Suicide due to four speargun shots: a case report.

In the literature, only a few cases of deaths related to the use of atypical firearms are present and even more rare are cases of suicide due to multiple lesions. In the present case, the authors report a rare occurrence of suicide due to 4 speargun shots, 3 to the chest and 1 to the head.A complete forensic approach led to attribute the death to acute cardiac failure due to hemopericardium after the injury of the left coronary artery.Scene investigation and autoptic findings allowed authors to hypothesize that injury to the head was a last attempt of suicide, elapsed during the progression of

Medical mountain rescue in the Mont-Blanc massif.

Because of its peculiar geographic location beneath Mont-Blanc, the Chamonix Hospital plays an important role in the management of mountaineering- and skiing-related traumatic injuries. The authors, thanks to the study of 5200 mountain rescue medical reports, set out to explain the different aspects of this activity: epidemiology, specific equipment, care in the field, reception and primary survey, dispatching, common therapeutic attitudes, progress, and results. In the dangerous environment of the high mountains, emphasis is put on the physicians' experience, allowing an early evacuation in t

Hypothermic neuroprotection by targeted cold autologous blood transfusion in a non-human primate stroke model.

Over decades, nearly all attempts to translate the benefits of therapeutic hypothermia in stroke models of lower-order species to stroke patients have failed. Potentially overlooked reasons may be biological gaps between different species and the mismatched initiation of therapeutic hypothermia in translational studies. Here, we introduce a novel strategy of selective therapeutic hypothermia in a non-human primate ischemia-reperfusion model, in which autologous blood was cooled ex vivo and the cool blood transfusion was administered at the middle cerebral artery just after the onset of reperfu

In vivo demonstration of a novel non-invasive model for inducing localized hypothermia to ameliorate hepatotoxicity.

Moderate hypothermia (32 °C) has been previously shown to ameliorate drug-induced liver injuries in vitro. However, there are concerns regarding its clinical relevance as it remains a challenge to perform selective liver cooling in a non-invasive manner. To reconcile this dilemma, we propose the use of pulsed cooling for regional hypothermic conditioning in liver. This involves intermittent cooling applied in pulses of 15 min each, with a one-hour recovery interval between pulses. Cooling is achieved by applying ice packs to the cutaneous region overlying the liver. Through an in vivo C57BL/6N

Comparison of three treatment procedures for minimizing ankle sprain swelling.

The purpose of this study was to compare the effects of cold, heat, and contrast bath treatments on the amount of edema in first- and second-degree sprained ankles during the postacute phase of rehabilitation. Thirty subjects with postacute sprained ankles were assigned to a cold (n = 10), heat (n = 10), or contrast bath (n = 10) treatment group. A specially constructed tank was used to take pretreatment and posttreatment volumetric measurements of subjects' sprained ankles. Descriptive statistics, a 3 x 3 two-way analysis of variance for repeated measures, and Tukey's Honestly Significant Dif

[Iatrogenic injury of peripheral nerves].

There are many risks of iatrogenic peripheral-nerve injuries during routine medical procedures. These injuries may occur during venipuncture for drawing blood, endoscopic treatments, punctures of joints or ganglions, various kinds of surgical procedures, and in numerous other situations. It is important to create a "Manual" of such accidents or incidents. In case an accident occurs, both the medical staff and the injured patient should receive adequate support to avoid any anxiety. The doctor must examine the person's injury carefully, and must judge its severity as soon as possible. The docto

A Review of the Risk Factors and Approaches to Prevention of Post-Reperfusion Syndrome During Liver Transplantation.

Post-reperfusion syndrome (PRS) is a severe and highly lethal syndrome that occurs after declamping the portal vein forceps during liver transplantation. It is marked by severe hemodynamic disturbances manifested by decreased mean arterial pressure, increased heart rate and elevated pulmonary artery pressure. The complex pathogenesis of PRS remains understudied. It is generally believed to be related to the large amount of acidic, cold blood that enters the circulation after release of the portal clamp. This blood is rich in oxygen-free radicals and metabolic toxins, which not only aggravate t

An electron microscopic and electron probe study of the microcirculation in cold-induced oedema.

An ultrastructural study of adult mongrel dogs and cats was made to evaluate the changes in the microcirculation during cerebral oedema formation. Two to five cold injuries were made in one hemisphere in dogs and one lesion was made in cats. In several dogs arterial hypertension was induced with a balloon in the aorta. Intracranial pressure (ICP) and water content were measured. The specimens from the oedematous region were studied with transmission electron microscopy (TEM) and electron probe x-ray micro-analysis (EPMA). The TEM data showed swelling of the endothelium and astrocytic foot proc

The effect of a hydrogen sulfide releasing molecule (Na2S) on the cold storage of livers from cardiac dead donor rats. A study in an ex vivo model.

Liver transplantation is currently the preferred treatment option for end-stage liver disease. Donation after cardiac death was a common practice in the early years of organ donation before brain death criteria were established. Those organs were subjected to variable periods of warm ischemia that might intensify cold ischemia/reperfusion injuries. In the present, shortage of brain dead donors has led to the reassessment of organ donation after cardiac death. Since many cytoprotective roles have been describe for H2S during ischemia/reperfusion on a variety of tissues, we hypothesized that gra

Knockdown of spinal opioid receptors by antisense targeting beta-arrestin reduces morphine tolerance and allodynia in rat.

The development of morphine tolerance and sciatic nerve injury-induced allodynia after functional knockdown of spinal opioid receptors using antisense oligonucleotides targeting beta-arrestin was investigated. Ineffectiveness of morphine in neuropathic pain suggests an implication of the same mechanism in these two processes. The development of morphine tolerance (10 microg intrathecally (i.th.), every 12 h) was significantly inhibited in rats, which received i.th. beta-arrestin antisenses (2 nM). Acute and chronic (6 days) i.th. administration of antisenses antagonized the allodynia in the ra

Horse-racing injuries in children before and after the introduction of safety regulations in Mongolia.

The aim of this study was to assess the effectiveness of safety regulations governing traditional Mongolian horse racing on the frequency and severity of injuries among child jockeys. Regulations introduced in 2019 mandate the wearing of helmets and protective clothing, prohibit the participation of jockeys aged < 7 years, and ban horse racing during the cold season (November-April). National injury surveillance data were used to compare the profile of injuries that occurred among children aged < 15 years in the 4-year periods before and after the introduction of the regulations (2015-2018 and

Glial inhibitors influence the mRNA and protein levels of mGlu2/3, 5 and 7 receptors and potentiate the analgesic effects of their ligands in a mouse model of neuropathic pain.

Metabotropic glutamate (mGlu) receptors, which are present on neurons and glial cells, have been shown to play a role in neuropathic pain. The present study sought to investigate how the glial inhibitors minocycline and pentoxifylline alter the effect that chronic constriction injury (CCI) has on the expression of mGlu receptors and on their associated ligands. RT-PCR analysis revealed that seven days after CCI, the mRNA levels of glial markers C1q and GFAP, as well as those of mGlu5 and mGlu3, but not mGlu7, were elevated in the lumbar spinal cord - ipsilateral to the injury. The protein leve

Post-Liver Transplantation Management and Specimen Collection in Daurian Ground Squirrels for Single-Cell RNA Sequencing.

Graft functional recovery and graft-host immunocompatibility represent major prognostic concerns following liver transplantation. Critically, conventional rodent models using rats or mice offer limited insight into the mechanisms of hypothermia-induced graft injuries during preservation. This gap underscores the need for alternative models. Since liver grafts from the cold-adaptive Daurian ground squirrel (DGS; Spermophilus dauricus) can sustain prolonged cold preservation and hence support high survivability in post-transplantation DGSs, consequently, this protocol details the essential post-

Equestrian chilblain: another outdoor recreational hazard.

Herein, we describe two cases and review 14 cases of equestrian chilblain or 'equestrian cold panniculitis' in the literature. The first, a 23-year-old healthy female horse trainer, presented with burning nodular swelling on her lateral thighs. The second was a 34-year-old healthy woman with recurrent nodular eruption on the lateral thighs after horseback riding in the winter. Physical examination of both patients revealed erythematous to violaceous nodules with eczema craquelé-like changes. Laboratory workup for systemic and autoimmune connective tissue disease was negative. Punch biopsies fr

In situ demonstration of improvement of liver mitochondria function by melatonin after cold ischemia.

In a previous investigation, reperfusion with a melatonin-containing medium was demonstrated to enhance bile production and tissue ATP levels in rat livers, cold-preserved with University of Wisconsin (UW) or Celsior solutions, with respect to melatonin-free reperfusion; lipid peroxidation products in the perfusate were not influenced by the indole. This was ascribed to an increased efficiency of the hepatocyte mitochondria induced by melatonin. Reactive oxygen species (ROS) normally leak from the electron transfer chain in mitochondria and excessive ROS production is presumed to mediate ische

Pain control after surgery: a patient's guide, Agency for Health Care Policy and Research.

What is pain? Pain is an uncomfortable feeling that tells you something may be wrong in your body. Pain is your body's way of sending a warning to your brain. Your spinal cord and nerves provide the pathway for messages to travel to and from your brain and the other parts of your body. Receptor nerve cells in and beneath your skin sense heat, cold, light, touch, pressure, and pain. You have thousands of these receptor cells, most sense pain and the fewest sense cold. When there is an injury to your body--in this case surgery--these tiny cells send messages along nerves into your spinal cord an
