Committee Opinion No 705 Summary: Mental Health Disorders In Adolescents.

Mental health disorders in adolescence are a significant problem, relatively common, and amenable to treatment or intervention. Obstetrician-gynecologists who see adolescent patients are highly likely to see adolescents and young women who have one or more mental health disorders. Some of these disorders may interfere with a patient's ability to understand or articulate her health concerns and appropriately adhere to recommended treatment. Some disorders or their treatments will affect the hypothalamic-pituitary-gonadal axis, causing anovulatory cycles and various menstrual disturbances. Adole

Surgical improvement of speech disorder caused by amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a progressive debilitating neurological disease. ALS disturbs the quality of life by affecting speech, swallowing and free mobility of the arms without affecting intellectual function. It is therefore of significance to improve intelligibility and quality of speech sounds, especially for ALS patients with slowly progressive courses. Currently, however, there is no effective or established approach to improve speech disorder caused by ALS. We investigated a surgical procedure to improve speech disorder for some patients with neuromuscular diseases with vel

Interpersonal Trust and Borderline Personality Disorder: Insights From Clinical Practice and Research: Introduction.

Individuals suffering from borderline personality disorder (BPD) show a pervasive sense that others cannot be trusted, are vulnerable to negative therapeutic reactions, and can oscillate between idealized and persecutory interactions with others. These trust processing impairments impact both the immediate and wider social milieu of individuals with BPD, including therapist-patient interactions. Recently, research started unraveling the social-cognitive mechanisms of these impairments in BPD. In this Special Issue, we attempt to close the gap between research findings and clinical theories on

Neuromyelitis Spectrum Disorders.

The understanding of neuromyelitis optica spectrum disorder (NMOSD) has evolved substantially since its initial description over a century ago. The discovery in 2004 of a pathogenic autoantibody biomarker targeting aquaporin 4 IgG revolutionized diagnosis and therapeutic development. Although NMOSD resembles multiple sclerosis (MS), differences were identified and articulated in the late 1990s. New diagnostic criteria incorporating the biomarker as well as better understanding of the clinical and radiologic features of NMOSD now permit accurate diagnosis and differentiation from MS. Aquaporin

Akinetic mutism and the story of David.

Following a description about the characteristics of akinetic mutism (AM) and how it differs from locked-in syndrome (LIS) and a disorder of consciousness (DOC), we present the case of David, a 71-year-old man with AM. David sustained a stroke following a middle cerebral artery (MCA) thrombus. A CT scan at the time detected old ischaemic infarcts affecting the right frontal lobe, the left occipital lobe and the left parietal lobe so David had bilateral brain damage. Initially thought to have a DOC, further neuropsychological assessments administered when David had improved a little, resulted i

[Temporomandibular joint septic arthritis with secondary condylar resorption].

INTRODUCTION: Septic arthritis are serious infections rarely observed for the temporomandibular joint. They are mainly hematogenous or transmitted by contiguity. OBSERVATION: Our patient presents the case of an infection of the temporomandibular joint by maxillary sinusitis of dental origin further complicated by cerebral abscess and empyema. Initial treatment consisted of an endonasal and intraoral drainage, intravenous empirical antibiotic therapy, a close clinicoradiological monitoring, and rehabilitation following a long-term active physiotherapy. Furthermore, the patient reported the ons

Taking a PAWS to Reflect on How the Work of a Therapy Dog Supports a Trauma-Informed Approach to Prisoner Health.

Canada's Correctional Investigator has found that mental health disorders, alone or in combination with alcohol and drug abuse, challenge public health and safety. Trauma is a key contributor among Canada's inmate population. Therapy dogs can assist in supporting individuals with mental health, addiction, and trauma concerns. This case report presents the work of a St. John Ambulance therapy dog in a trauma-informed approach to prisoner health. The Substance Abuse and Mental Health Services Administration articulates six evidence-based trauma principles for service providers; safety; trustwort

Diagnosis and treatment of myofacial pain-dysfunction (MPD) syndrome.

The successful management of patients with MPD syndrome is dependent on establishing an accurate diagnosis and using proper therapy based on an understanding of the etiology of the disorder. Establishing an accurate diagnosis is accomplished by taking a careful history, doing a thorough examination, and having a knowledge of the various other conditions that can produce signs and symptoms similar to those of MPD syndrome. Using proper therapy is related to recognition that MPD syndrome is a stress-induced psychophysiologic disease originating in the muscles of mastication and not an organic di

Longitudinal study of the development of obstruent correctness from ages 3 to 5 years in 108 Danish children with unilateral cleft lip and palate: a sub-study within a multicentre randomized controlled trial.

BACKGROUND: Speech-sound development in preschoolers with unilateral cleft lip and palate (UCLP) as a group is delayed/disordered, and obstruents comprise the most vulnerable sound class. AIMS: To evaluate the development of obstruent correctness (PCC-obs) and error types (cleft speech characteristics (CSCs) and developmental speech characteristics (DSCs)) from ages 3-5 and to investigate possible predictors (error types, velopharyngeal dysfunction (VPD) and gender) of PCC-obs at age 5 in two groups of children with UCLP. METHODS & PROCEDURES: Subgroup analysis was conducted within a multice

[Foreign accent syndrome caused by the left precentral infarction-a case report].

A 57-year-old right-handed man was admitted to our hospital because of right facial paresis and acute-onset dysarthria. He presented with non-fluent aphasia. His aphasia gradually improved, but he started speaking with a strange accent and intonation from the fifth hospital day. He was diagnosed with foreign accent syndrome (FAS), which lasted for 2 months. MRI revealed ischemic infarction with edema in the superior, middle, and inferior parts of the left precentral gyrus. One year later, MRI revealed old, small infarct lesions in the left precentral gyrus, middle frontal gyrus, and postcentra

Children's perception of their synthetically corrected speech production.

We explore children's perception of their own speech - in its online form, in its recorded form, and in synthetically modified forms. Children with phonological disorder (PD) and children with typical speech and language development (TD) performed tasks of evaluating accuracy of the different types of speech stimuli, either immediately after having produced the utterance or after a delay. In addition, they performed a task designed to assess their ability to detect synthetic modification. Both groups showed high performance in tasks involving evaluation of other children's speech, whereas in t

BACKGROUND: Restoration of the face function and cosmetic appearance after a traumatic complex wound is a challenge for the plastic surgeon. Worldwide, few cases have been reported about face replantation. OBJECTIVE: To present the case of the first partial face replantation reported in the national bibliography, using the labial artery for revascularization. CLINICAL CASE: On June 19th 2011, a 7 years old male presented to the emergency room of the Mexican Institute of Social Security at Monterrey, Mexico, 4 hours after a partial face amputation secondary to a dog bite. The amputated segmen

Cerebral mechanisms involved in word reading in dyslexic children: a magnetic source imaging approach.

The purpose of the present investigation was to describe spatiotemporal brain activation profiles during word reading using magnetic source imaging (MSI). Ten right-handed dyslexic children with severe phonological decoding problems and eight age-matched non-impaired readers were tested in two recognition tasks, one involving spoken and the other printed words. Dyslexic children's activation profiles during the printed word recognition task consistently featured activation of the left basal temporal cortices followed by activation of the right temporoparietal areas (including the angular gyrus

Kinematic and Acoustic Responses to Predictable and Unexpected Auditory Feedback Perturbations in Speakers With and Without Parkinson's Disease.

PURPOSE: Individuals with Parkinson's disease (IwPD) frequently suffer from speech articulation impairments that result in reduced intelligibility. While some studies have previously investigated auditory feedback and feedforward mechanisms as potential explanations behind these impairments, they have done so using only acoustic measures, which may not be sensitive enough to capture small changes. The current study assessed the sensorimotor control of speech articulation in IwPD and control speakers (CS) by measuring both acoustic and kinematic responses to either predictable or unexpected err

The Effects of Palatal Expansion on Speech Articulation in Children With Unilateral Versus Bilateral Cleft Lip and Palate.

OBJECTIVE: Children with cleft lip and palate (CLP) often require palatal expansion before secondary alveolar bone grafting (SABG). As individuals with CLP frequently experience speech disorders, the expander may further impact speech due to the space it occupies intraorally. This study aimed to evaluate the effects of expansion and cleft type on speech outcomes. METHODS: This was a prospective cohort study of 30 patients (37% females; mean age 8.95 ± 1.04 y) with CLP, including 16 bilateral CLP and 14 unilateral CLP. Speech recordings and self-perception questionnaires were obtained at 6 tim

Enhancing Intervention for Residual Rhotic Errors Via App-Delivered Biofeedback: A Case Study.

PURPOSE: Recent research suggests that visual-acoustic biofeedback can be an effective treatment for residual speech errors, but adoption remains limited due to barriers including high cost and lack of familiarity with the technology. This case study reports results from the first participant to complete a course of visual-acoustic biofeedback using a not-for-profit iOS app, Speech Therapist's App for /r/ Treatment. METHOD: App-based biofeedback treatment for rhotic misarticulation was provided in weekly 30-min sessions for 20 weeks. Within-treatment progress was documented using clinician pe

Articulatory-acoustic vowel space: application to clear speech in individuals with Parkinson's disease.

BACKGROUND/PURPOSE: Individuals with Parkinson disease (PD) often exhibit decreased range of movement secondary to the disease process, which has been shown to affect articulatory movements. A number of investigations have failed to find statistically significant differences between control and disordered groups, and between speaking conditions, using traditional vowel space area measures. The purpose of the current investigation was to evaluate both between-group (PD versus control) and within-group (habitual versus clear) differences in articulatory function using a novel vowel space measure
