast-Phase Nystagmus in Off-Vertical Axis Rotation of Mice.

Vestibular research is essential for understanding and treating disorders such as vertigo and Meniere's disease. The vestibulo-ocular reflex (VOR) is a key method for assessing vestibular function and an essential tool for diagnosing vertigo. Traditionally, the VOR comprises angular VOR (aVOR) and translational VOR (tVOR), which originate from the vestibular semicircular canals (SCCs) and otolith organs, respectively. VOR consists of both fast-phase and slow-phase eye movements, which functionally interact to contribute to gaze control. However, to calculate the gain and phase parameters of th

Vestibular schwannoma: negative growth and audiovestibular features.

At the University Medical Center Utrecht, non-operative management was used for 44 patients with a unilateral vestibular schwannoma between 1990 and 1997. During that period, consecutive tumor sizes were determined by magnetic resonance imaging. Three of the 44 patients showed an average decrease in tumor size of 16.7% according to American Academy of Otolaryngology-Head and Neck Surgery standards. This study describes the initial vestibular status and audiometric changes measured over up to 10 years in these three patients. Vestibular function was determined once, by means of the bithermal ca

Vestibular function in children with the CHARGE association.

BACKGROUND: Histopathological examinations and computed tomographic scans of the temporal bone in patients with the CHARGE association (a malformative syndrome that includes coloboma, heart disease, choanal atresia, retarded development, genital hypoplasia, and ear anomalies, including hypoplasia of the external ear and hearing loss) have shown an absence of semicircular canals and a Mondini form of cochlear dysplasia. Until recently, no information was available concerning a possible loss of vestibular function, which could be a factor in retarded posturomotor development. To our knowledge, t

Vestibular evaluation with video head impulse test in pediatric cochlear implant patients.

BACKGROUND: The cochlear implant (CI) is effective for rehabilitating patients with severe to profound sensorineural hearing loss. However, its placement and use have been associated with various complications, such as those affecting the vestibular system. The objective of this study was to compare vestibular function using the video head impulse test (vHIT) in pediatric patients before and after CI placement. METHODS: A descriptive and retrospective study was conducted. The outcomes of 11 pediatric patients of both sexes with a history of profound hearing loss were evaluated. The results of

Long-term clinical outcome in vestibular neuritis.

PURPOSE OF REVIEW: To review recent work on clinical and imaging aspects of vestibular neuritis (or acute vestibular syndrome), in particular with a view to identifying factors predicting long-term clinical outcome. RECENT FINDINGS: Evidence for a role of inflammation in the vestibular nerve, and the presence of Gadolinium enhancement acutely in vestibular neuritis, is accruing. Visual dependence, anxiety and somatization traits predict the development of chronic dizziness after acute vestibular neuritis. Adaptation to asymmetric rotation is impaired in vestibular neuritis and this may indica

Cervical vestibular evoked myogenic potentials and caloric test results in individuals with auditory neuropathy spectrum disorders.

Auditory neuropathy spectrum disorder is a type of hearing loss where outer hair cell function are normal (as evidenced by the preservation of OAEs and cochlear microphonics), whereas auditory nerve functions are abnormal (as evidenced by abnormal auditory brainstem evoked potentials beginning with wave I of the ABR) and acoustic reflexes to ipsilateral and contralateral tones are absent. It is likely that in cases with auditory neuropathy spectrum disorder not only the cochlear nerve, but also the vestibular nerves might get involved. The present study was conducted with an aim of finding out
