BACKGROUND: Traumatic atlantoaxial dislocation combined with locked atlas lateral mass and odontoid process fracture is a complex injury and is extremely rare. We describe the surgical technique by presenting a clinical case study in managing a traumatic lateral atlantoaxial dislocation combined with a locked atlas lateral mass and a type II odontoid fracture (Grauer type IIB).
CASE DESCRIPTION: This is a clinical case study of a 38-year-old female patient who presented with severe neck pain without neurological deficit following a traffic accident. Computed tomography showed a type IIB odont

Thirty-four patients who had fractures of the atlas (the first cervical vertebra) were reviewed at an average follow-up of 4.5 years. Seventeen patients had bilateral fracture of the posterior arch of the first cervical vertebra. Eight were treated with immobilization in a cervical orthosis, with no long-term problems secondary to the injury. Nine of these patients had additional fractures in the first and second cervical vertebral complex, complicating the management of the fractures of the posterior arch. Two of the nine patients died, and the treatment of the other seven was dependent on th

OBJECTIVE: To describe the chiropractic management of a patient with paresthesia on the entire left side of her body and magnetic resonance imaging (MRI)-documented cervical spinal cord deformation secondary to cervical spinal stenosis.
CLINICAL FEATURES: A 70-year-old special education teacher had neck pain, headaches, and burning paresthesia on the entire left side of her body. These symptoms developed within hours of being injured in a side-impact motor vehicle accident. Prior to her visit, she had been misdiagnosed with a cerebrovascular accident.
INTERVENTION AND OUTCOMES: Additional di

Globally, cervical cancer is a prevalent cancer among women and ranks as the fourth leading cause of mortality in gynecological cancers. However, how telomeres affect cervical cancer remains uncertain. The research entailed the procurement of telomere-associated genes (TRGs) from TelNet. The Cancer Genome Atlas (TCGA) database provided the clinical data and TRG expression rates in patients with cervical cancer. Within the TCGA-CESC dataset, 327 TRGs were identified, differentiating between cancerous and healthy tissues. Subsequently, we combined genes linked to aging and telomeres, leading to

In India, Atlantoaxial dislocation (AAD) is the commonest skeletal craniovertebral junction (CVJ) anomaly, followed by occipitalisation of atlas and basilar invagination. The usual presentation is progressive neurological deficit (76-95% cases) involving the high cervical cord, lower brainstem and cranial nerves. The association between vertebro-basilar insufficiency and skeletal CVJ anomalies is well recognised and angiographic abnormalities of the vertebrobasilar arteries and their branches have been reported; however, initial presentation of CVJ anomaly as thalamic syndrome due to posterior

INTRODUCTION AND OBJECTIVES: The objective of the study was: (1) to describe changes in the shape of the atlas during growth, including gender and side differences; (2) to assess the dimension essential for identification of the optimal entry point; (3) to determine the age limit for a safe insertion of 3.5-mm screws into the lateral masses according to our own limiting parameters.
MATERIALS AND METHODS: Dimensions of the atlas were measured on 200 CT scans of the craniocervical junction in individuals aged 0-18 years and on 34 anatomical specimens of the first cervical vertebra (aged 2.5-18

BACKGROUND: Atlas fracture occurs in 3%-13% of all cervical spinal injuries and is often associated with other injuries. The factors associated with concomitant transverse ligament disruption and vertebral artery injury remain underexamined.
METHODS: We retrospectively reviewed 97 consecutive cases of atlas fractures. We analyzed demographic and clinic characteristics, including mechanism of injury, fracture type, and associated injuries. We identified factors independently associated with vertebral artery injury and/or transverse ligament disruption.
RESULTS: On multivariable analysis, vert

BACKGROUND: Understanding common and unique mechanisms driving oncogenic processes in human tumors is indispensable to develop efficient therapies. Recent studies have proposed Twinfilin Actin Binding Protein 1 (TWF1) as a putative driver gene in lung cancer, pancreatic cancer and breast cancer, however a systematic pan-cancer analysis has not been carried out.
METHODS: Here, we set out to explore the role of TWF1 in 33 tumor types using TCGA (The Cancer Genome Atlas), GEO (Gene Expression Omnibus) dataset, Human Protein Atlas (HPA), and several bioinformatic tools.
RESULTS: As part of our a

Beauty parlor stroke syndrome (BPSS) is a rare condition characterized by mechanical impingement of a vertebral artery (VA) during neck rotation and/or hyperextension followed by vertebrobasilar insufficiency. However, there have been no reports of BPSS in which the cause of mechanical impingement was identified and no cases for which surgical treatment was reported. The authors report the case of a 56-year-old Japanese man who presented with presyncope that occurred during cervical extension. Given the possibility of vertebrobasilar insufficiency, digital subtraction angiography and CT angiog

OBJECTIVE: This study reviews the relevant literature regarding the management of craniovertebral junction (CVJ) metastases. These rare tumors present significant diagnostic and treatment challenges.
METHODS: A PubMed search of cervical spine, cervical spine metastasis, craniovertebral junction, atlantoaxial spine, and metastasis radiation was conducted to define the epidemiology, imaging, and treatment protocols in the management of metastatic CVJ tumors.
RESULTS: CVJ tumors represent less than 1% of spinal metastases, and the literature is limited to small case series. CVJ tumors present w

Surgical decompression of the vertebral artery (VA) between the suboccipital area and C-1 is typically performed via a large midline incision or a far-lateral approach. Such traditional open approaches are often associated with significant muscle dissection and blood loss. In this case, a 12-year-old boy suffered a stroke related to a VA rotational occlusion (bow hunter syndrome) and dissection due to a prominent suboccipital bone mass. Successful decompression of the VA was performed via a minimally invasive 22-mm tubular retractor. This is the first reported case report of a minimally invasi

Atlantal, long bone, and craniofacial measurements of prehistoric Polynesian and Thai skeletons were analysed to ascertain the extent and direction of bilateral asymmetry. The atlantal articular mass was constantly and significantly higher on the right side than on the left. In Thais, the midlongitudinal axis of the atlanto-occipital facet on the right side had a steeper inclination than that on the left. In both groups the dental occlusion was less mesial on the right side and the mandible was shorter, significantly so in the Thais. The long bones were more asymmetric in the Polynesians than

Cervical spines (with occiputs) of 44 patients with rheumatoid arthritis (RA) were compared with 44 control spines from patients without RA, matched for sex and age. There were six severe and seven slight atlanto-axial dislocations (both horizontal and vertical, two of which had been treated surgically) in the RA group and none in the control group. Ankylosis, synchondrosis, syndesmosis, bone erosions, and redness of synovial tissue in the atlanto-axial, atlanto-occipital and cervical intervertebral joints were more frequent in the RA group than in the control spines. INTRODUCTION: The unstable atlas burst fracture ("Jefferson fracture") is a fracture of the anterior and posterior atlantal arch with rupture of the transverse atlantal ligament and an incongruence of the atlanto-occipital and the atlanto-axial joint facets. The posterior atlantoaxial fusion is frequently used to reconstruct the stability of atlantoaxial joint. Conventional posterior atlantoaxial fixations are associated with high rates of pseudoarthrosis and chronic atlantoaxial instability. As a modified three-point fixation the bilateral C1-2 transarticular screws combined with C1 laminar h

Many injuries to the cervical spine can be treated conservatively. Treatment options include early functional, mobilizing and immobilizing procedures. If a structural injury can be ruled out by morphological imaging, early functional mobilization should be performed in combination with adequate analgesia according to the World Health Organization (WHO) step by step scheme to avoid chronification. If a structural injury to the cervical spine is present, a stability test is crucial for the decision on treatment. Stable fractures include, for example, types I, II and V fractures of the atlas acco

BACKGROUND: Cervical cancer, prevalent in low- and middle-income countries, is primarily caused by high-risk HPV16. Vesicle-Associated Membrane Protein 8 (VAMP8), involved in vesicle trafficking and autophagy, may influence HPV16-related cervical cancer progression.
METHODS: VAMP8 expression was evaluated in cervical tissue specimens from patients with HPV16-positive lesions (including low- and high-grade squamous intraepithelial lesions and cancer) and HPV-negative normal controls using proteomics, qPCR, and immunohistochemistry. A Cox proportional hazards model for prognosis was developed u
