Missed total occlusion due to the occipital artery arising from the internal carotid artery.

A 56-year-old man was referred for digital subtraction angiography (DSA) with an ultrasound diagnosis of right proximal internal carotid artery (ICA) stenosis for possible carotid artery stenting. DSA revealed total occlusion of the ICA and an occipital artery arising from the stump and simulating continuation of the ICA. An ascending pharyngeal artery also arose from the same occipital artery. This case is of interest because this is a rare variation besides being a cause of misdiagnosis at carotid ultrasound.

Double stenting procedure and coil embolization in a patient with carotid stenosis and incidental ipsilateral intracranial aneurysm. A case report and dosimetric evaluation.

We describe a case of incidental detection of an intracranial left ICA wide-necked aneurysm during digital subtraction angiography performed to assess a sub-occlusive and calcified stenosis in the extracranial portion of the same artery. Angioplasty and stenting of ICA stenosis, plus intracranial stent deployment across the aneurysm neck was performed during the same procedure. Aneurysm coil embolization was postponed to a further session one month later. The radiation dose and irradiated areas were also evaluated during endovascular procedures.

[Traumatic basilar-cavernous fistula complicated by basilar artery aneurysm: report of one case].

A 19-years-old boy was admitted after brain trauma with subarachnoid hemorrhage, left oculomotor nerve palsy, elevated pressure in the cavernous sinus and intracranial noise. The diagnosis of traumatic basilar-cavernous fistula complicated with basilar artery aneurysm was established by digital subtraction angiography. After endovascular occlusion of the vessels twice using detachable coils, the patient was cured without relapse during the 2-year follow-up. It is at the time being second report globally and the first one in China.

Aneurysm of aberrant right subclavian [corrected] artery arising from diverticulum of Kommerell. Report of a case with tracheal compression.

A 74-year-old woman presented with severe dyspnea without dysphagia. Computed tomographic scans and Digital subtraction angiography revealed the left aortic arch with an aberrant right subclavian artery arising from the Kommerell's diverticulum and tracheal compression. The aortic arch and the Kommerell's diverticulum were aneurysmal and were responsible for this compression. Surgical relief was accomplished by replacement of the aortic arch and reconstruction of the four brachiocephalic vessels with vascular prostheses through a median sternotomy incision extending into the right supraclavicu

Acute posttraumatic rupture of the thoracic aorta: the role of angiography in a 7-year review.

Between 1983 and 1989, 15 patients with acute rupture of the thoracic aorta by blunt trauma were seen. Superior mediastinal widening and obscuration of the aortic arch were the most important findings on chest radiograph. Computed tomography examinations in 7 patients showed mediastinal hematomas but did not reveal aortic lesions. Definitive diagnosis of traumatic aortic rupture was established by aortography in all 15 patients. Intraarterial digital subtraction angiography proved to be as accurate as conventional film aortography and saved time.

Basilar artery ectasia: an unusual cause of trigeminal neuralgia.

Basilar artery ectasia may be associated with trigeminal neuralgia as a consequence of trigeminal nerve compression by the aberrant artery. The case histories are presented of four patients, investigation of whom demonstrated the presence of an ectatic basilar artery thought to be the cause of facial pain. The value of intravenous digital subtraction angiography in the investigation of this condition is emphasised. The surgical management is discussed with particular reference to the choice between percutaneous thermocoagulation and posterior fossa microvascular decompression.

[Rational diagnosis of traumatic aortic rupture].

The traumatic aortic rupture has an extremely high spontaneous mortality rate. Important for surviving the trauma is a fast and efficient diagnostic procedure. The suspected diagnosis results from relatively typical criteria in the chest radiograph, especially the continuous widening of the mediastinum. The diagnostic procedure of 26 patients with polytrauma, abnormal chest radiograph and further examinations (intra-arterial digital subtraction angiography [i.a.DSA]: n = 10, contrast-enhanced CT: n = 19, CT and DSA: n = 5) was reviewed. Six patients had an aortic rupture. In 4 patients the dia

Rare Sighting of Posterior Communicating Artery Infundibulum With Posterior Cerebral Artery Origin.

Infundibula are funnel-shaped lesions that develop at the intersections of major intracranial arteries. These lesions are prone to being misdiagnosed as intracranial aneurysms. The most common arterial infundibula have been noted in the posterior communicating artery (PCoA) branch of the internal carotid artery (ICA). Digitally subtracted angiography performed included catheter angiography of the vertebral artery and ipsilateral carotid to evaluate the suspected lesion. Right vertebral angiography demonstrated an infundibulum seen at the right PCoA/posterior cerebral artery (PCA) junction, wit

Vertebrojugular Arteriovenous Fistula Following Failed Central Venous Catheter Insertion: A Rare Case.

Vertebrojugular arteriovenous fistulas (AVFs) are rare vascular anomalies characterized by an abnormal communication between the extracranial vertebral artery and the internal jugular vein. They may arise from trauma, iatrogenic injury, or, rarely, occur spontaneously. We present a 50-year-old male who sustained an iatrogenic injury and developed progressive pulsatile swelling on the right side of the neck, which raised suspicion of an underlying vascular lesion. Subsequent carotid Doppler, MRI with non-contrast angiography, and digital subtraction angiography revealed a vertebrojugular AVF. T

Preoperative Imaging for Facial Transplant: A Guide for Radiologists.

Facial transplant (FT) is a viable option for patients with severe craniomaxillofacial deformities. Transplant imaging requires coordination between radiologists and surgeons and an understanding of the merits and limitations of imaging modalities. Digital subtraction angiography and CT angiography are critical to mapping vascular anatomy, while volume-rendered CT allows evaluation of osseous defects and landmarks used for surgical cutting guides. This article highlights the components of successful FT imaging at two institutions and in two index cases. A deliberate stepwise approach to perfor

Evaluation of characteristics of left-sided colorectal perfusion in elderly patients by angiography.

BACKGROUND: Handling of the inferior mesenteric artery (IMA) and maintaining anastomotic perfusion are important in radical resection of left-sided colorectal cancer. However, the branching of this artery and the drainage patterns of this vein vary among individuals, and the characteristics and perfusion region of this artery in elderly patients remain unclear. AIM: To evaluate the characteristics and perfusion region of the IMA in elderly patients using angiography. METHODS: We enrolled 154 patients (> 65 years old) who underwent digital subtraction angiography of the IMA. The characteristi

Physiological provocation compared to acetazolamide in the assessment of cerebral hemodynamics: a case report.

BACKGROUND: Severe large vessel disease may lead to cerebral hemodynamic failure that critically impairs cerebral blood flow (CBF) regulation elevating the risk of ischemic events. Assessment of the condition is often based on changes in CBF during vasodilatation; however, pharmacologically induced vasodilation does not reflect the physiological condition during an ischemic event caused by hemodynamic failure. We compared a [15O]H2O PET brain scan during vasodilation to a [99mTc]HMPAO SPECT brain scan during an ongoing transient ischemic attack (TIA). CASE PRESENTATION: A single patient prese

Accuracy of dual-bolus CT in the diagnosis of active arterial bleeding in adult pelvic trauma.

INTRODUCTION: A single-phase dual-bolus CT (DB-CT) simultaneously opacifies both arterial and venous systems and can be utilised in the trauma setting to aid in the diagnosis of active bleeding while also allowing for optimal assessment of the abdominal and pelvic viscera. Active bleeding can be venous or arterial, the latter being amenable to angiography and potentially embolisation. We aimed to establish the accuracy of single-phase DB-CT vs commonly performed portal venous CT (PV-CT) in the diagnosis of active bleeding when compared to formal digital subtraction angiography as the gold stan

Radiation exposure reduction in peripheral interventions using digital variance angiography versus conventional angiography.

Digital variance angiography (DVA) exhibits promising prospects with respect to radiation exposure in digital subtraction angiography (DSA). This study aimed to determine the reduction of radiation dose in endovascular peripheral interventions (EPI) using DVA. The DVA imaging tool v6.0 (Kinepict Medical Imaging Tool, version 6.0.4, Kinepict Health Ltd., Budapest, Hungary) was utilized for patients undergoing EPI using digital angiography. EPI normal dose (EPI-ND) protocols were adapted from 1.20 to 0.81 µGy/frame to EPI low dose (EPI-LD) protocols using DVA-LD acquisitions with 0.36 µGy/frame

Renovascular hypertension: a non-invasive Duplex scanning screening.

A Duplex-Scanner was used to screen 62 patients with severe hypertension, queried renovascular. Ten results were technically unsatisfactory and in the remaining 52 highlighted flow alterations in the renal arteries of 29 patients were found. Control angiographies confirmed renal artery stenosis in 27 cases. Comparative statistical analyses proved the test to be both highly sensitive and specific, considering other screening methods currently used in diagnosing renovascular hypertension--which include endovenous digital subtraction angiography. Results confirmed the Duplex-Scanner as a reliable
