Allergic contact dermatitis to personal care products and topical medications in adults with atopic dermatitis.

BACKGROUND: Atopic dermatitis (AD) is associated with skin-barrier disruption, immune dysregulation, and application of emollients and topical medications that might predispose a person toward developing allergic contact dermatitis. OBJECTIVE: To determine the predictors of allergic contact dermatitis and relevant allergens in AD. METHODS: A retrospective chart review was performed for 502 adults (age ≥18 years) who were patch tested to an expanded allergen series during 2014-2017. RESULTS: Overall, 108 (21.5%) had current AD and 109 (21.7%) had past AD. Patients with and without current AD…

[CHRONIC DERMATITIS OF THE DORSUM OF THE FEET AND SHINS, DUE TO ALLERGIC CONTACT DERMATITIS TO LEATHER PRODUCTS].

A 40 year old woman presented with dermatitis mainly involving her dorsal feet and shins, a condition which was present for a prolonged period of time. The distribution was suggestive of allergic contact dermatitis, possibly to her shoes. Patch tests disclosed allergy to chromates, cobalt, colophony, all present in leather products, and allergy to myroxylon pereirae (Balsam of Peru). The patient was instructed to avoid wearing some of her shoes and sitting on her leather sofa. As a consequence, her symptoms ameliorated. This case demonstrates the importance of patch testing with correct interp…

Children with Atopic Dermatitis Should Always be Patch-tested if They Have Hand or Foot Dermatitis.

Atopic dermatitis is the most common chronic inflammatory disease among children in industrialised countries. Many factors influence this disease in a negative way and contact allergy is one such factor. The aim of the study was to examine the frequency of contact allergy among children with the diagnosis atopic dermatitis. Contact allergy was found in 22/82 children (26.8%), the most common from Amerchol L101 (11.0%), potassium dichromate (7.3%), and nickel sulfate (4.9%). A statistically significant difference in contact allergy frequency was demonstrated for those with hand and/or foot ecze…

Guidelines of care for the management of atopic dermatitis: section 1. Diagnosis and assessment of atopic dermatitis.

Atopic dermatitis (AD) is a chronic, pruritic, inflammatory dermatosis that affects up to 25% of children and 2% to 3% of adults. This guideline addresses important clinical questions that arise in the management and care of AD, providing updated and expanded recommendations based on the available evidence. In this first of 4 sections, methods for the diagnosis and monitoring of disease, outcomes measures for assessment, and common clinical associations that affect patients with AD are discussed. Known risk factors for the development of disease are also reviewed.

Treatment of seborrhoeic dermatitis with ketoconazole: I. Response of seborrhoeic dermatitis of the scalp to topical ketoconazole.

A randomized, double-blind, placebo-controlled cross-over study was made of ketoconazole shampoo in the treatment of 20 subjects with seborrhoeic dermatitis of the scalp. Responses were measured by clinicians using clinical gradings, and by the patients using a linear analogue scale. Scaling and itching of the scalp improved significantly with ketoconazole and no response was seen with placebo. Topical ketoconazole appears to be an effective therapy for seborrhoeic dermatitis of the scalp, and more suitable for long-term treatment than the oral preparation.

Dermatitis herpetiformis in monozygous twins: discordance for dermatitis herpetiformis and concordance for gluten-sensitive enteropathy.

A monozygous female twin pair discordant for dermatitis herpetiformis and concordant for gluten-sensitive enteropathy is reported. The diagnosis of dermatitis herpetiformis was verified by demonstrating granular IgA deposits in the uninvolved skin. Gluten-sensitive enteropathy was confirmed according to the ESPGAN criteria. Monozygosity was proved by the standard genetic characteristics.

Atopic Dermatitis and Sleep Disorders.

PURPOSE: Atopic dermatitis (AD) is a common inflammatory skin disease affecting 129 million people worldwide and is characterized by recurrent eczema, severe pruritus, and skin barrier damage. Previous studies on AD-sleep disorder associations were limited by small sample sizes or single-center designs, lacking large-scale epidemiological evidence. This study aimed to investigate the association between AD and sleep disorders using the UK Biobank prospective cohort and to explore effects of sex, race, and obesity on this relationship. METHODS: A prospective cohort design was employed, includi…

Atopic Dermatitis and Academic Performance.

IMPORTANCE: Atopic dermatitis (AD) may impair school performance through sleep disturbance, comorbidities, or psychosocial effects; however, population-based longitudinal evidence is limited. OBJECTIVE: To investigate the association of AD with academic performance and whether this association is modified or confounded by disease phenotype and socioeconomic background. DESIGN, SETTING, AND PARTICIPANTS: Parallel, population-based cohort studies in Denmark and England, including children with or without AD. In Denmark, children born between 1986 and 2000 from nationwide linked registries were…

Management of Atopic Hand Dermatitis.

Atopic hand dermatitis (AHD), a manifestation of atopic dermatitis, can have a profound negative effect on a patient's disease-related quality of life due to its visibility, chronic nature, and overall discomfort that it causes. AHD differs from other forms of chronic hand eczema due to its likely distinct, complex pathogenesis, which is a combination of environmental triggers, genetic predisposition, and immune dysfunction. A proper diagnosis of AHD is made through clinical evaluation and the ability to establish subtle clinical differences between AHD and other conditions. Diagnosis is the f…

Dermatitis Artefacta: A Multidisciplinary Approach.

BACKGROUND Dermatitis artefacta is a challenging and often under-recognized psychodermatosis characterized by self-induced skin injury. Its clinical presentation frequently mimics other chronic ulcers, leading to frequent misdiagnosis and diagnostic delays. This delay prolongs symptoms and perpetuates the detrimental itch-injury cycle, whereby chronic pruritus drives compulsive scratching, creating a barrier to effective healing. CASE REPORT We report the case of a 59-year-old man with a 3-year history of a refractory, intensely pruritic ulcer on the right temporal scalp, which subsequently ex…

Vulvar Dermatitis: Diagnosis and Management.

Candidiasis, herpes simplex virus, bacterial vaginosis, irritant contact dermatitis, allergic contact dermatitis, psoriasis, and eczema/lichen simplex chronicus are often misdiagnosed, mistreated, and underrecognized causes of vulvar discomfort. Vulvar discomfort is frequently encountered in dermatology, gynecology, urology, and primary practice. A lack of ownership among specialties often leads to significant knowledge gaps among these providers and delays in care. Clinical symptoms of vulvar dermatitis can overlap, posing a diagnostic challenge. Herein, we provide a tool to guide the diagnos…
