[Postoperative Neurological Deficit due to Transient Hypoperfusion in the Pretreated Contralateral Hemisphere after Bypass Surgery in an Adult Patient with Moyamoya Disease:A Case Report].

Postoperative neurological deficits frequently occur in adult moyamoya disease. In this case report, we describe the time course and disease state of a patient with adult moyamoya disease, who experienced a postoperative neurological deficit due to transient hypoperfusion in the pretreated contralateral hemisphere. A 68-year-old female presented with a sudden onset of left hemianopia due to ischemic moyamoya disease. She had severely low cerebral blood flow(CBF)in the right hemisphere and deterioration of cerebrovascular reactivity in the left hemisphere. First, right combined bypass surgery w

Indocyanine green visualization of middle meningeal artery before craniotomy during surgical revascularization for moyamoya disease.

BACKGROUND: The middle meningeal artery (MMA) is well known to function as an important collateral channel to the territory of the anterior cerebral artery in moyamoya disease. This study was aimed to evaluate whether indocyanine green (ICG) videoangiography could visualize the anterior branch of the MMA before craniotomy during surgical revascularization for moyamoya disease. METHODS: This study included 19 patients who developed TIA, ischemic stroke or hemorrhagic stroke due to moyamoya disease. Plain CT scan and three-dimensional time-of-flight MR angiography were performed in all patients

External carotid stenting for symptomatic stenosis in a patient with patent EDAS for Moyamoya disease.

BACKGROUND: Moyamoya disease is characterized by progressive narrowing of the internal carotid artery (ICA). Symptomatic patients typically undergo cerebrovascular intervention via extracranial-intracranial (EC-IC) bypass, most often with the use of the superficial temporal artery. This case of Moyamoya disease is of particular interest as the patient presented with a unilateral atherosclerotic external carotid artery (ECA) stenosis after EC-IC bypass that eliminated the benefit of his original surgery, resulting in a symptomatic presentation. CLINICAL PRESENTATION: A 53-year-old man presenti

Endovascular recanalization of symptomatic flow-limiting cervical carotid dissection in an isolated hemisphere.

OBJECT: Internal carotid artery dissection (ICAD) is a common cause of stroke in young patients, which may lead to major transient or permanent disability. Internal carotid artery dissection may occur spontaneously or after trauma and may present with a rapid neurological deterioration or with hemodynamic compromise and a delayed and unstable neurological deficit. Endovascular intervention using stent angioplasty can be used as an alternative to anticoagulation and open surgical therapy in this setting to restore blood flow through the affected carotid artery. METHODS: The authors present the

Direct bypass techniques for the treatment of pediatric moyamoya disease.

Moyamoya is an increasingly recognized cause of stroke in children and adults. Identification of the disease early in its course with prompt institution of therapy is critical to providing the best outcome for patients. Revascularization surgery seems to be effective in preventing stroke in moyamoya, with direct techniques providing durable protection when performed at experienced centers.

Sickle cell anemia with moyamoya disease: outcomes after EDAS procedure.

Moyamoya disease is a relatively uncommon neurovascular complication of sickle cell anemia. We report a case series of six patients with sickle cell anemia who developed moyamoya disease and underwent encephaloduroarteriosynangiosis procedures. These six patients presented with either cerebrovascular accidents, transient ischemic attacks, or seizures, and subsequent magnetic resonance imaging scans were suggestive of moyamoya-like changes in the cerebral vasculature. Conventional cerebral angiography was used to confirm the diagnosis in all six patients. Four of six patients manifested a cereb

Hyperperfusion syndrome after extracranial-intracranial bypass in a patient with moyamoya disease--case report.

A 47-year-old female developed hyperperfusion syndrome after superficial temporal artery-middle cerebral artery (STA-MCA) bypass for moyamoya disease. She presented with right hemiparesis and motor aphasia due to left cerebral infarction. She underwent left STA-MCA bypass. One day after surgery, she manifested neurological deterioration. Magnetic resonance (MR) imaging 4 days after the operation indicated regional edema in the territory supplied by the bypass, and single photon emission computed tomography 17 days after the operation demonstrated hyperperfusion in that area. Symptoms improved

Aneurysm arising from the petrous portion of the internal carotid artery: case report.

A 21-year-old man with hearing loss and recurrent otorrhagia was shown angiographically to have an aneurysm arising from the intrapetrous internal carotid artery (ICA). Endovascular balloon occlusion of the aneurysm with preservation of the parent ICA was attempted twice, but each time the balloon deflated and the aneurysm reappeared two weeks and three months, respectively, following the endovascular procedures. The patient was then successfully treated with a surgical trapping procedure combined with superficial temporal artery-middle cerebral artery bypass.

Indications for extra-intracranial bypass surgery. New orientation after the Toronto Bypass Study based on angiographic and non-invasive ultrasound flow measurements.

UNLABELLED: Angiographic and flow measurement results in 18 cases, who underwent extra-intracranial bypass surgery, are presented. The method was the Mavis ultrasound technique. MAIN RESULT: Patients with unilateral internal carotid artery (ICA) occlusion and additional contralateral ICA stenosis or occlusion had a permanent cerebral blood flow (CBF) increase as a consequence of the anastomosis. On the contrary, patients without contralateral flow impairment or with good spontaneous extra-intracranial anastomosis did not have a real CBF improvement but only a temporary flow increase on the an

External carotid-vertebral artery anastomosis for vertebrobasilar insufficiency.

Surgical treatment of vertebrobasilar insufficiency has long lagged behind that of carotid disease. However, as more aggressive microsurgical techniques have been developed, surgical intervention in vertebrobasilar occlusive disease is seen with increasing frequency. The authors describe three cases of external carotid vertebral artery anastomosis, a rarely reported operative procedure, in which blood flow was successfully restored to the diseased vertebral artery. Awareness of this new technique will be useful to the radiologist servicing an active neurosurgery practice. The clinical, radiolo

Intracranial aneurysm associated with moyamoya disease in childhood.

We report a 6-year-old girl with an intracranial aneurysm associated with moyamoya disease. The patient did not have a subarachnoid hemorrhage but had an ischemic attack. The aneurysm, located on the proximal portion of the lateral posterior choroidal artery, disappeared after external carotid-internal carotid anastomosis. The characteristics of the aneurysms associated with moyamoya disease are briefly reviewed. We emphasize that the treatment of choice in moyamoya disease is cerebral revascularization, because it reduces the increased blood flow through the moyamoya vessels, basilar artery a…

Bilateral occlusive carotid disease: simultaneous or staged operation?

In a series of 231 carotid operations in 202 patients, 8 patients were operated on bilaterally by staged (group A), and 15 patients by simultaneous procedures (group B). Additionally 4 patients had a combined external carotid desobliteration plus extra-intracranial arterial bypass (group C), 3 of them in a simultaneous operation. Patients of the groups A and C had an uneventful recovery throughout. An additional group A patient, however, died during the staging interval due to an ischemic cerebral infarction at the not-yet-operated side. In group B one patient died after myocardial reinfarctio

Preoperative computed tomography perfusion in pediatric moyamoya disease: a single-institution experience.

OBJECTIVE: Moyamoya disease is a progressive occlusive arteriopathy for which surgical revascularization is indicated. In this retrospective study, the authors investigated the use of preoperative CT perfusion with the aim of establishing pathological data references. METHODS: The authors reviewed the medical records of children with moyamoya disease treated surgically at one institution between 2016 and 2019. Preoperative CT perfusion studies were used to quantify mean transit time (MTT), cerebral blood volume (CBV), cerebral blood flow (CBF), and time to peak (TTP) for the anterior, middle,

Rapid intracranial clot removal with a new device: the alligator retriever.

BACKGROUND AND PURPOSE: Despite availability of an approved drug to treat acute cerebral ischemia, most patients with stroke do not realize a good outcome. A method that would rapidly increase or restore cerebral perfusion before irreversible cell death should improve patient outcomes. MATERIALS AND METHODS: We recently had the opportunity to treat 6 middle-aged-to-elderly patients who presented with signs and symptoms of acute cerebral ischemia, by mechanically removing their (predominantly) middle cerebral artery clots by using a new retrieval device that had been previously approved by the

Superficial temporal artery to middle cerebral artery bypass with interposed saphenous vein graft in patients with atherosclerotic internal carotid artery occlusive disease and impaired cerebral hemodynamics.

BACKGROUND: The aim of this study was to evaluate the efficacy of interposed saphenous vein graft (SVG) usage during extracranial-intracranial (EC-IC) bypass surgery in patients with cerebral ischemia and appropriate donor artery absence. METHODS: Since October 2010 till January 2014, 62 EC-IC bypass surgeries were applied in 60 patients. In nineteen patients SVG was used to create low-flow bypasses between superficial temporal artery trunk and cortical segment of middle cerebral artery. SVG was used due to inappropriate diameter of scalp artery in 12 cases, inadequate flow in intended donor

Role of Point-of-Care Cerebral Ultrasonography in Determining the Etiology of Post-carotid Endarterectomy Deficit: A Case Report.

Carotid revascularization surgery is notorious for its neurological morbimortality. We report the case of a 74-year-old hypertensive patient, who underwent left internal carotid artery endarterectomy for a 90% stenosis under general anesthesia, presenting in the immediate postoperative period with right hemiplegia without consciousness disorders. Evaluation by cerebral ultrasound at bedside led to suspicion of intracerebral hemorrhage, which was confirmed by cerebral CT scan. The patient was treated by neuroresuscitation measures in the absence of the possibility of surgical intervention. This

Intravenous Thrombolysis Facilitates Successful Recanalization with Stent-Retriever Mechanical Thrombectomy in Middle Cerebral Artery Occlusions.

AIM: Several factors influence the outcome after acute ischemic stroke secondary to proximal occlusions of cerebral vessels. Among others, noneligibility for intravenous thrombolysis (IVT) and incomplete revascularization have been identified as predictors of unfavorable outcome. The aim of this study was to investigate whether concomitant IVT influences the revascularization efficacy in mechanical thrombectomy (MT). METHODS: This study conducted a retrospective analysis of all consecutive patients presenting with an anterior circulation stroke due to large-artery occlusion with imaging evide

Extracranial-intracranial bypass for symptomatic occlusive cerebrovascular disease not amenable to carotid endarterectomy.

OBJECT: The role of cerebral revascularization remains unclear in symptomatic occlusive cerebrovascular disease refractory to medical therapy. Despite the disappointing findings of the Cooperative Study on Extracranial-Intracranial Bypass, a subpopulation of patients with ischemic cerebrovascular disease and poor hemodynamic reserve may benefit from extracranial-intracranial (EC-IC) bypass. The authors reviewed the records of 65 patients who underwent 71 EC-IC bypass procedures at their institution over the past 6 years. METHODS: All patients except one presented with repeated transient ische…
