Bone and joint infections caused by gram-negative bacteria.

Infections of the bones and joints caused by gram-negative bacteria from the family Enterobacteriaceae account for 10%-15%. This family usually cause opportunistic infections, due to a low virulence factor, host and environmental factors. Gram-negative bacteria cause osteomyelitis, infectious arthritis and Reiter's syndrome. The family Enterobacteriaceae belong to the physiological flora of man and cause opportunistic and hospital infections. In order to reach an accurate etiologic diagnosis, it is therefore important for the samples for bacteriologic examination to be properly taken. The qual

[The diagnostic value of so-called Reiter cells in joint effusions].

In 1967, Pekin, Malinin and Zvaifler described macrophages with one or more phagocytized granulocytes. These authors declared these cell phagocytes as a characteristic cytologic sign of Reiter's syndrome. Because we could observe this phenomenon in other joint diseases, too, we studied systematically different inflammable and non-inflammable joint effusions for the diagnostic value of these cells. We counted 1,000 cells in each joint effusion and determined the numbers of phagocytizing and the numbers and sorts of phagocytized cells and heterophagic vacuoles with cell fractions, respectively.

Psoriasiform disorders with joint symptoms.

We present a 49-year-old man seen at the dermatology outpatient department with a 3-year history of painful swollen digits of hands and feet. On enquiry he reported dysuria. On examination we saw extensive swelling of the digits, keratosis of the nails, and some psoriasiform skin lesions on the soles of the feet. The differential diagnosis included acrodermatitis continua suppurativa, reactive arthritis and psoriatic arthritis. Radiographic imaging revealed the presence of arthritis. Testing proved negative for rheumatoid factor and positive for HLA-B27 making spondyloarthropathy the most like

Psoriatic arthritis: phenotypic variance and nosology.

Psoriatic arthritis (PsA) has been recognized as a distinct entity different from rheumatoid arthritis. Classification and phenotyping of PsA have progressed substantially since the first classification criteria of the disease were published in 1973 by Moll and Wright. The initial disease patterns described by Moll and Wright have been found to overlap and change over time. There has been controversy about whether these should be maintained or whether the phenotype of PsA should include peripheral and axial disease only. PsA is a multifaceted disease that can present as different clinical phen

[Spondyloarthritides--clinical features].

The spondyloarthritides are group of interrelated and overlapping arthritic conditions which primarily include ankylosing spondylitis, reactive arthritis/Reiter's disease and arthritis associated with psoriasis and inflammatory bowel disease. They are characterised by the absence of rheumatoid factor and by association with HLA-B27 antigen. The main clinical features are inflammatory back pain, spondylitis, sacroileitis, asymmetric arthritis of lower limbs, enthesitis, dactylitis, besides fatigue, uveitis, skin and mucous membrane lesions, cardiac and pulmonary involvement. Although there are
