OBJECTIVE: To describe a case of multiple catecholamine-secreting paragangliomas, with a hemorrhagic stroke as the main clinical manifestation.
METHODS: We present a case report with clinical, laboratory, histologic, and genetic details.
RESULTS: A 23-year-old woman with a history of hypertension treated with orally administered medications presented to our emergency department because of sudden onset of hemiplegia of the left side of the body. A computed tomographic scan of the brain showed a right frontoparietal hematoma, and her blood pressure was 185/115 mm Hg. She was admitted to the De

Today's overachieving professionals labor longer, take on more responsibility, and earn more than the workaholics of yore. They hold what Hewlett and Luce call "extreme jobs", which entail workweeks of 60 or more hours and have at least five often characteristics-such as tight deadlines and lots of travel--culled from the authors' research on this work model. A project of the Hidden Brain Drain Task Force, a private-sector initiative, this research consists of two large surveys (one of high earners across various professions in the United States and the other of high-earning managers in large …

BACKGROUND AND PURPOSE: Intraventricular clot secondary to brain hemorrhage has still one of the worst prognosis among all stroke subtypes, regardless of conservative therapy or surgical interventions. The rapid clot resolution with thrombolytic agents could improve the outcome by restoring the impaired cerebrospinal fluid circulation, for this reason, the authors examined the safety and efficacy of Urokinase therapy in a randomized, controlled study.
METHODS: They enrolled 27 patients with severe intraventricular hemorrhage between 1998 and 2002. All patients had supratentorial intracerebral

Intracranial pressure (ICP) monitoring is a cornerstone of the management of patients with acute brain injury. External ventricular drains (EVDs) and intraparenchymal pressure sensors are the two most widely used invasive monitoring modalities. Although both techniques are routinely employed in neurocritical care, discrepancies between simultaneously recorded ICP values may occur because of technical limitations, sensor drift, or physiological intracranial pressuregradients. In this study, we evaluated dual ICP monitoring using EVD and intraparenchymal pressure sensor. The primary objective wa

The central nervous system (CNS) evokes a complex inflammatory response to injury. Inflammatory cascades are present in traumatic, infectious, and noninfectious disorders affecting the brain. It contains a mixture of pro- and anti-inflammatory reactions involving well-known proteins, but also numerous proteins less explored in these processes. The aim of this study was to explore the distinct inflammatory response in traumatic brain injury (TBI) compared with other CNS injuries by utilization of mass-spectrometry. In total, 56 patients had their cerebrospinal fluid (CSF) analyzed with the use

Background and Clinical Significance: Cerebral candidiasis (Candida albicans meningoencephalitis) is a rare but severe central nervous system (CNS) infection, usually associated with neurosurgical procedures or indwelling devices. Diagnosis is challenging due to frequent negativity of cerebrospinal fluid (CSF) cultures, and mortality remains high despite antifungal therapy. Case Presentation: We describe a 64-year-old woman who underwent retrosigmoid resection of a left vestibular schwannoma. The early postoperative course was complicated by fever, neurological deterioration, and hydrocephalus

Cavernous sinus dural arteriovenous fistulas are generally considered to have a benign clinical course, as spontaneous thrombosis of the fistula or draining veins can result in regression of the lesion. However, we encountered a rare case in which this hemodynamic change led to rapid neurological deterioration, necessitating microsurgical intervention. A 72-year-old woman presenting with chemosis and exophthalmos was diagnosed with a cavernous sinus dural arteriovenous fistula. Cerebral angiography revealed 2 venous drainage routes: the superior ophthalmic vein anteriorly and the superior petr

Systematic studies on the venographic assessment of the draining veins of brain arteriovenous malformations (bAVMs) are lacking. Therefore, in this study, we described the patterns and characteristics of the efferent vein structures using multiplanar reformation in arbitrary planes of the volume-rendering images of three-dimensional rotational angiography. Patients with bAVM who underwent the first session of endovascular therapy in our department between February 2018 and April 2025 were identified. The structure of each efferent vein unit was categorized into simple, unified, bifurcated, or

Chronic spinal subdural hematoma is an extremely rare condition. We recently encountered a case of symptomatic thoracolumbar chronic spinal subdural hematoma in an older patient caused by a fall. The patient was a man in his 80s with a history of cerebral infarction, who was receiving oral antiplatelet therapy. He was hospitalized for conservative treatment for a brain contusion and mild acute subdural hematoma, and was discharged home after 6 days. However, 9 days after the injury, the patient developed back pain, weakness in both lower limbs, and urinary incontinence and was brought to our h

Carotid-cavernous fistula (CCF) is a rare sight and potentially life-threatening disorder arising from an abnormal connection between the carotid artery and the cavernous sinus. It can be classified into direct or indirect according to different arteriovenous shunts. Direct CCF usually has dramatic ocular presentations, whereas indirect CCF has a more insidious course and may be associated with neurologic symptoms in posteriorly draining fistulas. A 61-year-old gentleman presented with five days history of altered behavior and double vision preceding a bulging left eye. Ocular examination show

OBJECTIVE: We describe a patient treated with transarterial Onyx embolization for a tentorial dural arteriovenous fistula (DAVF) who presented with hemifacial spasm (HFS).
CASE PRESENTATION: A 56-year-old man suffered from right blepharospasm for 4 years, and the symptom gradually spread to the right side of his face with oculo-oral synkinesis. MRI of the brain revealed abnormal multiple flow voids at the surface of brainstem and cerebellar hemisphere. MRA (time of flight) and spoiled gradient recalled echo-revealed abnormal vessels at the posterior fossa indicated arteriovenous shunting. 3D-

INTRODUCTION: Mycetoma is a neglected tropical disease that commonly affects the lower extremity. The disease is attributed to subcutaneous granulomatous inflammation leading to distinct clinical features of gradual painless swelling accompanied by nodules and drains. Orbital mycetoma is an extremely rare entity of the disease. We reported the clinical presentation, diagnosis, and surgical outcomes in a case of orbital mycetoma with cranial extension.
CASE PRESENTATION: A 25-years-old male complained of left eye protrusion for 8 years, followed by complete loss of vision on the left eye for 7

Damage to the hypothalamus may result from direct surgical injury or from hemorrhage and ischemia caused by the procedure. Patients with hypothalamus damage can be comatose and exhibit hyperthermia. Here, we present a patient whose hypothalamus was directly damaged by a drainage catheter. His clinical manifestations included diabetes insipidus, hyperthermia and adrenocorticotropic hormone (ACTH) deficiency. The patient was a 48-year-old male and had a body weight of 95 kg. He was admitted to the hospital on August 31, 2019 for memory impairment and nonspecific dizziness that persisted for four

BACKGROUND: We describe a case of convulsive status epilepticus caused by intracranial hypotension, a complication of spinal surgery. Intracranial hypotension (IH) is typically characterized by an orthostatic headache. There have been limited case reports describing surgery-associated IH presenting with seizures.
METHODS: Case report and review of the literature.
RESULTS: A 71-year-old woman with chronic back pain developed convulsive status epilepticus, characterized by generalized clonic seizures, immediately following scoliosis surgery. She had no history of seizures or other seizure risk

BACKGROUND AND PURPOSE: Myocarditis is an inflammatory and autoimmune cardiovascular disease that causes dilated myocardiopathy and is responsible for high morbidity and mortality worldwide. Cortistatin is a neuropeptide produced by neurons and cells of the immune and vascular systems. Besides its action in locomotor activity and sleep, cortistatin inhibits inflammation in different experimental models of autoimmune diseases. However, its role in inflammatory cardiovascular disorders is unexplored. Here, we investigated the therapeutic effects of cortistatin in a well-established preclinical m

Susceptibility to progressive infection with the fungus Cryptococcus neoformans is associated with an allergic pattern of lung inflammation, yet the factors that govern this host response are not clearly understood. Using a clinically relevant mouse model of inhalational infection with virulent C. neoformans H99, we demonstrate a role for IL-33-dependent signaling in host immune defense. Infection of BALB/c mice with 10(4) CFU of C. neoformans H99 caused a time-dependent induction of IL-33 with accumulation of type 2 pulmonary innate lymphoid cells and alternatively activated macrophages in th

OBJECT: The basal vein of Rosenthal (BV) courses from the premesencephalic cistern, through the ambient cistern, and terminates in the quadrigeminal cistern. The aim of this study was to describe and quantitate the surgical anatomy of this structure and specifically to provide landmarks for identifying this vessel along its course. These data may be of use, for example, to surgeons using subtemporal operative approaches through regions where this vessel is concealed.
METHODS: The authors examined 15 latex-injected adult cadaveric brains (30 sides) to delineate the morphological characteristic

Previous studies of Cryptococcus neoformans infection have revealed a role for CD4+ T cells and CD8+ T cells in anticryptococcal resistance in the lungs, but such a role has been revealed only for CD4+ T cells in the brains of experimentally infected mice. In this study, we found that mice genetically engineered to lack CD4+ T cells could be successfully vaccinated to express resistance to a rechallenge with Cryptococcus neoformans, provided the challenge dose was kept to lower than 1000 organisms per mouse. The challenge infection was uniformly lethal for unvaccinated control mice. Depletion
