[Vesico-uterine fistula occurring after a normal labor in a patient with a scarred uterus].

We report a vesico-uterine fistula occurring after vaginal delivery in a woman with previous cesarean section. Diagnosis of this rare complication, only 2 cases reported in the literature, is based on clinical findings and retrograde cystography. Medical treatment should be attempted first, but laparotomy is often required for resection of the fistula and simple suture of the bladder and the uterus.

Trimethoprim susceptibility in E. coli community-acquired urinary tract infections in France.

OBJECTIVES: Trimethoprim has been recently included in the French guidelines for the treatment of urinary tract infections, but no epidemiological data supports its use. We aimed to determine the trimethoprim susceptibility of Escherichia coli isolates responsible for community-acquired urinary tract infections in women of childbearing age. MATERIALS AND METHODS: We conducted a national prospective survey. A total of 350 strains of E. coli isolated from urines in 35 laboratories were included. Antibiotic susceptibility testing was performed in each laboratory. RESULTS: We reported a suscepti

The changing pattern of antimicrobial resistance within 42,033 Escherichia coli isolates from nosocomial, community and urology patient-specific urinary tract infections, Dublin, 1999-2009.

OBJECTIVE: To investigate the changing pattern of antimicrobial resistance in Escherichia coli urinary tract infection over an eleven year period, and to determine whether E. coli antibiotic resistance rates vary depending on whether the UTI represents a nosocomial, community acquired or urology patient specific infection. PATIENT AND METHODS: A retrospective analysis of the 42,033 E. coli urine isolates from the 11-year period 1999-2009 in a single Dublin teaching hospital was performed. WHONET(TM) software was used to analyse the changing pattern of sensitivity and resistance of E. coli to

Nosocomial infections in the rehabilitation department.

AIM: The patients of a Rehabilitation Department are at high risk of nosocomial infections because they generally have a long term hospitalisation and more and more frequently immune-compromised subjects, like old patients or with chronic illness, are admitted to rehabilitation programs. However, to evaluate the real infectious risk of a Rehabilitation Unit, it is important to consider also that a high number of patients are transferred from other hospitals after a specific therapy of the acute phase of their medical or surgical pathology and so many nosocomial microrganisms previously acquire

Use of norfloxacin for prevention of symptomatic urinary tract infection in chronically catheterized patients.

Thirty-four elderly inpatients with long-term urethral catheters were randomly assigned to receive either norfloxacin (200 mg/day) or placebo in a double-blind study with cross-over after three months. Twenty-three patients completed the entire study. Norfloxacin treatment was associated with a persistent decrease in gram-negative isolates (p < 0.005) and the acquisition of gram-positive norfloxacin-resistant flora. It also resulted in a highly statistically significant reduction of symptomatic urinary tract infections (1 vs. 12, p < 0.02), a decrease in catheter-associated local complications

Oral antibiotics to prevent postoperative urinary tract infection: a randomized controlled trial.

OBJECTIVE: To evaluate whether nitrofurantoin prophylaxis prevents postoperative urinary tract infection (UTI) in patients receiving transurethral catheterization after pelvic reconstructive surgery. METHODS: In a randomized, double-blind, placebo-controlled trial, participants undergoing pelvic reconstructive surgery were randomized to 100 mg nitrofurantoin or placebo once daily during catheterization if they were: 1) discharged with a transurethral Foley or performing intermittent self-catheterization; or 2) hospitalized overnight with a transurethral Foley. Our primary outcome was treatmen

In vitro activity of furazidin and comparator agents against bacterial isolates from urinary tract infections.

PURPOSE: Furazidin is a nitrofuran derivative used in Eastern Europe (e.g., Latvia, Lithuania and Poland) and Russia for treating uncomplicated urinary tract infections (UTI), while nitrofurantoin, another nitrofuran, is a widely used for this indication. This study (23-ADA-01) evaluated the activity of furazidin, nitrofurantoin, and fosfomycin against bacterial isolates collected between 2020 and 2022 from female UTI patients in 19 European and the Mediterranean countries. METHODS: A total of 3,111 isolates were tested for susceptibility to furazidin, nitrofurantoin and fosfomycin using brot

Management of urinary tract infections in paediatrics.

Urinary tract infections (UTIs) are a common cause of morbidity in the paediatric population, with prevalence varying according to age, sex, and circumcision status. Differentiating between upper and lower tract infections is crucial, as upper UTIs carry a greater risk of renal scarring and long-term complications. Accurate diagnosis and timely management reduce acute symptoms and prevent sequelae such as chronic kidney disease and hypertension. Current guidance recommends urgent referral for infants under 3 months of age, short-course oral antibiotics for uncomplicated cystitis in older child

Results of TOR001: An open-label single patient study using targeted bacteriophage therapy for the treatment of chronic urinary tract infection.

Chronic urinary tract infections are persistent bacterial infections with the potential to drive antibiotic resistance. Like other persistent bacterial infections, intracellular bacterial reservoirs and biofilm formation hinder the clearance of pathogens despite long courses of antibiotic therapy. New strategies for treatment of these persistent infections are needed. Here we describe the results of an open-label individual patient study using bacteriophage therapy to treat a chronic urinary tract infection in an immunocompetent 72-year-old woman without underlying urolithiasis or indwelling d

Comparison of Urinary Tract Infection Rates Between Transperineal Prostate Biopsies with and Without Prophylactic Antibiotics: An Updated Systematic Review and Meta-Analysis.

Background and Objectives: The European Association of Urology (EAU) Guidelines on Prostate Cancer note emerging evidence suggesting that antibiotic prophylaxis may not be necessary for transperineal prostate biopsies. However, formal recommendations are pending further research. This meta-analysis compares urinary tract infection (UTI) rates following transperineal prostate biopsies with and without antibiotic prophylaxis. Materials and Methods: We searched PubMed, EMBASE, and the Cochrane Library for relevant studies published up until June 2024. The inclusion criteria were as follows: (a) p

Urinary tract infections: pathogenesis, host susceptibility and emerging therapeutics.

Urinary tract infections (UTIs), which include any infection of the urethra, bladder or kidneys, account for an estimated 400 million infections and billions of dollars in health-care spending per year. The most common bacterium implicated in UTI is uropathogenic Escherichia coli, but diverse pathogens including Klebsiella, Enterococcus, Pseudomonas, Staphylococcus and even yeast such as Candida species can also cause UTIs. UTIs occur in both women and men and in both healthy and immunocompromised patients. However, certain patient factors predispose to disease: for example, female sex, histor

Flemish consensus statement on the prevention, diagnosis and treatment of urinary tract infections in older nursing home residents.

BACKGROUND: Urinary tract infections (UTIs) are one of the most commonly reported infections in Belgian nursing home residents. In older adults, UTI diagnosis and management is complex, often leading to over-diagnosis and irrational antimicrobial use, stressing the need for a guideline approach. OBJECTIVES AND METHODS: A consensus statement on the prevention, diagnosis and treatment of UTIs in older adults residing in nursing homes was developed in a collaborative effort between the Flemish Hospital Outbreak Support Teams, the Flemish Agency for Care and Health, the Association of the Flemish

Diagnostic validation study of rapid urinary tract infection diagnosis kit at peripheral health facilities of West Bengal, India.

Patients reporting to the outpatient departments of peripheral health care settings in India with symptoms of urinary tract infection (UTI) receive one or the other antibiotic before culture confirmation and out of the total culture confirmed UTI cases, in less than one third cases the prescribed antibiotics matches to the antibiotic sensitivity test result. Hence, in this study, an indigenous point-of-care (POCT) rapid diagnostic kit (Rapidogram) for UTI was validated against conventional urine culture and sensitivity to understand its possible applicability at peripheral health care settings
