Giant left main coronary artery aneurysm in association with severe atherosclerotic coronary disease.

Aneurysms of the coronary arteries occur in 1.5-4.9% of coronary angiograms. Only a few cases of a giant aneurysm of the left main coronary artery in conjunction with severe atherosclerotic coronary disease have been reported. This report details the exceptional case of a 72-year-old patient with a giant "true saccular" aneurysm branched on the left main coronary artery. The development of an aneurysmal dilatation in conjunction with progression of coronary arteriosclerosis, observed from two different angiographic studies at an interval of 25 years, is very illustrative for evolutive atherosc

[A peculiar anomaly of the main biliary duct: choledochal cystic dilatation].

The Authors report a case of choledochal cystic dilatation and examine this particular anomaly of the main biliary duct analysing the complex classification. After a brief review of the embryology of the hepato-pancreatic ring, etiopathogenetic theories, clinic characteristics, as well as diagnostic and therapeutic possibilities for this affection are taken into account. It is concluded that the best therapeutic choice should be always based on an accurate evaluation of the anatomo-pathological conditions in each single case.

Loss of ganglion cells and marked attenuation of bowel wall in cecal dilatation.

Cecal dilatation may be encountered in volvulus, distal colonic obstruction, and colonic pseudo-obstruction. Our study attempts to determine whether the dilatation is related to loss of ganglion cells and bowel wall attenuation. The records and colonic pathology of 27 patients who had undergone resection of the right colon for obstruction at the Veterans Affairs Medical Center, Buffalo, New York, were reviewed. Patients with severe ischemic changes and bowel wall necrosis were excluded. The remaining 10 cases were all elderly males who had cecal dilatation associated with cecal volvulus (7 cas

Wolfram's (DIDMOAD) syndrome and its urological manifestation.

We report 2 patients with DIDMOAD syndrome (diabetes insipidus, diabetes mellitus, optic atrophy and neural deafness), with emphasis on the urological aspects and their management. Both patients underwent thorough radiological endoscopic and urodynamic evaluation, in addition to detailed evaluation of other systems involved. Each had the characteristic hyper-reflexive neurogenic bladder with sphincteric dyssynergia, which resulted in severe urinary tract dilation. One patient was diagnosed at this institution and managed conservatively with clean intermittent catheterisation and anticholinergi

Laryngoceles and saccular cysts in infants and children.

Laryngoceles and saccular cysts are an unusual cause of respiratory obstruction in infants and children. Laryngoceles in particular are less common in the pediatric population than in adults. They typically produce intermittent upper airway obstruction and hoarseness (due to episodic filling with air), and are, therefore, difficult to diagnose. Saccular cysts produce similar symptoms but are not episodic. While smaller anterior saccular cysts are readily managed by endoscopic excision, larger saccular cysts tend to recur repeatedly. Review of experience with 20 infants and children having sacc

Classic and vigorous achalasia: a comparison of manometric, radiographic, and clinical findings.

Compared with classic achalasia, vigorous achalasia has been defined as achalasia with relatively high esophageal contraction amplitudes, often with minimal esophageal dilation and prominent tertiary contractions on radiographs, and with the presence of chest pain. However, no study using current manometric techniques has compared manometric, radiographic, and clinical findings in vigorous and classic achalasia or questioned the usefulness of making this distinction. Fifty-four cases involving patients with achalasia whose radiographic and manometric studies were performed within 6 months of e

[One-stage operation in a patient with Marfan's syndrome, severe pectus excavatum and annulo-aortic ectasia].

A case of Marfan's syndrome complicated by annulo-aortic ectasia with pectus excavatum (Wada's 3rd grade classification) and aortic regurgitation (Seller's 1st grade) is reported in a 36-year-old man. We performed one-stage operation combining Bentall's procedure for the annulo-aortic ectasia and sternal elevation for the pectus excavatum. Few reports of one-stage operation for pectus excavatum and annulo-aortic ectasia in Marfan's syndrome have been published. If both diseases are present and surgical indications permit, one-stage operation can be performed to ensure protection of the heart a

Late volume changes in retrosternal colon bypass.

Esophageal obstruction by malignancy, chemical burns, or other less common entities presents a challenge for the surgeon. Either for esophageal substitution after esophageal resection or as a bypass for the obstructed esophagus, colon interposition is often the best available option. Massive colonic enlargement requiring resection of the interposed dilated colon developed recently in 2 of our patients who had a colon bypass 22 and 10 years earlier, respectively.

Plain film diagnosis in meconium plug syndrome, meconium ileus and neonatal Hirschsprung's disease. A scoring system.

Abdominal plain films of 133 neonates, with 82 cases of meconium plug syndrome (MPS), 27 cases of meconium ileus (MI) and 24 cases of neonatal Hirschsprung's disease (HD), were reviewed to assess the value of such radiographs for diagnosis. The radiographs were examined according to a list of 11 parameters. By using multivariate discriminant analysis, it appeared that 4 parameters i.e. dilatation of bowel loops, varying loop calibre, fluid levels and colonic gas were most important in discriminating among the three disorders. For each parameter the weight (in points) was derived. To classify p

Transient gall bladder dilatation associated with hypokalaemia in a patient with Bartter syndrome.

A 5.5-month-old male infant with hypokalaemia and gall bladder dilatation is reported. The child was shown to have Bartter syndrome. After oral treatment with potassium and indomethacin, serum potassium levels became low normal and the gall bladder enlargement resolved. This entity should be included in the clinical spectrum of neuromuscular disturbances resulting from hypokalaemia and should be considered in a hypokalaemic infant with a right-sided abdominal mass.

[The Beals-Hecht syndrome (congenital contractural arachnodactyly) revealed in a neonate].

A new case of congenital contracture arachnodactyly (CCA) revealed in the neonatal period is reported. CCA is a dominantly inherited syndrome associating arachnodactyly, kyphoscoliosis, multiple congenital joint contractures and crumpled ears. This condition differs from Marfan's syndrome by the usual absence of visceral involvement, although cardiac complications are possible. The neonatal forms result from new mutations are are generally severe.

[Unilateral patulous eustachian tube with tinnitus, inner ear damage, vertigo and sudden deafness--collagen injection].

In elderly patients an unilateral sensorineural hearing loss is frequently associated with a relatively more patent eustachian tube on the involved side. A simple method of investigation is observation under the operating microscope during tubal inflation by the patient. In right-handed patients the abnormally patent tube most often lay on the left side. Powerful self inflation in these patients induces acute hearing loss and vertigo. Acute hearing loss is commoner on the left side. The air bone gap is greater at higher frequencies due to mobility of the stapes, loosening of the incudal joints

The role of dilatation in bilharzial ureters.

Endoscopic dilatation of bilharzial ureteric strictures was carried out in 35 men and four women. The procedure had to be repeated in 10 patients after 2 months and three patients have required regular dilatation every 4 months for the last 2 years. The selection of patients is important since they should have neither an active lesion nor a stenotic calcific one.

Large brushite stone in a dilated prostatic urethra.

We report a rare case of a large brushite stone lodged in a dilated prostatic urethra. The patient had been asymptomatic except for an episode of hematuria and he underwent successful removal of the stone. The stone weighed 35 gm., had a concentric layered structure and was composed mainly of brushite.
