Antibiotics and nasal surgery.

Despite the widespread use of prophylactic antibiotics in nasal surgery, the subject has been paid little attention in the literature. In a retrospective analysis of 210 patients who underwent septal surgery or a rhinoplasty, only 5 developed minor infections. Patients undergoing septal surgery or rhinoplasty (174) were evaluated in a randomized sequence, prospectively, to study the efficacy of systemic antibiotic prophylaxis. Only 4 patients developed minor infections; and 173 patients demonstrated no clinical nor radiographic evidence of subsequent sinusitis. The incidence of infectious comp

Vertigo after head injury--a five year follow-up.

Three hundred and twenty-one head injury patients investigated at the Workmen's Compensation Board Hospital and Rehabilitation Centre were studied. The patients were classified into two groups, minor and moderate according to the duration of post-traumatic amnesia. Post-traumatic vertigo was a significant symptom in 34 per cent and 50 per cent of the minor and moderate groups respectively. Based on the findings of full otoneurologic and vestibular examination, objective vestibular disorder was noted in 40 per cent and 65 per cent of the two vertiginous groups respectively. An approach to the i

Primary solitary intralabyrinthine schwannoma: A report of 7 cases and a review of the literature.

Intralabyrinthine schwannomas (ILSs) are uncommon benign tumors that originate in the Schwann cell sheath of the intralabyrinthine distal branches of the vestibulocochlear nerve. They have no initial involvement in the internal auditory canal, although that might develop later. These lesions can arise inside the cochlea, originate in the vestibule or, in rare cases, develop in the semicircular canals. From these sites, spread might take place via the anatomic connections between the perilymphatic spaces in the scala vestibuli and the anterior vestibule. Thus, ILSs centered in the cochlea might

Vestibular evoked myogenic potentials in patients with ankylosing spondylitis.

Ankylosing spondylitis (AS) is a chronic systemic inflammatory disease with unknown etiology. Although sacroiliac joint involvement is the classic sign along with the formed immune mediators, it may result in immune-mediated inner ear disease and may cause damage to the audiovestibular system. Vestibular evoked myogenic potentials (VEMP) is a clinical reflex test used in the diagnosis of vestibular diseases and is performed by recording and evaluating the muscle potentials resulting from the stimulation of the vestibular system with different stimuli. The aim of this study is to evaluate the c

Applications of magnetic resonance imaging in otology.

Magnetic resonance (MR) has been employed for the study of patients with peripheral and retrolabyrinthine otologic pathology. Its usefulness has been compared with that of computed tomography (CT). CT remains at present the best study for the assessment of patients with conductive hearing loss and sensorineural hearing loss or vestibular disorders of the peripheral type. However, MR is the study of choice of retrolabyrinthine pathology occurring in the internal auditory canals, cerebellopontine cisterns, and brainstem.

History taking in dizziness. The most important diagnostic step.

When faced with a complaint of dizziness, the primary care physician may be tempted to assume that the cause is inner ear dysfunction and that the patient needs to be referred to an otologist. According to Dr Hybels, however, most dizzy patients do not have an ear problem and a carefully executed history is likely to pinpoint the correct diagnosis. The following article delineates the fine points of history taking and discusses some of the most common causes of dizziness.

Evaluation of audiovestibular status in leprosy.

One hundred twenty five cases of lepromatous leprosy and 25 cases of tuberculoid leprosy were investigated for audiovestibular status. Impaired hearing was detected in 52 percent and vestibular hypofuction in 7.2 per cent of lepromatous cases. Conductive deafness was due to eustachian tube catarrh secondary to atrophic rhinitis associated with the disease. The perceptive deafness and vestibular hypofuction were due to end organ lesion probably due to E.N.L. reaction. The vestibulococlear nerve involvement was considered to be unlikely. In tuberculoid leprosy derangement in hearing was not obse

Positional vertigo and nystagmus of central origin.

Four cases are described illustrating the clinical features of positional vertigo and nystagmus due to posterior fossa tumors and a case of obstructive hydrocephalus. In these cases positional vertigo was the first and only presenting symptom of central nervous system disease. One case of subependymoma of the fourth ventricle and one with hydrocephalus had characteristic symptoms of benign positional vertigo; each showed positional nystagmus of the benign paroxysmal type.

[The diagnostic value of vertical nystagmus in the simultaneous bilateral calibrated caloric vestibular test (author's transl)].

Vertical nystagmus provoked by simultaneous bilateral caloric stimulation has been known since 1907 (Bàràny). However, if a controlled and calibrated injection os water at 44 degrees C or 30 degrees C for 30 seconds at a flow rate of 250 ml/minute is given in normal subjects, this stimulus is insufficient to provoke a response, whilst in subjects with a central vestibular disorder there appear upward vertical movements with hot stimulation and downward movements with cold stimulation. The degree of this response is proportional to the degree of horizontal nystagmic responses to classical unila

Vestibular syndrome and vascular anomaly in the cerebello-pontine angle.

During the investigation of patients presenting symptoms related to the Vth and VIIth nerves, frequently associated symptoms relevant to the VIIth nerve were found, such as tinnitus, disequilibrium or vertigo. On the other hand, there also came to our attention patients whose major complaints were of VIIIth origin, especially of the vestibular component, and who occasionally presented associated symptoms of Vth or VIIth nerve involvement. Following the successful treatment of trigeminal neuralgia and hemi-facial spasm by liberation of the Vth and the VIIth nerve from the mechanical irritative

Multi-sequence MRI signal intensity ratios as candidate imaging correlates for audiovestibular dysfunction in vestibular schwannoma.

INTRODUCTION: Vestibular schwannoma (VS) typically induces gradual sensorineural hearing impairment and vestibular dysfunction. Nevertheless, clinically applicable non-invasive candidate imaging markers capable of reflecting pathological alterations within the inner ear have not yet been well established. The present study sought to systematically investigate the correlative relationships between labyrinth signal intensity ratios acquired from multi-sequence inner ear magnetic resonance imaging (MRI) and audiovestibular functional deficits in patients diagnosed with VS. METHODS: A total of 22

Cytomegalovirus-induced pathology in human temporal bones with congenital and acquired infection.

OBJECTIVE: Publications on histopathology of human temporal bones with cytomegalovirus (CMV) infection are limited. We aim to determine histopathology of the inner ears and the middle ears in human temporal bones with congenital and acquired CMV infections. METHODS: Temporal bones from 2 infants with congenital and 2 adults with acquired CMV infection were evaluated by light microscopy. RESULTS: Two infants with congenital CMV infection showed striking pathological changes in the inner ear. There was a hypervascularization of the stria vascularis in the cochlea of the first infant, but no ob

Velocity Storage Integrator (VSI): Mechanisms, circuits, and clinical implications A narrative review.

The velocity storage integrator (VSI) is a central, gravity-referenced neural network that prolongs and spatially orients vestibular/optokinetic signals beyond semicircular-canal dynamics. Classical systems analyses framed it as a leaky integrator in an indirect pathway, complementing a high-frequency direct pathway that rapidly stabilizes gaze. Converging unit recordings, lesions, and modeling now localize the spatiotemporal core of VSI to commissural vestibular-nuclei networks-particularly vestibular-only (VO) and vestibular-pause-saccade (VPS) populations-under cerebellar nodulus-uvula (NU)
