Mechanisms of repigmentation induced by photobiomodulation therapy in vitiligo.

Photobiomodulation (PBM) therapy is based on the exposure of biological tissues to low-level laser light (coherent light) or light-emitting diodes (LEDs; noncoherent light), leading to the modulation of cellular functions, such as proliferation and migration, which result in tissue regeneration. PBM therapy has important clinical applications in regenerative medicine. Vitiligo is an acquired depigmentary disorder resulting from disappearance of functional melanocytes in the involved skin. Vitiligo repigmentation depends on available melanocytes derived from (a) melanocyte stem cells located in…

IL-17A is not a treatment target in progressive vitiligo.

Multiple reports confirm elevated circulating IL-17 levels and increased numbers of Th17 lymphocytes in patients with non-segmental vitiligo. Additionally, melanocyte damaging characteristics have been ascribed to IL-17. A single-arm pilot study using secukinumab in active non-segmental vitiligo was conducted. The large majority of patients developed additional skin depigmentations limiting further enrollment. Overall, laboratory analysis revealed no change in secreted chemokines or Th subsets. Th17 lymphocytes correlated with Th2, Th9, and Th22 cells while an inverse link with Th1 cells and s…

Identification of pathogenic genes and transcription factors in vitiligo.

Our study aimed to identify the key genes and upstream regulators in vitiligo. To screen the pathogenic genes of vitiligo, an integrated analysis was performed by using the microarray datasets in vitiligo derived from the Gene Expression Omnibus (GEO) database. The functional annotation and potential pathways of differentially expressed genes (DEGs) were further explored by Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses. We constructed a vitiligo-specific transcriptional regulatory network to identify crucial transcriptional factors that target the DE…

[Prevalence of an association between coeliac disease and vitiligo].

Coeliac disease and vitiligo are immune-mediated disorders that are often associated with other immune-mediated disorders. In a prospective study we included 174 patients with vitiligo between the ages of 3 and 79 years (mean 38.2 years) to investigate whether there is an increased risk for coeliac disease in patients with vitiligo. We determined immunoglobulin A and IgA- and IgG-antibodies against tissue transglutaminase, while also optionally measuring blood count, ferritin, and endomysial-IgA-antibodies. In 3 of 174 (1.7%) vitiligo patients, coeliac disease was diagnosed serologically and b…

Re-appraisal of Keratinocytes' Role in Vitiligo Pathogenesis.

BACKGROUND: Vitiligo is a common pigmentary disorder. Studies on its pathogenesis extensively investigated melanocytes' abnormalities and few studies searched for keratinocytes' role in disease development. Liver X receptor-α (LXR-α) is a member of nuclear hormone receptors that acts as a transcription factor. Its target genes are the main regulators of melanocyte functions. AIM: The aim of this study is to investigate keratinocytes' role in vitiligo pathogenesis through immunohistochemical expression of LXR-α in lesional, perilesional, and distant nonlesional vitiligo skin. MATERIALS AND ME…

IMMUNOHISTOCHEMICAL STUDY OF DEPIGMENTED SKIN OF VITILIGO PATIENTS.

Immunohistochemical analysis was used to study depigmented skin areas such as macular of depigmentation and skin perimakular areas in vitiligo patients. It has been shown that the cells containing melanocytic cell marker TRP1 are localized both in macular and perimakular areas. Within the macula of depigmentation all TRP1 positive cells are in close contact with the basement membrane. In perimakular areas many cells that have lost contact with the basement membrane, were localized deep in the epidermis. About 92 % of TRP1 positive perimakular cells were also vimentin positive. Vimentin positiv…

Evaluation of the serum zinc level in patients with vitiligo.

INTRODUCTION: Vitiligo is an acquired, idiopathic disorder characterized by circumscribed depigmented macules and patches, which affects approximately 0.1-2% of the general population worldwide. Zinc is an essential trace element that is necessary for growth and development at all stages of life. Some studies have reported an association between serum zinc levels and vitiligo. AIM: To measure the serum zinc level in patients with vitiligo compared to healthy subjects. MATERIAL AND METHODS: One hundred patients with vitiligo and 100 healthy controls were referred to our clinic. The two groups…

Interleukin-22 participates in the inflammatory process of vitiligo.

Vitiligo is an acquired depigmentary skin inflammatory disorder. The pathogenesis of inflammatory skin disease involves the release of cytokines from keratinocytes, including interleukin (IL)-1β. IL-22 belongs to a family of cytokines structurally related to IL-10, including IL-19, IL-20, IL-24, and IL-26. In contrast to IL-10, IL-22 has proinflammatory activities. Among skin cell populations only keratinocytes are the major targets of IL-22. In the present study, we demonstrated that IL-22 promoting IL-1β secretion from keratinocytes via the Reactive oxygen species (ROS)-NOD-like receptor fam…

[ELEMENTAL STATUS OF PATIENTS WITH VARIOUS FORMS OF VITILIGO].

Vitiligo is a multifactorial disease in which, in each specific case of its manifestation, different mechanisms of its pathogenesis and different levels of melanin formation in the skin can be involved. Skin is one of the most metabolically active organs. Carrying out a number of vital functions (barrier, protective, respiratory, excretory, metabolic, immune, etc.), it needs microelementss. Of the 92 naturally occurring chemical elements, 81 are found in the human body. Lack of the vital elements, leads to the emergence of diseases, which are based on deficiency, excess or imbalance of micro- …

Prevalence of Thyroid Abnormalities in Thai Patients with Vitiligo.

BACKGROUND: Vitiligo is an acquired hypopigmentary disorder. The prevalence of vitiligo is 0.1-2% worldwide. Numerous autoimmune diseases are associated with vitiligo, including autoimmune thyroid diseases. The prevalence of thyroid abnormalities is up to 34% in vitiligo patients depending on ethnicities. OBJECTIVE: This study aims to investigate thyroid abnormalities in Thai patients with vitiligo. METHODS: Medical records of vitiligo patients attending outpatient dermatology clinic at a university-based hospital from 2012 to 2016 were retrospectively reviewed. Data regarding vitiligo, clin…

Presentations, Signs of Activity, and Differential Diagnosis of Vitiligo.

Vitiligo has a variety of presentations, including focal, acrofacial, segmental, and generalized forms. Thorough knowledge of these presentations is important to make the correct diagnosis. Signs of activity are important to recognize so that treatment is optimized. Clinical findings of confettilike depigmentation, trichrome and inflammatory vitiligo, and the Koebner phenomenon should alert the clinician that a patient's disease is likely to worsen. These patients may require systemic treatment to stabilize their disease. Many other skin disorders present with hypopigmentation or depigmentatio…
