New guidelines for newborn resuscitation--a critical evaluation.

UNLABELLED: The 2010 International Liaison Committee on Resuscitation guidelines for newborn resuscitation represent important progress. The criteria for assessment are simplified based on heart rate and respiration only and there is no timing of stages after the first 60 sec. Instead of giving supplemental oxygen, the guidelines state that 'it is best to start with air'. However, the optimal oxygen concentration later in the process and for premature babies is not yet clear. A description of an adequate heart rate response is not given, and the cut-off of 100 bpm may be arbitrary. There are s

Total antioxidant capacity and total oxidant status in perinatal asphyxia in relation to neurological outcome.

This study aimed to investigate the global oxidant/antioxidant status of infants with perinatal asphyxia and its relation to neurological outcomes. A prospective controlled study including term infants with perinatal asphyxia was conducted. Blood samples were obtained from patients and controls at 6-24 h and on the 3rd day of life for TAC and TOS measurement and OSI values were calculated. Neurodevelopment was evaluated at 12 months of age in survivors using Bayley scales of infant development II (BSID II). 17 term infants with perinatal asphyxia and 17 healthy controls were enrolled. On the f

Therapeutic hypothermia during neonatal transport.

AIM: To compare the effectiveness of different cooling methods used during neonatal transport in maintaining target temperature. METHODS: Retrospective review of transport data for all infants centralized for therapeutic hypothermia between April 2008 and October 2009. RESULTS: A total of 46 infants were retrieved for therapeutic hypothermia during the study period. During transport, 10 infants were passively cooled, 17 infants were actively cooled with adjuncts and 19 infants were actively cooled with a purpose-built cooling machine. On arrival at the tertiary unit, 84% of infants who were

Early initiation of basic resuscitation interventions including face mask ventilation may reduce birth asphyxia related mortality in low-income countries: a prospective descriptive observational study.

AIM OF THE STUDY: Early initiation of basic resuscitation interventions within 60 s in apneic newborn infants is thought to be essential in preventing progression to circulatory collapse based on experimental cardio-respiratory responses to asphyxia. The objectives were to describe normal transitional respiratory adaption at birth and to assess the importance of initiating basic resuscitation within the first minutes after birth as it relates to neonatal outcome. METHODS: This is an observational study of neonatal respiratory adaptation at birth in a rural hospital in Tanzania. Research assis

A survey of intravenous sodium bicarbonate in neonatal asphyxia among European neonatologists: gaps between scientific evidence and clinical practice.

BACKGROUND: Birth asphyxia is characterized by intermittent periods of hypoxia/ischemia leading to metabolic acidosis. The use of intravenous sodium bicarbonate (IVSB) is still a matter of controversy in clinical practice. OBJECTIVE: To assess the use of IVSB in birth asphyxia by attending neonatologists in European hospitals. DESIGN/METHODS: Survey using a questionnaire administered to neonatologists in Europe whose electronic addresses were provided by the national councils of neonatology of each participating country. The questionnaire consisted of a brief theoretical introduction followe

Reversible asphyxial status in a newborn due to neonatal form of carnitine palmitoyltransferase II deficiency.

OBJECTIVES: To present a term newborn with severe asphyxial status due to dysrrhythmia induced by the neonatal form of carnitine palmitoyltransferase II deficiency (CPT II). RESULTS: Term newborn delivered spontaneously (birth weight 3450 grams, birth length 52 cm, values of Apgar score 10/10) with good direct adaptation, on second day of life he manifested severe asphyxial status followed by cardiorespiratory insufficiency with circulatory failure. After prolonged resuscitation of 3 hours, the child was admitted to our neonatological department. Diagnosis of CPT II was confirmed (free carnit

Air versus oxygen for resuscitation of infants at birth.

BACKGROUND: 100% oxygen is the commonly recommended gas for the resuscitation of infants at birth. There is growing evidence from both animal and human studies that room air is as effective as 100% oxygen and that 100% oxygen may have adverse effects on breathing physiology and cerebral circulation. There is also the theoretical risk of tissue damage due to free oxygen radicals when 100% oxygen is given. The use of room air has, therefore, been suggested as a safer and possibly more effective alternative. OBJECTIVES: In newborn infants requiring resuscitation, does the use of room air reduce

Perinatal and neonatal mortality and morbidity in Iraq.

Perinatal mortality rates in Iraq have been extremely high during the last 10 years, although detailed data are not available due to inadequate collection of health information during the Gulf conflict and the period of sanction which has continued since August 1990 until the present. The average perinatal mortality rate for Iraq was estimated to be 28 per 1000 live births during the period 1980-90 (the period before sanctions). This rate is similar to the rate in other countries in the region in the same period. The average perinatal mortality rate during the period 1990-1999 is estimated to

Familial asphyxiating thoracic dysplasia: clinical variability and impact of improved neonatal intensive care.

We describe 3 siblings with asphyxiating thoracic dysplasia whose neonatal symptoms range from mild respiratory distress to asphyxia and death. The youngest sibling received aggressive modern respiratory intensive care, survived, and at 2 years showed no respiratory symptoms. Improved neonatal intensive care has implications for clinical decision making and genetic counseling.

Neuropsychological long-term sequelae of perinatal asphyxia.

OBJECTIVE: To investigate the long-term neuropsychological consequences of perinatal asphyxia (PA). METHODS: A group of adolescents were assessed with antecedents of mild (n = 8) and moderate (n = 20) PA, and a matched group of 28 healthy adolescents as a control group. Neuropsychological assessment included tests of memory, perceptual-motor skills, and frontal lobe functions, because these are areas of cognitive functioning susceptible to hypoxic conditions. RESULTS: Subjects with moderate PA showed significant differences from the control group on tests related to delayed recall for both v

[Perinatal ischemic brain damages and apoptosis].

Perinatal brain damages are caused mainly by circulation insufficiency due to intrauterine or birth asphyxia. Hypoxic-ischemic encephalopathy (HIE) and pontosubicular neuronal necrosis (PSN) are typical perinatal ischemic diseases. We investigated the expression of apoptosis in these diseases, using TdT-mediated dUTP nick end labeling (TUNEL) method and immunohistochemistry with Bcl-2, Bcl-x, Bak and caspase 3 (CPP 32). In HIE, Purkinje cells showed overexpression of Bcl-2 and CPP 32, and some karyorrhectic cells were positive for TUNEL. In PSN, neurons of the pontine nuclei showed overexpress

Birth asphyxia.

The term Birth asphyxia covers a number of clinical and physiological definitions. Birth asphyxia is a relatively common clinical event. In the majority of cases the outcome in terms of brain damage and future development of the child is excellent. However, a small number of children go on to develop patterns of brain damage which are then associated with disability. The article seeks to provide a basic understanding of the various mechanisms involved in producing injury.

Reperfusion injury as the mechanism of brain damage after perinatal asphyxia.

Upon reperfusion of ischemic tissues, reactive oxygen metabolites are generated and are responsible for much of the organ damage. Experimental studies have revealed two main sources of these metabolites: 1) the oxidation of hypoxanthine to xanthine and on to uric acid by the oxidase form of xanthine oxidoreductase and 2) neutrophils accumulating in ischemic and reperfused tissue. Blocking either source will reduce reperfusion damage in a number of experimental situations. Although xanthine oxidoreductase activity may be unmeasurably low in organs other than liver and intestine, it may be invol
