Candidacy for dorsal preservation rhinoplasty (DPR) techniques is determined by a combination of preoperative nasal analysis and intraoperative assessment of the bony vault and septum. Classic ideal candidates have a pleasing frontal aesthetic, shorter nasal bones, bony cap at or cephalic to the rhinion, straight and appropriately narrow dorsal aesthetic lines (DALs), and a small to medium-sized hump.Alongside the growing popularity of DPR in recent years there have been numerous new methods described for approaching the bony vault and septum that expand both the definition of dorsal preservat

Velopharyngeal dysfunction (VPD) is the inability to achieve proper closure of the velopharyngeal (VP) port, affecting speech and swallowing. After an auditory-perceptual speech evaluation by a speech-language pathologist, objective assessment of the VP port is required to determine the need for surgical intervention. This 3-part series provides a comprehensive discussion on (1) the anatomy and physiology of the velopharyngeal mechanism; (2) fundamental speech terminology and principles of perceptual speech assessment for VPD; and (3) techniques for objective evaluation of the VP port and surg

BACKGROUND: Uncertainty around a patient's prognosis at the end of life remains a major barrier to timely palliative-care involvement and alignment of treatments with patient goals. Artificial intelligence (AI)-based tools have recently emerged to provide structured mortality predictions and identify patients at risk of deterioration to support clinical decision-making.
OBJECTIVE: This mini-review summarises recent literature evaluating AI-based prognostic and decision-support tools in end-of-life and palliative care, focusing on predictive accuracy, early implementation outcomes, communicati…

Training to achieve expert performance requires the trainees to have intense deliberate practice performing the physical and cognitive skills of experts. Designing effective training requires explicitly identifying what experts do and the reasons for their actions. This identification is deconstructing their expertise. We discuss methods for deconstructing expertise, particularly the cognitive skills of high performing endoscopists. Through structured interviews we determined what information they sought, and how they applied that information to make decisions. We particularly focused on key f

BACKGROUND: Management of intraductal papillary mucinous neoplasms (IPMNs) requires choosing between surgical resection and active surveillance, yet current diagnostic tools cannot reliably distinguish which lesions harbor high-grade dysplasia or invasive cancer. As a result, many patients undergo major pancreatic surgery for low-grade disease, while others are observed until progression emerges. This uncertainty contributes to substantial variation in surgeon risk estimates, intervention thresholds, and treatment recommendations. Patients likewise differ in their tolerance for cancer risk, vi

BACKGROUND: Low back pain (LBP) is a common complex condition, where specific diagnoses are hard to identify. Diagnostic clinical prediction rules (CPRs) are known to improve clinical decision-making. A review of LBP diagnostic-CPRs by Haskins et al. (2015) identified six diagnostic-CPRs in derivation phases of development, with one tool ready for implementation. Recent progress on these tools is unknown. Therefore, this review aimed to investigate developments in LBP diagnostic-CPRs and evaluate their readiness for implementation.
METHODS: A systematic review was performed on five databases …

Evolution in the management of Crohn's disease [CD] has been characterized by recent paradigm changes. First, new biological therapies induce intestinal healing and full disease control in a substantial number of patients, particularly when introduced early in the disease course. However, they are expensive and associated with potentially severe side effects, raising the question of optimal treatment duration. Secondly, progress in biomarkers and medical imaging performance has enabled better refinement of the definition and prediction of remission or relapse of the disease through monitoring

INTRODUCTION: Cancer treatment decision making (CTDM) presents important challenges among older adults due to variation in health and functional status, presence of comorbidities, differing goals/values, quality and quantity of life considerations, and limited inclusion in clinical trials. Nursing standards and guidelines call for competence in supporting CTDM and nurses advocate for greater involvement. However, clear understanding of the existing evidence to inform nursing practice is lacking. We aimed to map and synthesize evidence that provides insight into nursing practice in CTDM among o

Although surgery is unambiguously recommended for symptomatic primary hyperparathyroidism (PHPT) patients, management options for asymptomatic patients have varied between surgery, medications and follow-up. To deal with this issue, four International Workshops have taken place, in 1990, 2002, 2008 and 2014, during which a team of experts drew up criteria to stratify asymptomatic patients as surgery-eligible and surgery-ineligible. The efficacy of the criteria have however been questioned over the years and the majority of patients, both eligible and ineligible, may eventually have undergone s

BACKGROUND: Treatment choices in hepatocellular carcinoma (HCC) involve consideration of tradeoffs between the benefits, toxicities, inconvenience, and costs. Stated preference elicitation methods have been used in the medical field to help evaluate complex treatment decision-making. The aim of this study was to conduct a scoping review to assess the evidence base for the use of preference elicitation tools or willingness to pay/willingness to accept methods for HCC treatment decision-making from both the patient and provider perspective.
METHODS: We performed a scoping review to identify abs

BACKGROUND: Shared decision-making (SDM), which increases the patient's well-being, adherence, and success of treatment, is becoming increasingly important in medicine and especially in oncology. To empower patients to participate more actively in consultations with their physicians decision aids have been developed. In non-curative settings, such as the treatment of advanced lung cancer, decisions differ substantially from the curative setting, as uncertain gains in terms of survival outcomes and quality of life have to be weighed against the severe side effects of treatment regimens. There i

RATIONALE: Post-test probabilities are reported as precise numbers even though the pretest probabilities they depend on are uncertain and, for an individual patient, unobservable. Clinicians need a way to tell the mathematical question-how strongly is a change in the pretest estimate carried through to the post-test probability-apart from the clinical question of whether the remaining uncertainty is wide enough to alter management.
AIMS AND OBJECTIVES: To show that the prevalence threshold ( ϕ e ) marks the tipping point at which uncertainty expressed in percentage points is passed on un

INTRODUCTION: Traditional cardiology orientation training offers limited exposure to acute overnight decision-making under high cognitive load. Simulation provides a safe, reproducible environment for deliberate practice and reflection.
METHODS: A multi-scenario, high-fidelity "night-float" simulation was implemented for incoming cardiology fellows at the NCH Rooney Heart Institute. Fellows managed sequential acute cardiovascular emergencies with embedded nurses and standardized patients, followed by structured debriefing using the PEARLS framework. Pre- and post-simulation surveys assessed c

OBJECTIVE: To evaluate the concordance between Thoracolumbar Injury Classification and Severity Score (TLICS) recommendations and actual treatment decisions in thoracolumbar trauma.
SUMMARY OF BACKGROUND DATA: TLICS is widely used to guide management of thoracolumbar fractures, recommending nonoperative treatment for scores ≤3 and operative intervention for scores ≥5. However, treatment variability may occur in cases with indeterminate scores, uncertainty in posterior ligamentous complex (PLC) integrity, or clinical modifiers not fully cap

During multidisciplinary team meetings, treatment plans are discussed for patients with colorectal cancer. Guidelines and protocols guide these decisions, but these are also dependent on the perception of the treatment outcomes by the involved specialists. With the increasing treatment options for increasingly older patients with more comorbidities, predicting outcomes for specific subgroups becomes more challenging. In a survey pertaining to treatment outcomes, we found that surgeons' estimations are not very accurate, especially not for specific risk groups. The limited accuracy of predicted

Molar-incisor hypomineralization (MIH) presents a significant clinical challenge in orthodontics, with its complex etiology, variable presentation, and profound impact on enamel homeostasis. Utilizing Simon Sinek's "Golden Circle" method, this article systematically explores the implications of MIH within orthodontic practice. The "Why" emphasizes the critical need for orthodontists to understand MIH, given its prevalence and potential to disrupt treatment outcomes. MIH affects up to 40.2% of the global population, and its association with increased caries risk, hypersensitivity, and restorati
