Closed medial total subtalar joint dislocation without ankle fracture: a case report.

INTRODUCTION: Total subtalar dislocation without fracture of the ankle is a rare clinical entity; it is usually due to a traumatic high-energy mechanism. Standard treatment is successful closed reduction under general anesthesia followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 weeks. CASE PRESENTATION: We present the case of a 30-year-old Moroccan woman who was involved in a road traffic accident. She subsequently received a radiological assessment that objectified a total subtalar dislocation without fracture of her ankle. She was immediately admitted to t

Percutaneous intramedullary fixation of lateral malleolus fractures: technique and report of early results.

Twenty-four patients with Weber B and low Weber C displaced lateral malleolus fractures who underwent closed reduction and percutaneous internal fixation with an intramedullary, fully threaded, self-tapping screw were retrospectively reviewed. Nineteen of these patients were followed for an average of 63.4 weeks. A good radiographic reduction was obtained in 87.5% of patients, a fair reduction in 8.3%, and a poor reduction in 4.2%. The reduction that was obtained was maintained in all patients. Fracture union was achieved in 95.5% of patients, with an average time to union of 8.2 weeks. In all

[Verification of the Necessity for the Open Reduction and Tension Band Wiring of the Displaced Olecranon Fracture in Younger Children].

PURPOSE OF THE STUDY Olecranon fractures in skeletally immature patients are rather rare and represent up to 7% of elbow skeletal injuries. Although the majority of olecranon fractures is constituted by undisplaced fractures treated conservatively with good outcomes, a few of them require surgery. The aim of the study was to compare two different approaches of surgical treatment - the open reduction with tension band wiring - cerclage (ORCe) and the closed reduction and percutaneous pinning (CRPP). MATERIAL AND METHODS 37 patients (28 boys, 9 girls) were included in the retrospective multicent

Elastic intramedullary nailing for recurrent radial fracture associated with persistent ulnar bowing deformity in a pediatric patient: a case report.

BACKGROUND: Pediatric forearm bowing fracture is an occult incomplete injury with a high misdiagnosis rate. Persistent ulnar bowing deformity is a potential risk factor for recurrent radial fracture due to altered forearm biomechanical stress distribution, and unified management protocols for this combined injury remain lacking. This case report describes the diagnosis and treatment of an 8-year-old girl who sustained a second ipsilateral radial fracture four years after an initial forearm trauma accompanied by ulnar bowing deformity at 4 years old, aiming to provide practical clinical evidenc

Open versus closed nailing of femoral fractures in the polytrauma patient.

Thirty-four patients with severe multiple injuries underwent either open or closed nailing of 35 femoral fractures. Open nailing was performed in 17 femurs and closed nailing in 18 femurs. The average abbreviated injury score was 27 in both the open group (range: 17-45) and closed group (range: 22-36). Soft tissue injuries were present in eight (47%) cases in the open group compared to three (16%) in the closed group. The treatment protocol was similar in both groups. Intramedullary nailing was delayed an average of 11 days in the closed group. This was significantly different than the open gr

Trochanteric osteotomy for safe surgical approach to bilateral hip dislocations with femoral head fractures.

A 23-year old male with bilateral hip dislocations and associated femur head fractures (Pipkin type-II) presented with pain and flexion deformity of both hips after 9 days. After imaging, closed reduction was attempted but failed. Open reduction through Kocher-Langenbeck approach was performed and the femoral head fracture was accessed through Ganz's safe surgical dislocation. The fracture was reduced anatomically and fixed with headless Herbert screws. After two years, the patient was walking without pain or limp and there was no evidence of osteonecrosis. Simultaneous sequential Ganz's safe

The management of forearm fractures in children: a plea for conservatism.

A retrospective review was undertaken to evaluate the efficacy of primary nonoperative treatment (closed reduction and long-arm casting) along with pins and plaster as a salvage technique for those reduction failures. A total of 730 closed fractures (1987-1993) was compiled, of which 300 required closed reductions and casting. Excluded from the study were teenagers whose growth plates were closed. Of the 300 fractures requiring closed reductions, 22 went on to require remanipulations, and 12 required the use of pins-and-plaster technique to obtain or maintain satisfactory reduction. Complicati

Single-Stage Hybrid Fixation of a Grade III Open, Unstable Trimalleolar Ankle Fracture with Syndesmotic Disruption and Posterior Talar Subluxation.

INTRODUCTION: Medial epicondyle fracture dislocations are common in the pediatric population; however, they often lead to favorable patient outcomes. Rarely, the median nerve (MN) can be trapped within the joint space or fracture upon reduction. If the nerve is trapped within the fracture, the MN slowly gets compressed as the fracture heals, leading to progressive deficits in motor function and sensation. These symptoms present as a mixture of pain, numbness, weakness, and paresthesia in the distribution of the MN. Therefore, careful monitoring of the patient post-reduction is crucial in makin

Mandibular Subcondylar Fracture: Improved Functional Outcomes in Selected Patients with Open Treatment.

BACKGROUND: Subcondylar fractures represent 25 to 35 percent of all mandibular fractures, yet the treatment paradigm has remained controversial. Closed treatment relies on the plasticity of the condyle head during recovery, whereas open treatment is challenging and risks facial nerve injury. Perioperative, functional, and patient-reported outcomes were measured to compare methods of open versus closed treatment of subcondylar fractures. METHODS: Selected displaced subcondylar fracture cases with open (open reduction and internal fixation of subcondylar fracture with maxillomandibular fixation

Operative Fractures of the Phalangeal Head and Neck in Children-Does Open Reduction Affect Outcomes?

BACKGROUND: Closed reduction percutaneous pinning of displaced pediatric phalangeal head and neck fractures is preferred to prevent malunion and loss of motion and function. However, open reduction is required for irreducible fractures and open injuries. We hypothesize that osteonecrosis is more common in open injuries than closed injuries that require either open reduction or closed reduction percutaneous pinning. METHODS: Retrospective chart review of 165 phalangeal head and neck fractures treated surgically with pin fixation at a single tertiary pediatric trauma center from 2007 to 2017. F

[Dislocation-disassembly of bipolar hip arthroplasty--case report].

Bipolar hip arthroplasty dislocation is rare. A case of bipolar hip arthroplasty dislocation in patient treated because of femoral neck fracture was described. Patient had neurological problems. The arthroplasty was made with posterolateral approach. Disassembly of bipolar prosthesis occurred during closed reduction. Open reduction with bipolar head exchange was necessary. To avoid this complication reduction should be made in anesthesia with muscles relaxation.

Subcoracoid trapping of the proximal fragment of a fractured clavicle.

An unusual indication--not previously reported--for surgical treatment of a clavicle fracture, in a 56-year-old male, is presented. The proximal fragment had been entrapped under the coracoid, and there were symptoms from impairment of the musculocutaneous nerve. Reduction by closed means proved unsuccessful, and an open procedure was necessary for this rare type of the common clavicle fracture.

Fracture separation of the distal humeral epiphysis in children younger than three years old.

This study evaluated the change of carrying angle and the causes of cubitus varus after the fracture separation of the distal humeral epiphysis in young children. Twelve cases of fracture separation of distal humeral epiphysis were treated from January 1995 to July 1997. The age of all patients was younger than 3 years old. A metaphyseal fragment was seen in all cases (Salter-Harris type II), but the size of the metaphyseal fragment was either a very small flake or a large Thurston-Holland fragment. Posteromedial displacement was seen in all cases. There were three treatment groups: closed red

Research on the evaluation and optimization model of community public space during the epidemic prevention period based on TOPSIS.

BACKGROUND: Nasal bone fractures are common in children but can be challenging to diagnose accurately in the first days due to swelling and tenderness. While X-rays and computed tomography have limitations, ultrasound may be a radiation-free and cost-effective alternative for diagnosing and treating nasal fractures. METHODS: A prospective cohort study at a tertiary referral hospital between 2021-2023. Children who had sustained nasal trauma were included. A radiologist and a non-radiologist blindly reviewed ultrasound scans, and the results were compared to the physical examination performed

Improving Results in Closed Nasal Reduction: A Protocol for Reducing Secondary Deformity.

BACKGROUND: Nasal fractures are the most common facial fracture. Improper reduction is a common occurrence, resulting in a residual deformity that requires secondary surgery. A treatment protocol for nasal fracture management is presented with the aim of reducing secondary deformities requiring corrective surgery. METHODS: After institutional review board approval, a retrospective review of all closed nasal reductions performed by a single surgeon between 2006 and 2015 was conducted. Patient age, sex, presence of secondary deformity, and need for a correctional operation were recorded. Clinic

Common Errors in the Management of Pediatric Supracondylar Humerus Fractures and Lateral Condyle Fractures.

Supracondylar humerus fractures and lateral condyle fractures are the two most common pediatric elbow fractures that require surgical intervention. Although most surgeons are familiar with supracondylar humerus fractures and lateral condyle fractures, these injuries present challenges that may lead to common errors in evaluation and management and, thus, compromise outcomes. It is well agreed upon that nondisplaced supracondylar fractures (Gartland type I) are best managed nonsurgically with cast immobilization. Errors may be made, however, in the treatment of type II fractures because the ext

Analysis of Factors Affecting the Union of Closed Subtrochanteric Femur Fractures treated by Cephalomedullary Nailing.

INTRODUCTION: With surgical advancements and improved implants and instrumentation, nailing is the procedure of choice in subtrochanteric fractures. However, failure in achieving reduction of the multidirectional displacement of the fragments prior to passing the nail, contributes to delayed/non-unions at the fracture site leading to implant failures. In this study, we aim to analyse the factors affecting union of closed subtrochanteric fractures treated by nailing. MATERIALS AND METHODS: In this retrospective study, closed subtrochanteric fractures treated with cephalomedullary nailing betwe

Using the Starr Frame and Da Vinci surgery system for pelvic fracture and sacral nerve injury.

BACKGROUND: Sacral fracture and sacral nerve injury remain problems in orthopedics, especially in a sacral fracture combined with an anterior sacral nerve injury. Treating a sacral nerve injury with open reduction neurolysis or more conservative treatment cannot meet the clinical needs. Open reduction sacral nerve neurolysis will increase the number of severe, life-threatening injuries, regardless of whether the anterior or posterior approach is used. In recent years, computer- and robot-assisted orthopedic surgery has emerged as part of many clinical treatments. METHODS: For an unstable pelv
