BACKGROUND: Hirayama disease (HD) is a rare disease traditionally treated with nonsurgical procedures. Surgical treatment, which is performed by decompression and/or fusion of the cervical spine, is generally considered for rapidly progressive or advanced cases, and the choice of surgical technique is debatable. We describe the first severe case of HD that was successfully treated by posterior lateral mass screw fixation without decompression or fusion.
CASE DESCRIPTION: The patient was a 17-year-old adolescent boy who presented with progressive symptoms bilaterally with an unsatisfactory his

Minimally invasive modalities have demonstrated efficacy in the treatment of neurogenic claudication. Direct comparisons, however, between complication rates of these newer techniques with open surgical techniques for lumbar decompression are lacking. This single-institution study examined neurogenic claudicants between August 2007 and June 2009. A total of 26 patients received open surgical decompression, and 23 patients microendoscopic decompression. Baseline demographic characteristics, peri-operative morbidity and mortality, length of hospital stay, and final disposition following hospital

Thoracic outlet syndrome (TOS) results from compression of the neurovascular structures that pass through the thoracic outlet. TOS may be classified as neurogenic TOS (NTOS), venous TOS (VTOS), or arterial TOS (ATOS). NTOS presents with upper-extremity symptoms attributable to compression of the brachial plexus. VTOS, also known as Paget-Schröetter Syndrome, involves subclavian-axillary venous occlusion, thrombus formation, and rare embolization. ATOS results from obstruction of the subclavian artery with claudication, thrombus formation, and possible embolization. Adjunct diagnostic studies f

We suggest that a clinical diagnosis of posterior cord syndrome indicates primary posterior decompression in cervical spondylotic myelopathy cases. We present two unique cases of failed anterior decompression in neutrally aligned necks with compressive myelopathy and a literature review. Two recent cases of cervical spondylotic myelopathy that failed to respond after anterior surgical decompression and fusion surgery were observed at our institution. Both patients had motor strength preservation but were unable to stand and walk independently and had other clinical findings consistent with pos

Spinal stenosis is an acquired or congenital narrowing of the spinal or nerve-root canals. Surgical treatment is often effective. Acquired spinal stenosis most commonly occurs in those with degenerative disk disease and arthritic facets. If the degenerative process stabilizes and there is adequate room to accommodate the neural contents, symptomatic patients become asymptomatic. Residual stability after decompression must be assessed in patients having multilevel decompression. Fusion maybe indicated. In women with osteoporosis coexisting with degenerative scoliosis and spinal stenosis, decomp

INTRODUCTION: Intraoperative ultrasound (IOUS) has gained recognition as a valuable imaging modality for enhancing surgical precision in neurosurgical procedures. However, its routine clinical integration in spine surgery remains limited.
RESEARCH QUESTION: This study aims to elucidate role of intraoperative ultrasound (IOUS) in spinal surgery and to propose the Spinal Cord Pulsatility Index (SCPI) as a novel, ultrasound-based parameter for evaluating spinal cord decompression.
MATERIAL AND METHODS: This retrospective single-center case series included all consecutive patients who underwent

BACKGROUND: Surgical site infection (SSI) in spinal surgery contributes to significant morbidity and healthcare resource utilization. Few studies have directly compared the rate of minimally invasive surgery (MIS) SSI with open surgery.
OBJECTIVE: To investigate whether MIS techniques had a lower SSI rate in degenerative lumbar procedures as compared with traditional open techniques.
METHODS: A single-center, retrospective review of a prospectively collected database was queried from January 2013 to 2016 for adult patients who underwent lumbar decompression and/or instrumented fusion for whi

BACKGROUND: Therapy of traumatic optic neuropathy (TON) is still discussed controversially. Studies of medical treatment and surgical decompression of the nerve could not find any correlation between therapy and result. Today's knowledge of the treatment in TON is to be analyzed by the latest results in the literature, supplemented by personal experiences with our own patients, who underwent a combination of corticosteroids and surgical decompression.
METHODS: The study group consisted of 9 patients at the age of 13-58 years. 8 patients suffered from a cranial trauma, 1 patient had sinus surg

This paper examines advancements in minimally invasive posterior decompression techniques for lumbar degenerative diseases. It focuses on the unilateral approach for bilateral decompression and bilateral approach for contralateral decompression, in which the entry side is determined independently of the symptomatic side to achieve a facet joint preservation rate of ≥ 70%, while also emphasizing contralateral foraminal decompression(CFD). These techniques address spinal instability by minimizing facet joint resection, reducing postoperative instability. CFD is noteworthy because it enables sing

BACKGROUND/AIM: Surgical treatment of odontogenic cysts in childhood could be accompanied by injury of important anatomical structures. Even though enucleation is considered to be preferable treatment of odontogenic cysts, the specificities of pediatric age favor more conservative surgical approach. The aim of this study was to assess the effectiveness of decompression as the uttermost treatment of odontogenic cysts in the pediatric age.
METHODS: This retrospective study included 22 patients, 7-16 years old, with a single jaw cystic lesion. The majority of these lesions were dentigerous cyst

Unilateral biportal endoscopic (UBE) surgery is an innovative minimally invasive technique for treating lumbar disc herniation. However, conventional surgical positioning, which combines preoperative surface localization with intraoperative fluoroscopy, has several limitations, including suboptimal accuracy, procedural complexity, and reduced operative efficiency. Additionally, incomplete decompression and over-decompression are common issues in UBE surgery, might leading to disastrous outcomes. However, determining the extent of laminectomy remain a significant challenge. This study explores

Dysthyroid optic neuropathy (DON) is the most common cause of visual loss in thyroid associated ophthalmopathy, for which steroid pulse therapy and orbital decompression are the common treatments currently. While steroid pulse therapy was recommended as the first-choice for DON, orbital decompression remained as an indispensable therapy although controversies on such treatment still exist. The mechanism, approaches and outcomes of orbital decompression, as well as its advantages and disadvantages over steroid pulse therapy were retrospectively reviewed, and it was suggested that the combinatio

The authors review the relevant anatomy and provide technical tips for endoscopic decompression of the cubital tunnel. Cubital tunnel syndrome is the second most common nerve compression syndrome in the upper extremity. Until recently, surgeons focused on open decompression combined with submuscular or subcutaneous transposition of the nerve. Decompression was usually limited to the region of the medial epicondyle, and related morbidity was relatively high. Endoscopic decompression is a promising technique because the dissection range can be extended and the scar length can be reduced. The aut

In order to demonstrate the safety and efficacy of transnasal orbital decompression for malignant Graves' ophthalmopathy, we carried out a retrospective chart review and clinical follow-up examination of 78 consecutive patients who were operated on for compressive optic neuropathy (CON) with loss of visual acuity or visual field defects. The intervention - strictly transnasal, endoscopically controlled, bilateral decompression of the medial and inferomedial wall of the orbit - was performed when medical and radiation therapy had failed. A total of 145 endonasal decompressions were performed on

Intimate knowledge of local anatomy is required, especially in the biportal endoscopic approach due to its minimally invasive nature. Conditions such as aberrant anatomy, advanced degeneration, and deformities can make endoscopic lumbar decompression more challenging. The computed tomography (CT)-based surgical navigation system is employed with the biportal endoscopic technique to manage lumbar compressive pathologies. The CT-based navigation system offers clear advantages in precision, allowing for careful decompression without excessive bony work, thus preventing instability and reducing th

OBJECTIVE: The surgical approaches previously reported for facial nerve decompression have focussed on achieving good exposure of the lateral or superior aspects of the geniculate ganglion. This report aims to describe a unique case of facial nerve decompression beneath the geniculate ganglion.
PATIENT: A 30-year-old woman with right-sided facial palsy due to a temporal bone fracture.
INTERVENTION: Bony fragments at the base of the geniculate ganglion were removed via a trans-tensor tympani approach with extended posterior tympanotomy.
RESULTS: The patient’s facial movement recovered succes

Classical trigeminal neuralgia (CTN) is a unilateral and severe facial pain disease, which seriously affects the patient's quality of life. Microvascular decompression (MVD) is currently the most effective surgical method, and it is the only treatment for the etiology of CTN. Imaging for MVD has been increasingly used, and the advantages and disadvantages of endoscopy-assisted vascular decompression surgery have been controversially debated. In this review, we aimed to discuss the advantages of MVD in the treatment of patients with CTN, the importance of using imaging in disease management, an
