Laparoscopic surgery in very acute cholecystitis.

The objective of this study was to demonstrate the safety and feasibility of laparoscopic cholecystectomy in empyematous or gangrenous cholecystitis. During the period from August 1998 to April 2000, we operated laparoscopically on 64 patients, without any selection, in which we established, preoperatively or intraoperatively, the diagnosis of empyematous or gangrenous cholecystitis using clinical criteria (fever, leukocytosis, persistent pain, abdominal tenderness or guarding), echographic findings and intraoperative or pathological aspects of the gallbladder. The operations were performed by…

Byler disease associated with acute cholecystitis.

A case of Byler disease, which has a rare incidence of l in 90000, is reported. A 21-year-old woman known to have the disease since childhood presented with acute cholecystitis and underwent laparoscopic cholecystectomy. To the best of our knowledge, after a review of the English literature, this is the second patient with Byler disease to live beyond the second decade of life since the disorder was first described by Clayton 30 years ago, and the first case associated with acute cholecystitis that was managed successfully by laparoscopic cholecystectomy.

[Surgical treatment in acute cholecystitis emergencies].

The aim of the study was to demonstrate the importance of early laparoscopic cholecystectomy for acute cholecystitis, without "conservative" treatment (intravenous fluids and antibiotics for 48-72 hours) to reduce inflammation. Early laparoscopic cholecystectomy reduces bile duct injury and the percentage of conversion to open cholecystectomy. Thirty-five patients with acute cholecystitis were submitted to early laparoscopic cholecystectomy, equally divided according to sex. All patients were submitted to US scans preoperatively and operated on by surgeons skilled in emergency laparoscopic ope…

Pseudo-Mirizzi syndrome in acute cholecystitis.

Common hepatic duct obstruction secondary to an impacted cystic duct stone is commonly referred to as the Mirizzi syndrome. Mirizzi syndrome is an uncommon cause of obstructive jaundice and can be mimicked by several other rare conditions. We describe a patient with a massively distended gallbladder due to acute cholecystitis who presented with clinical and cholangiographic findings simulating the Mirizzi syndrome. Endoscopists should be aware of acute cholecystitis as a possible etiology of common hepatic duct obstruction.

Early laparoscopic cholecystectomy for acute cholecystitis.

BACKGROUND: The timing of laparoscopic cholecystectomy for acute cholecystitis remains controversial. METHODS: One hundred ninety-four patients with acute cholecystitis were reviewed. The conversion rates for the various number of days of symptoms before surgery were analyzed. The conversion rate dramatically increased from 3.6% for those patients with 4 days of symptoms to 26% for those patients with 5 days of symptoms. The mean number of days of symptoms prior to surgery in those patients who underwent successful laparoscopic cholecystectomy was 4.1 as compared to 8.0 in those patients who …

[Surgical problems in acute acalculous cholecystitis].

The authors report on the prevalence of acute cholecystitis in the absence of calculi with special attention paid to the development of this disease as a complication of prolonged parenteral nutrition. In 1990-1993 at the Surgical Clinic in Plzen 125 cases of acute cholecystitis were operated, incl. 14 acalculous ones (11.2%). These 14 patients comprised 5 (28.6%) who were on parenteral nutrition for a prolonged period. The authors analyze the causes of the development of acute acalculous cholecystitis. They deal with the problem of diagnosis where in severe postoperative and posttraumatic cas…

Xanthogranulomatous cholecystitis masquerading as gallbladder carcinoma.

We herein present a case of xanthogranulomatous cholecystitis which involved both the liver and transverse colon, clinically mimicking gallbladder carcinoma. Such cases may sometimes be judged inoperable due to extensive extra-gallbladder invasion, and thus it is necessary for physicians to take this lesion into consideration when making a diagnosis. An intraoperative biopsy is necessary, therefore, even when the features seem to clearly indicate inoperable carcinoma.

[Percutaneous gallbladder drainage in acute cholecystitis].

12 high-risk patients underwent percutaneous cholecystostomy for serious acute cholecystitis using a transhepatic approach under ultrasonic guidance. Eight patients had acute acalculous cholecystitis, and four had acute calculous cholecystitis. Following the drainage procedure a mean decrease in body temperature of 0.7 degrees centigrade was noted in 11 patients and a reduction of pain and defence in five out of nine patients. Two patients had a perforated gallbladder and pericholecystic abscess; both cases were successfully treated by percutaneous drainage. Four patients with acute acalculous…

[Intralymphatic antibacterial therapy of acute cholecystitis].

Clinical examination of 34 patients showed that lymphotropic administration of antibiotics creates concentrations in organs and tissues involved in the inflammatory process in acute cholecystitis which are 1.63-14.95 times higher than those produced by intramuscular injection. Study of the course of the clinical picture of the disease in 122 patients with acute destructive cholecystitis revealed the high efficacy of this method of antibiotic therapy: the laboratory indices of the inflammation became normal within a shorter period of time, the inflammatory process in the gallbladder was arreste…

Gangrenous cholecystitis: new observations on sonography.

We studied 25 patients with gangrenous cholecystitis and observed a new sonographic finding--striated thickening of the gallbladder wall--and three patterns of pericholecystic fluid collections. Heterogeneous thickening of the gallbladder wall was characterized by either multiple striations (alternating hypoechoic and hyperechoic layers) or irregular mass-like protrusions projecting into the gallbladder lumen. We observed striated thickening far more frequently (in 10 of 25 patients) than other findings reported previously as being associated with gangrenous cholecystitis, such as intraluminal…

[Duodenal motility disorders in calculous cholecystitis].

In acute calculous cholecystitis, the impairment of motor duodenal function was revealed in 81.6% of the patients, in chronic calculous cholecystitis-in 72.2%. Hypotension in the duodenum is fraught with the development of duodenostasis after cholecystectomy. The reliability of intraoperative duodenal manometry in studying the motor duodenal activity in patients with calculous cholecystitis is substantiated.

[Clinico-pathological study of xanthogranulomatous cholecystitis].

Xanthogranulomatous cholecystitis (XGC) is an uncommon lesion which may form a tumor-like mass in inflamed gallbladders. In a review of 44 cases there were 40 associated with gallstones which had been incarcerated in the neck of the gallbladder, 10 with past histories of abdominal surgeries, 15 with diabetes mellitus, three with carcinomas in the neck of the gallbladder and four with carcinomas in the other organs. Radiologically the differential diagnosis of gallbladder cancer and XGC was difficult in several cases. Thirty five cases of XGC have been diagnosed as chronic cholecystitis and 7 h…

[Predicting surgical risk in acute cholecystitis].

From mathematical treatment of the results of operative treatment of 163 patients with acute cholecystitis on a computer with the use of program complexes on the basis of the mathematical theory of image recognition the authors obtained a decisive rule which allowed calculation of the prognostic index reflecting the severity of the patient's condition and the prognosis of the surgical treatment according to the initial values of total protein, leucocytosis, AlAT, and the patient's age. Besides, the efficacy of the preoperative management can be judged from the dynamics of changes of the index.…

Infective endocarditis related to acute cholecystitis.

A 30-year-old woman with a homologous dura-mater valve prosthesis developed infective endocarditis related to acute purulent cholecystitis occurring in chronic cholelithiasis. During anti-bacterial treatment refractory heart failure ensued which compelled replacement of the infected prosthesis. She died in the post-operative period. Autopsy revealed endocarditis on the implanted bioprosthesis, purulent cholecystitis and a gallbladder stone. It is warningly concluded that the surgical treatment of endocarditis was unsuccessful because of persistence of purulent cholecystitis that led to reinfec…
