Sinus node function and conduction system after complete repair of tetralogy of Fallot.

Twenty-three children underwent electrophysiological studies during routine postoperative catheterization two months to five years after complete correction of tetralogy of Fallot. The aim of the study was to investigate the whole conduction system, including sinus node function, suing His bundle recordings and atrial pacing. H-V intervals were normal at rest and with pacing in twenty-two patients, including four patients with evidence of bifascicular block on the surface ECG. One patient with cardiomegaly and evidence of diffuse myocardial damage had a prolonged H-V interval but did not devel

Supraventricular tachycardia. ECG diagnosis and anatomy.

This paper reviews the anatomical substrates responsible for the induction and maintenance of supraventricular tachycardia and discusses the ECG findings associated with these tachycardias. The normal anatomy of the supraventricular conducting system, particularly within the atria, is complex with conduction proceeding along preferential pathway, which are in turn determined in part by the anisotropic properties of the atrial myocardium. There appear to be at least dual inputs to the atrioventricular node, a posteriorly situated slow pathway and an anterior fast pathway. It is sometimes possib…

[Bradyarrhythmias: diagnostics].

Bradyarrhythmias cause symptoms such as syncope, dizziness, or exercise intolerance and lead to the implantation of > 100 000 pacemakers in Germany annually. They are caused either by sinus node dysfunction (sinus bradycardia, sinoatrial block, sinusarrest, preautomatic pauses in paroxysmal atrial fibrillation, chronotropic incompetence) or by atrioventricular (AV) conduction abnormalities (AV block, complex bundle branch block). Optimal therapy of bradycardia crucially depends on ECG documentation which frequently requires Holter monitoring in the case of intermittent bradycardia. The differe

Unusual left-sided variant orientation of the slow conduction zone in adenosine-sensitive atrial tachycardia.

Adenosine-sensitive atrial tachycardia (AT) is typically associated with a slow conduction zone (SCZ) near the atrioventricular node or tricuspid annulus. We report an unusual case of adenosine-sensitive AT with a SCZ located between the left atrial appendage and the left atrial anterior septum, which was successfully ablated 9.2 mm from the left atrial earliest activation site. This case highlights the importance of advanced mapping and entrainment techniques in the identification and management of rare left-sided SCZ variants of adenosine-sensitive AT.

Transvenous pacing after the Mustard procedure: considering the complications.

Disturbances of rhythm and conduction in patients undergoing surgery for transposition of the great arteries have been widely reported. Some of these patients require implantation of a permanent pacemaker, especially those in whom symptomatic sick sinus syndrome is diagnosed. We present the case of a 29-year-old male corrected with a Mustard procedure, who received a pacemaker for progressive atrioventricular conduction disturbances and sinus node dysfunction, and we review the possible complications associated with transvenous pacemaker implantation in these patients. [Cardiotoxicity of amantadine in acute poisoning: a case report].

We report the case of an adolescent, 15-year-old woman, who ingested 4.0 g of amantadine in a suicide attempt 2 hours before admission. She developed central nervous system symptoms including confusion and psychomotor agitation. The main cardiovascular symptoms were due to disturbances in atrioventricular conduction and propagation of stimuli in sinus node. All cardiovascular symptoms disappearanced after 12 hours of admission, CNS symptoms were observed 48 hours. No other systems were involved. She was discharged in a good state on day 5th.

Value of pre-operative carotid sinus massage.

The usefulness of carotid sinus massage in pre-operative assessment is discussed with case examples. The technique and risks of carotid sinus massage are described. Carotid sinus massage is of particular value in the demonstration of sino-atrial and atrioventricular node disease in the absence of definitive abnormalities on the routine resting electrocardiogram, and in the identification of patients who show a hypersensitive response to vagal stimulation. These patients should be identified prior to procedures that involve marked vagal activity, since prolonged sinus arrest may occur.

Focal giant-cell cardiomyopathy.

A newborn with supraventricular arrhythmia diagnosed during fetal life was found at autopsy to have a small nodule with hypertrophic and multinucleated myocardial cells in the atrial septum, near the atrioventricular node. There were no stigmata of Beckwith-Wiedemann syndrome or tuberous sclerosis. This focal giant-cell cardiomyopathy has been described only once, in an infant with Beckwith-Wiedemann syndrome. It is suggested that these dysplastic myocardial cells may be responsible for the atrial arrhythmia and that this hamartomatous lesion has the potential for malignant transformation.

Transvenous atrial pacing in the management of sick sinus syndrome following surgical treatment of the univentricular heart: case report and review.

An 11-year-old boy with univentricular heart type A-III underwent surgical treatment at age 10 with a modified Fontan operation. Six months postoperatively he developed intermittent periods of cyanosis and fatigue associated with profound sinus bradycardia and nodal escape. After demonstrating normal atrioventricular conduction, a transvenous atrial pacemaker was implanted. This produced a marked clinical improvement. Transvenous atrial pacing is a satisfactory method of treating sinus node dysfunction in patients with univentricular heart following the Fontan operation provided that there is

Nifedipine overdose.

A 59-year-old man ingested 900 mg of nifedipine. Profound hypotension, sinus and atrioventricular node dysfunction, and hyperglycemia resulted. The patient's condition responded favorably to aggressive treatment with intravenous fluids, calcium, and dopamine. Similar effects and response to therapy have been reported in 17 cases of verapamil overdose and are briefly summarized. The effects of nifedipine overdose are discussed, and preliminary recommendations about its management are offered. With increasing use of nifedipine and other calcium channel blockers, more incidents of overdose can be…

Development of the cardiac conduction system in atrioventricular septal defect in human trisomy 21.

In patients with atrioventricular septal defect (AVSD), the occurrence of nonsurgical AV block has been reported. We have looked for an explanation in the development of the AV conduction system. Human embryos with AVSD and trisomy 21 and normal embryos were examined (age 5-16 wk gestation). Antibodies to human natural killer cell-1 (HNK-1), muscle actin (HHF-35), and collagen VI were used to delineate the conduction system. As in normal hearts, HNK-1 transiently stains the AV conduction system, the sinoatrial node, and parts of the sinus venosus in AVSD. A large distance is present between th

Effects of adenosine triphosphate on ventriculoatrial conduction--usefulness and problems in assessment of catheter ablation of accessory pathways.

The effects of adenosine triphosphate (ATP) on ventriculoatrial (VA) conduction were examined before and after accessory pathway (AP) ablation, with emphasis on assessment of the complication of dual atrioventricular (AV) node pathway. By evaluating the differences in the response to ATP of APs and other pathways, we assessed the usefulness and problems of this method. Of 59 patients who underwent AP ablation, 31 showed pre-excitation and 28 had concealed APs. A dual AV node pathway was found in 9 patients (15.3%) before ablation. After ablation, a dual AV node pathway was newly found in 9 pat

Action of ATP on ventricular automaticity.

ATP is an effective treatment of supraventricular tachycardia when the atrioventricular (AV) node is part of the reentrant circuit. However, the lower a pace-maker in the pacemaker hierarchy, the more sensitive it is to adenosine. Therefore, we investigated the effects of ATP on ventricular automaticity in in vivo and in vitro conditions. Wide and narrow QRS complex tachycardia in 46 patients was treated with 6, 12, and 18 mg ATP as sequential intravenous (i.v.) bolus. ATP terminated tachycardias in 67%. Bolus infusion ATP caused < or = 6.4-s asystole that was self-limited. Perfusion of isolat

Use of calcium channel entry blockers in the treatment of cardiac arrhythmias.

In clinical arrhythmias, the main therapeutic role of calcium channel entry blockers is related to their effect on the sinus and atrioventricular (AV) node. Consequently, in cardiac arrhythmias where the AV node is part of the reentry circuit, a beneficial effect of diltiazem and verapamil can be demonstrated. These include AV nodal reentry and orthodromic tachycardia in patients with Wolff-Parkinson-White syndrome. In addition, the ventricular response by the AV node during atrial tachycardias can also be controlled with these agents. A specific type of ventricular tachycardia seen in the abs

Diagnostic and therapeutic use of adenosine in patients with supraventricular tachyarrhythmias.

Adenosine has been shown to affect both sinus node automaticity and atrioventricular (AV) nodal conduction. The effects of increasing doses of intravenous adenosine were assessed in 46 patients with supraventricular tachyarrhythmias. Adenosine reliably terminated episodes of supraventricular tachycardia in all 16 patients with AV reciprocating tachycardia, in 13 of 13 patients with AV nodal reentrant tachycardia and in 1 of 2 patients with junctional tachycardia with long RP intervals. Adenosine produced transient high grade AV block without any effect on atrial activity in six patients with i

Koch's triangle in pediatric age: correlation with extra- and intracardiac parameters.

The atrioventricular node is situated in the lower atrial septum, at the apex of the Koch's triangle. The dimensions of the Koch's triangle are studied in adult humans, while no data exist about them in pediatric age. The knowledge of the dimensions of Koch's triangle in childhood is very important for safe and correct application of radiofrequency energy during transcatheter ablation. The dimensions of Koch's triangle were determined in 69 human pediatric hearts. The median age of the children was 3 months, with a range from 1 day to 14 years, 30 were female and 39 were male. Endogenous RGS proteins modulate SA and AV nodal functions in isolated heart: implications for sick sinus syndrome and AV block.

G protein-coupled receptors play a pivotal role in regulating cardiac automaticity. Their function is controlled by regulator of G protein signaling (RGS) proteins acting as GTPase-activating proteins for Galpha subunits to suppress Galpha(i) and Galpha(q) signaling. Using knock-in mice in which Galpha(i2)-RGS binding and negative regulation are disrupted by a genomic Galpha(i2)G184S (GS) point mutation, we recently (Fu Y, Huang X, Zhong H, Mortensen RM, D'Alecy LG, Neubig RR. Circ Res 98: 659-666, 2006) showed that endogenous RGS proteins suppress muscarinic receptor-mediated bradycardia. To

De subitaneis mortibus. XXIII. Rheumatoid arthritis and ankylosing spondylitis.

The conduction system of the heart was carefully examined at necropsy in two cases of rheumatoid arthritis and one of ankylosing spondylitis. All three patients had cardiac electrical instability and two fo the three died suddenly. The electrophysiological abnormalities of the three patients included paroxysmal atrial fibrillation in the first case, sustained atrial fibrillation with complete heart block and escape atrioventricular (A-V) junctional rhythm in the second case, and progressively increasing heart block eventually became complete in the third case. The sinus node exhibited extensiv

Decreased connexin 40 expression of the sinoatrial node mediates ischemic stroke-induced arrhythmia in mice.

BACKGROUND: Arrhythmia is the most common cardiac complication after ischemic stroke. Connexin 40 is the staple component of gap junctions, which influences the propagation of cardiac electrical signals in the sinoatrial node. However, the role of connexin 40 in post-stroke arrhythmia remains unclear. METHODS: In this study, a permanent middle cerebral artery occlusion model was used to simulate the occurrence of an ischemic stroke. Subsequently, an electrocardiogram was utilized to record and assess variations in electrocardiogram measures. In addition, optical tissue clearing and whole-moun
