Non-invasive measurement of pulmonary blood flow during prone positioning in patients with early acute respiratory distress syndrome.

In the daily clinical routine at the bedside, information on effective pulmonary blood flow (PBF) is limited and requires invasive monitoring, including a pulmonary artery catheter, to determine both cardiac output and intrapulmonary shunt. Therefore we evaluated a non-invasive method for the measurement of PBF in a clinical setting, including 12 patients with acute respiratory failure (acute respiratory distress syndrome) undergoing prone positioning. PBF was determined before (baseline), during and after prone positioning, by using a foreign gas rebreathing method with a new photoacoustic ga

[Different cardiovascular and hormonal consequences of A-V fistula in uremic patients (author's transl)].

The Authors studied 11 uremic patients for whom it was necessary to make an arterio-venous (A-V) fistula for dialysis purposes. Before and after creation of the fistula, the following parameters were determined: arterial pressure (PA) heart rate (HR), peripheral vascular resistances (PVR), renin, adrenalin and nor-adrenalin blood levels. The patients were divided in two groups according to the increase of the systolic output (SO) and o the cardiac index (CI) after the fistula (1st group of 5 patients: less than 30% increase of these values as compared to those before the fistula; 2nd group: mo

Exercise training in heart transplantation.

Heart transplantation remains the gold standard in the treatment of end-stage heart failure (HF). Heart transplantation patients present lower exercise capacity due to cardiovascular and musculoskeletal alterations leading thus to poor quality of life and reduction in the ability of daily self-service. Impaired vascular function and diastolic dysfunction cause lower cardiac output while decreased skeletal muscle oxidative fibers, enzymes and capillarity cause arteriovenous oxygen difference, leading thus to decreased peak oxygen uptake in heart transplant recipients. Exercise training improves

Late dissection of the ascending aorta after previous cardiac surgery: risk, presentation and outcome.

OBJECTIVE: Aortic dissection is a potentially life-threatening condition and may follow surgical interventions as a complication with distinct presentation and high mortality. Information on the incidence and etiology of aortic dissections following cardiac surgery is sparse and inconsistent. The true incidence of this entity may so far have been underestimated. METHODS: Data of 223 operations on the thoracic aorta performed exclusively at our institution between January 1990 and May 2001 were analysed for clinical and prognostic features. Patients with Marfan syndrome and traumatic cases wer

Endovascular treatment of pediatric intracranial arteriovenous shunt.

Intracranial arteriovenous shunts (ICAVS) in young children are characterized by frequent high-flow fistulas. In association with high-flow fistulas and the physiological condition of the developing brain and heart, each ICAVS type tends to present at a certain age with unique symptoms. Vein of Galen aneurysmal malformation (VGAM) and dural sinus malformation with arteriovenous (AV) shunt tend to present in the neonate with high output cardiac failure. In infancy, VGAM, pial arteriovenous fistula (AVF) and infantile dural AVF (DAVF) tend to present with hydrodynamic disorder such as macrocepha

Experimental high thoracic spinal cord injury impairs the cardiac and cerebrovascular response to orthostatic challenge in rats.

Spinal cord injury (SCI) impairs the cardiovascular responses to postural challenge, leading to the development of orthostatic hypotension (OH). Here, we apply lower body negative pressure (LBNP) to rodents with high-level SCI to demonstrate the usefulness of LBNP as a model for experimental OH studies, and to explore the effect of simulated OH on cardiovascular and cerebrovascular function following SCI. Male Wistar rats (n = 34) were subjected to a sham or T3-SCI surgery and survived into the chronic period postinjury (i.e., 8 wk). Cardiac function was tracked via ultrasound pre- to post-SCI

New Insights into Hepatic and Intestinal Microcirculation and Pulmonary Inflammation in a Model of Septic Shock and Veno-Arterial Extracorporeal Membrane Oxygenation in the Rat.

Despite significant efforts toward improving therapy for septic shock, mortality remains high. Applying veno-arterial (V-A) extracorporeal membrane oxygenation (ECMO) in this context remains controversial. Since the cannulation of the femoral artery for V-A ECMO return leads to lower body hyperoxia, this study investigated the impact of V-A ECMO therapy on the intestinal and hepatic microcirculation during septic shock in a rodent model. Thirty male Lewis rats were randomly assigned to receive V-A ECMO therapy with low (60 mL/kg/min) or high (90 mL/kg/min) blood flow or a sham procedure. Hemod

Assessment of the frequency and risk factors of gastrointestinal bleeding after cardiopulmonary bypass in paediatric cases.

INTRODUCTION AND AIM: Gastrointestinal bleeding is a potential complication in paediatric patients undergoing cardiopulmonary bypass, as it develops secondary to low gastrointestinal perfusion. This study aimed to examine the incidence of gastrointestinal bleeding and identify its risk factors in these patients. METHOD: This retrospective study was undertaken to examine the demographic features, clinical findings, and operative data of paediatric patients under years old who had undergone congenital heart surgery with cardiopulmonary bypass between November 1, 2021, and November 1, 2023. Administration of granulocyte-colony stimulating factor accompanied with a balanced diet improves cardiac function alterations induced by high fat diet in mice.

BACKGROUND/OBJECTIVES: High fat diet (HFD) is a major contributor to the development of obesity and cardiovascular diseases due to the induction of cardiac structural and hemodynamic abnormalities. We used a model of diabetic cardiomyopathy in C57Bl/6 mice fed with a HFD to investigate the effects of granulocyte-colony stimulating factor (G-CSF), a cytokine known for its beneficial effects in the heart, on cardiac anatomical and functional abnormalities associated with obesity and type 2 diabetes. METHODS: Groups of C57Bl/6 mice were fed with standard diet (n = 8) or HFD (n = 16). After 36 we

Regional anaesthesia for a Caesarean section in women with cardiac disease: a prospective study.

BACKGROUND: We conducted a prospective observational survey of pregnant women with cardiac disease. The aim was to analyse and present the mode of delivery, outcome, and haemodynamic changes during a caesarean section under regional anaesthesia in women with cardiac disease. METHODS: All pregnant women with a cardiovascular diagnosis, except hypertension, were included in the registry. Based on the cardiac diagnoses, and on the New York Heart Association classification, a multidisciplinary group made recommendations for each patient and decided on the mode of delivery. Cardiovascular function and treatment in β-thalassemia major: a consensus statement from the American Heart Association.

This aim of this statement is to report an expert consensus on the diagnosis and treatment of cardiac dysfunction in β-thalassemia major (TM). This consensus statement does not cover other hemoglobinopathies, including thalassemia intermedia and sickle cell anemia, in which a different spectrum of cardiovascular complications is typical. There are considerable uncertainties in this field, with a few randomized controlled trials relating to treatment of chronic myocardial siderosis but none relating to treatment of acute heart failure. The principles of diagnosis and treatment of cardiac iron l

High-Output Heart Failure from A Decade-Old Traumatic AV Fistula: A Case Report.

BACKGROUND: Chronic traumatic arteriovenous fistulas (AVFs) are uncommon but potentially devastating sequelae of penetrating vascular injuries. Persistent high-flow shunting can lead to progressive cardiopulmonary remodeling, pulmonary hypertension, atrial arrhythmias, and high-output heart failure (HOHF). CASE PRESENTATION: We report a 41-year-old male with a chronic high-flow AVF between the left mid-superficial femoral artery and superficial femoral vein secondary to a gunshot wound sustained over a decade prior. Initially managed nonoperatively and later lost to follow-up, he presented wi

Stress Echocardiography in the Diagnosis and Evaluation of Pulmonary Hypertension: Practical Recommendations, Haemodynamic Phenotyping, and Application in Adults and Children.

Pulmonary hypertension (PH) is a complex condition in which early diagnosis remains challenging, particularly in patients with exertional symptoms and normal or borderline resting haemodynamics. Although right heart catheterisation is the diagnostic gold standard, transthoracic echocardiography is the recommended first-line non-invasive test. However, resting echocardiography provides only a static assessment and may underestimate disease severity in early or latent pulmonary vascular disease due to preserved pulmonary vascular compliance and adaptive right ventricular responses. Because pulmo

Acute cardio-renal and Reno-Cardiac syndromes in critical care.

BACKGROUND: Cardiorenal syndrome (CRS) encompasses a spectrum of disorders in which acute or chronic dysfunction of the heart or kidneys are the key drivers of injury. CRS types 1 and 3 are the subtypes most frequently encountered in the intensive care unit (ICU) and carry a mortality exceeding 50% when renal replacement therapy is required. METHODS: We performed a narrative review of the literature published between January 2000 and October 2025, searching PubMed, OVID, and Embase using the terms "cardiorenal syndrome", "renocardiac syndrome", and "intensive care unit". Studies involving hum

Individualized Perioperative Blood Pressure and Fluid Therapy in Esophagectomy: A Prospective, Single-blind Randomized Controlled Trial.

BACKGROUND: Esophagectomy is a key treatment for esophageal cancer but carries a high risk of postoperative complications, some of which are potentially preventable through optimized hemodynamic management. Goal-directed fluid therapy individualizes cardiac output targets but often applies fixed blood pressure thresholds and is discontinued before major postoperative fluid shifts occur. Extending goal-directed fluid therapy into the postoperative period with individualized blood pressure thresholds may address these limitations. METHODS: In this single-center, prospective, blinded, randomized

The Predictive Value of Clinical Signs to Identify Shock in Critically Ill Patients.

Background/Objectives: Current guidelines recommend the use of clinical signs to diagnose shock and cellular hypoperfusion in critically ill patients. However, these recommendations are based on limited scientific evidence. The objective was to determine the predictive value of clinical signs to identify shock. Methods: Retrospective cohort study including adult (≥18 years) patients admitted to the critical care resuscitation unit of a tertiary hospital. The primary goal was to determine the predictive value of tachycardia, prolonged capillary refill time (CRT), skin mottling, weak radial puls

K(+)-channel openers: new antihypertensive drugs?

K(+)-channel openers are a new class of drugs. They interact in a not yet identified fashion with a not yet identified population of K+ channels resulting in an increase in K+ conductance of the surface membrane. K(+)-channel openers have a high affinity for K+ channels of smooth muscle and distinctly less for cardiac or neuronal K+ channels. The cellular effects of K(+)-channel openers in smooth muscle include hyperpolarization, decrease in membrane resistance, inhibition of propagation of excitation, suppression of spontaneous electrical and mechanical activity and inhibition of agonist-indu

Effects of glucose ingestion on blood flow in the small intestine and exercising limbs during endurance exercise.

Performance during endurance exercise depends on the balance between the blood flow (BF) to active muscles and their energy use. We compared the acute cardiovascular regulation after the ingestion of a 300-mL drink containing 50 g glucose (G trial) or not (water, W trial) during the ninth minute of 40 min of bilateral knee extension exercise in 12 healthy participants. The blood pressure, heart rate, cardiac output, gas exchange variables, and BF through the superior mesenteric (SMA), femoral (FA), and brachial (BA) arteries and skin in the forearm and chest were measured continuously. Quantifying Chaotic Behavior in Noisy Dynamical Systems: A Study on Heartbeat Dynamics.

BACKGROUND: Heart rate variability (HRV) series reflects the dynamical variation of R-R intervals in time and is one of the outputs of the cardiovascular system. This system has been recognized for generating nonlinear and complex dynamics, with the latter referring to a high sensitivity to small -theoretically infinitesimal - input changes. While early research associated chaotic behavior with the cardiovascular system, evidence of stochastic inputs, i.e., a physiological noise, invalidated those conclusions. AIM: We introduce a novel methodological framework for quantifying the presence of

[Relationship between cardiac output, heart rate and microcirculation in patients with multiorgan dysfunction syndrome].

BACKGROUND: Multiple organ dysfunction syndrome (MODS) is one of the main causes of death in intensive care units. There is evidence that microcirculation in sepsis and coronary shock is regulated separately from hemodynamics. This study investigates the relationship between heart rate (HR), cardiac output (CO) and microcirculation in patients with MODS. METHODS: This is a partial analysis of the "MODIFY study" (Reducing Elevated Heart Rate in Patients With Multiple Organ Dysfunction Syndrome [MODS] by Ivabradine). During the period 05/2010-09/2011, the microcirculation of 46 patients with se
