PURPOSE: Evaluate patient, clinical, and treatment variables impacting mortality in patients with brain abscesses.
BACKGROUND: Brain abscesses are intraparenchymal infectious foci with significant morbidity and mortality. Management includes antimicrobial therapy +/- surgical intervention, and is dependent on suspected pathogen/source, patient factors, and clinician judgement. Treatment type and duration vary substantially and are often guided by imaging, inflammatory markers, and symptoms.
METHODS: 186 patients with brain abscesses admitted at a single health system between 2010 and 2022 we…

BACKGROUND Nocardia species are opportunistic pathogens that primarily cause infections in immunocompromised individuals. Giant-cell arteritis is a granulomatous vasculitis of large- and medium-sized arteries in individuals aged ≥50 years, typically requiring prolonged glucocorticoid therapy, which increases susceptibility to opportunistic infections. CASE REPORT We report the case of a 73-year-old man with giant-cell arteritis treated with prednisolone who developed disseminated nocardiosis. Four months after giant-cell arteritis diagnosis, a new pulmonary nodule was detected on follow-up ima

2019 coronavirus disease (COVID-19) has attracted global attention primarily because of the severe acute respiratory symptoms associated with it. However, nearly one third of the patients also present with neurological symptoms. This report describes a case of a previously healthy woman with acute COVID-19 infection, who developed acute facial nerve palsy and rapid progression to coma due to otogenic brain abscess.A 63-year-old woman with acute COVID-19 infection exhibited acute facial nerve paresis, high fever, and purulent secretion from her left ear within 48 hours after COVID-19 onset. Cra…

BACKGROUND: Aggregatibacter aphrophilus(A. aphrophilus)is one of the organisms of the HACEK group. Previously reported cases of brain abscesses caused by A. aphrophilus infection have occurred in children with a basis for congenital heart disease, or in adults with a basis for dental disease. Rare cases of brain abscess caused by A. aphrophilus have been reported in adults with congenital heart disease or in patients without dental disease history. Herein we present a rare case of brain abscess caused by A. aphrophilus, who was in association with atrial septal defect for more than 20 years, a…

We present the case of a 58-year-old Japanese woman with a natural killer T (NK/T)-cell lymphoma complicated by brain abscess. NK/T-cell lymphomas represent a rare type of lymphoma derived from either activated NK cells or, rarely, cytotoxic T cells. They are aggressive Epstein-Barr virus (EBV)-associated lymphomas that involve mainly the nasal cavity. Brain abscess associated with primary extranodal nasal-type NK/T-cell lymphoma is extremely uncommon: to our knowledge, this is the first reported case of this lymphoma with brain abscess as the initial clinical manifestation. Endoscopic surgery…

Central nervous system infections, which are rarely seen in systemic lupus erythematosus (SLE), have considerably high mortality but they are difficult to distinguish from neuropsychiatric manifestation of lupus. This article reports the case of a patient with SLE with brain abscess which developed during immunosuppressive therapy for lupus nephritis. The patient completely recovered without neurological sequelae by open surgical drainage and 12-week antibiotic therapy. It is recommended that CNS infections must be excluded in patients with SLE, particularly who are receiving immunosuppressive…

We report here brain abscesses caused by Nocardia beijingensis in a 59-year-old-Thai male with human immunodeficiency virus (HIV) infection presenting with progressive right sided hemiparesis. A computed tomography scan of the brain showed multiple brain abscesses. A stereotactic brain biopsy and 16S rRNA sequencing showed Nocardia beijingensis. The patient was treated with trimethoprim-sulfamethoxazole and recovered completely. As far as we are aware, this is the first reported case of a brain abscess in an HIV infected patient due to Nocardia beijingensis.

We report a case of a brain abscess caused by Haemophilus influenzae type e in a 12 year-old patient suffering from Apert syndrome. Apert syndrome is characterized by the premature closure of cranial sutures. In 2010 the patient suffered head trauma in the frontal area with cranial fracture and a cerebrospinal fluid fistula. In February 2013 he was admitted to hospital with fever, vomiting and generalized tonic-clonic seizure with deteriorating mental status/progressive sensory impairment. The computerized axial tomographic scan showed a right frontal lesion, perilesional edema, mild ventricul…

Legionella endocarditis is extremely uncommon, and embolic phenomena have never been reported. We report the first case of Legionella micdadei prosthetic valve endocarditis complicated by brain abscess. A 57-y-old immunocompromised woman with a history of mitral valve replacement developed confusion and left-sided weakness. Brain magnetic resonance imaging showed a 3-cm peripheral-enhancing mass. Transoesophageal echocardiography suggested a perivalvular abscess. Blood cultures and valve cultures were negative. She was diagnosed with 16S rRNA polymerase chain reaction and silver stain, and was…

PRACTICAL RELEVANCE: As this series of cases indicates, successful resolution of a brain abscess due to a bite injury depends on early recognition and combined used of antimicrobials and surgical intervention. A particular aim of surgery is to remove any skull and foreign body (tooth) fragments that may represent a continuing focus of infection …

Ramichloridium mackenziei is a dematiaceous fungus that usually causes cerebral phaeohyphomycosis. We describe the aspiration cytology findings of a case of cerebral abscess caused by R. mackenziei in a 66-yr-old Saudi woman who had a long standing history of diabetes mellitus and a recent diagnosis of systemic lupus erythematosus. She was on long-term corticosteroid therapy. The patient developed rapidly progressive multiple brain abscesses and died despite aspiration of the abscess and administration of intravenous amphotericin B lipid complex and voriconazole.

A patient with hypopharyngeal cancer developed carotid blowout syndrome (CBS) treated by self-expandable stent-graft in the left carotid artery. CT scan for progressive right hemiparesis 4 months later showed multiple left cerebral abscesses and left carotid thrombosis. Although deployment of stent-grafts for CBS can achieve initial hemostasis in patients with head-and-neck cancer, the placement of a stent-graft in a field of necrosis and infection is associated with poor long-term outcome. We recommend the use of prophylactic antibiotics if endovascular foreign materials are placed in a conta

PURPOSE: To assess the usefulness of the 0.9-ppm peak from amino acids (-CH3 moieties from valine, leucine, and isoleucine) for the differentiation of brain abscesses and tumors at in vivo hydrogen 1 magnetic resonance (MR) spectroscopy.
MATERIALS AND METHODS: Amino acid concentrations were determined in vitro in 13 purulent samples from brain and nonbrain tissues and in nine aseptic fluids from necrotic brain tumors at two-dimensional (2D) 1H MR spectroscopy and liquid chromatography. Thirty-four patients with cystic intracerebral mass lesions (28 tumors, six abscesses) were examined at 1H M…

Two cases of cerebral abscess with different presentations are reported. The source of the abscess remained obscure in one case while in the other arose from infection of the right frontal sinus. While in one case a magnetic resonance image was obtained preoperatively, both cases underwent parallel monitoring in the postsurgery period by computed tomography and magnetic resonance imaging. The relative merits of these two imaging techniques in the diagnosis and monitoring of such lesions are discussed, and the literature is reviewed.

Brain abscess is a life-threatening intracranial infection typically managed with surgery followed by prolonged intravenous antibiotics. This study aimed to evaluate the feasibility of a short-course intravenous antibiotics regimen after surgery and to assess the utility of 4-week MRI as a biomarker to guide treatment discontinuation. This retrospective study included 60 patients who underwent surgery for bacterial brain abscess at a tertiary center between 2011 and 2024. A scheduled 4-week MRI including diffusion-weighted imaging was performed to assess radiologic resolution. The clinical out…

INTRODUCTION: Cladophialophora bantiana (C. bantiana) brain abscess is a rare dematiaceous mold infection often refractory to conventional treatment. Aspiration is commonly used for diagnosis and decompression, while resection may provide greater source control when anatomically feasible; however, comparative evidence remains limited and confounded.
METHODS: We report an illustrative surgically managed case and performed a PRISMA-guided systematic review with patient-level analysis to characterize management strategies and outcomes.
RESULTS: A 60-year-old immunocompromised male with symptoma…

Hypervirulent Klebsiella pneumoniae (hvKp) causes severe community-acquired infections and is associated with metastatic complications, including central nervous system infections. Brain abscess is a recognized manifestation of hvKp infection; however, reports describing the evolution of cerebral infarction into brain abscess during treatment for hvKp meningoencephalitis are limited. We report a case of rapidly progressive meningoencephalitis caused by hvKp characterized by the K2 serotype, rmpA/rmpA2 genes, and sequence type 65. Brain MRI on day 5 demonstrated cerebral infarction associated w…
