BACKGROUND: The refraction of the eye is altered significantly after the instillation of silicone oil into the vitreous cavity because of the silicone oil's high refractive index. The degree of the refractive change shows diurnal variation and depends on the patient's head position.

PURPOSE: To determine the effects of posterior chamber intraocular lens implantation in patients with ruptured capsules of after intracapsules cataract extraction.
METHODS: Using transscleral suture fixation, posterior chamber intraocular lenses were implanted in 23 eyes (23 cases), including 13 eyes after intracapsular cataract extraction and 10 eyes with ruptured capsules due to complicated extracapsular cataract extraction or traumatic cataract.

The authors analysed the records of 73 cases with sensory heterotropia, and discussed the correlation between the age of sensory obstacles in sensory heterotropia and the type of heterotropia, the restores of binocular vision after surgical correction and the reversion of eye position. We evaluated aphakia and pseudophakia in highly myopic patients whose axial lengths were 27 mm and over. Cataract surgery alone was performed on 99 eyes (aphakic group) and intraocular lens (IOL) implantation was performed on 84 eyes (pseudophakic group). We reviewed 604 eyes in 521 patients who had intracapsular cataract extraction. The follow-up period averaged 39 months. The incidence of rhegmatogenous aphakic retinal detachment (ARD) was 1.3% in the whole group. Many complicated postoperative vitreoretinal cases require reoperations. It is possible to perform some of these procedures outside the operating room. We describe examining room techniques for fluid-gas exchange, fluid and gas aspiration, and adherent vitreous strand removal. We also discuss a slit-lamp technique for using sodium hyaluronate (Healon) following fluid-gas exchange to eliminate optical distortion from endothelial striae.

Eight cases of delayed nonexpulsive suprachoroidal hemorrhage occurred after filtering operations in aphakic eyes. Sustained hypotonia resulting in serous choroidal detachments, scleral infolding, or both was observed prior to the hemorrhage in seven of the eight eyes. The prognosis was related to the extent of the hemorrhage and possibly to whether or not surgical drainage was performed. Only one of the five patients with massive hemorrhages recovered visual acuity better than hand motions; that patient had undergone prompt surgical drainage of the suprachoroidal blood.

Aphakic cystoid macular edema is a serious eye disease. Its etiology is unknown, but a widely held theory attributes it to an inflammatory response to surgical trauma. In a double-masked study, a 1% solution of fenoprofen sodium, a topically applied antiprostaglandin agent, was evaluated against placebo in determining the effectiveness of fenoprofen in the treatment of chronic aphakic cystoid macular edema. A total of 55 cataract extractions were performed on 53 patients aged 90 years or more in the period 1972 to 1982. Assessment of the outcome was based on data from hospital records and on questionnaires to the patient's ophthalmologist and general practitioner. Cataract surgery had improved visual acuity in 42 patients. Only 25 patients (47%) could, however, utilize the improved visual acuity, and obtain some practical benefit from the cataract extraction. We studied a series of 34 eyes in 31 patients in whom retinal detachment occurred during miotic therapy. In 14 eyes, the duration of miotic use before the development of detachment was two months or less. Most detachments occurred in detachment-prone eyes either by virtue of myopia (62%), aphakia (24%), ipsilateral lattice degeneration (38%) or retinal pathology, in the fellow eye (50%).
