Management of Acute Diverticulitis: Role of Minimally Invasive Surgery versus Nonoperative Management.

Acute diverticulitis (AD) is the most common complications of diverticular disease. AD can present with a spectrum of symptoms, ranging from mild, self-limiting inflammation to severe peritonitis. A paradigm shift in the treatment of uncomplicated diverticulitis has been noted, with cumulating evidence challenging the routine use of antibiotics. High-quality evidence suggests that antibiotics may be safely omitted in healthy patients with mild AD. Nonetheless, surgery remains the mainstay treatment of patients with generalized peritonitis or recurrent, debilitating disease. In summary, current…

Diverticulitis Causing Failure of Deep Inferior Epigastric Flap Harvest in Breast Reconstruction.

The deep inferior epigastric perforator flap is the gold standard in autologous tissue reconstruction after mastectomies. During the course of the dissection of the pedicle for this flap, one must peel the deep inferior epigastric artery off the peritoneum. In rare occasions, scarring from previous abdominal surgery can result in adherence of the deep inferior epigastric artery to the peritoneum. In this report, we describe a patient with adherence of the pedicle to the sigmoid colon due to diverticulitis in the absence of surgical history, which resulted in failure of flap harvest and long-te…

A Rare Cause of Right Lower Quadrant Abdominal Pain: Uncomplicated Ileal Diverticulitis.

Ileal diverticulitis is a rare cause of abdominal pain. Even though small intestine diverticulosis is relatively rare, resulting pathologies including diverticulitis are still clinically relevant in both an inpatient and outpatient setting often presenting with varying levels of severity. Most reported cases of ileal diverticulitis are complicated and managed surgically. In contrast to these more complicated presentations, this report illustrates an uncomplicated case of ileal diverticulitis presenting with right lower quadrant abdominal pain and leukocytosis treated successfully conservativel…

Difference in Clinical Features between Right- and Left-Sided Acute Colonic Diverticulitis.

Right colonic diverticulitis (RCD) and left colonic diverticulitis (LCD) may have different clinical features due to the different embryologic origins and anatomical locations of each colon. Therefore, we aimed to compare RCD and LCD in terms of the associated clinical features. We retrospectively collected clinical data from patients who were diagnosed with acute colonic diverticulitis based on computed tomography findings between 2011 and 2017. RCD was defined as colonic diverticulitis extending from the caecum to the transverse colon, and LCD was defined as extending from the splenic flexur…

[Non-surgical management of small bowel diverticulitis with localized perforation:a case report].

A 40-year-old man presented to the emergency department with periumbilical pain and fever. A computed tomographic scan confirmed multiple jejunal diverticulum with localized extraluminal air and panniculitis around it, and jejunal diverticulitis with localized perforation was suspected. His symptoms were mild, and extraluminal air was localized;therefore, he was advised bowel rest and administered only antibiotics. The patient's symptoms resolved without surgical treatment, and at the time of writing this report, there had been no recurrence. Small bowel diverticulitis is rare, and careful ana…

Rare aetiology of abdominal pain: contained abscess secondary to perforated jejunal diverticulitis.

A patient with a history of multiple jejunal diverticulosis (JD) presented with a non-peritonitic abdominal pain and leucocytosis. CT scan showed a thick-walled interloop collection within the left mid-abdomen with dilated bowels and mild diffuse air-fluid levels. Exploratory laparotomy revealed multiple diverticular outpouchings in the mid-jejunum, one of which was perforated, contained within the mesentery. Resection of the contained abscess and primary anastomosis were performed subsequently.

Right colonic diverticulitis in Caucasians: presentation and outcomes versus left-sided disease.

PURPOSE: To compare clinical features, computed tomography (CT) findings, and outcomes of right vs. left colonic diverticulitis (CD) in Caucasians. METHODS: This single-center retrospective case-control study of patients seen between July 2005 and February 2013 included 30 consecutive cases of right CD and 70 controls taken at random from a consecutive cohort of patients with left CD. The final diagnosis was established by consensus between a gastrointestinal surgeon and a gastrointestinal radiologist. Clinical features, treatment, and follow-up data were collected. Two radiologists blinded t…

Adult-onset IgA Vasculitis Initially Attributed to a Cutaneous Drug Reaction and Diverticulitis.

Immunoglobulin A vasculitis (IgAV), formerly Henoch-Schönlein purpura, is a small-vessel vasculitis characterized by palpable purpura, arthralgias, gastrointestinal involvement, and renal disease. Although IgAV is common in children, adult-onset disease is rare and may be associated with more severe manifestations, particularly renal involvement. In adults, IgAV may be difficult to recognize because of overlap with more common gastrointestinal, infectious, or medication-related conditions. We present a case of adult-onset IgAV initially attributed to presumed diverticulitis and later to a drug…

Acute abdomen secondary to perforated jejunal diverticulitis: an unusual clinical presentation - case report.

INTRODUCTION: Jejunal diverticulosis is an uncommon condition, usually asymptomatic that could lead to severe complications such as perforat … ry and was discharged on postoperative day six. DISCUSSION: The clinical manifestations of jejunal diverticulitis are often nonspecific and may mimic appendicitis, cholecystitis, colonic diverticulitis, or urologic disease, as in this case. Although CT is the diagnostic modality of choice, its sensitivity is limited, and definitive diagnosis often requires surg …

Laparoscopic Right Colectomy for Cecal Diverticulitis With Inguinal Abscess: A Rare Case Report.

Right-sided colonic diverticulitis is generally considered less prone to severe complications than left-sided colonic diverticulitis; progression to an inguinal subcutaneous abscess via retroperitoneal extension is extremely rare. Herein, we report a unique case of cecal diverticulitis, wherein a retroperitoneal abscess extended into the inguinal region. A 65-year-old man presented with right inguinal swelling. Imaging revealed an abscess of cecal diverticulitis with retroperitoneal tracking into the inguinal subcutaneous tissue. After initial treatment with antibiotics and drainage, an interv…

Evaluation of laparoscopic peritoneal lavage for perforated diverticulitis: a national registry-based study.

BACKGROUND: Laparoscopic lavage (LPL) has been suggested for treatment of non-feculent perforated diverticulitis. In this observational study, the surgical treatment of diverticular disease in Sweden outside prospective trials was investigated. METHODS: This population-based study used the National Patient Register to identify all patients in Sweden with emergency admissions for diverticular disease, as defined by ICD codes from July 2014 to December 2020. Demographics, surgical procedures and outcomes were assessed. In addition, register data since 1997 were retrieved to assess co-morbiditie…

Acute Jejunal Diverticulitis Induced by a Malignant Gastrointestinal Neuroectodermal Tumor: A Case Report.

Small intestinal diverticula induced by malignant mesenchymal tumors are extremely rare clinical entities. We present the case of a 46-year-old female who reported a one-week history of worsening generalized abdominal pain accompanied by constipation. A computed tomography (CT) scan of her abdomen revealed an ulcerative mass at the proximal/midjejunal junction, suggestive of jejunal diverticulitis. She initially underwent diagnostic laparoscopy, which was converted to an open procedure for resection of the jejunal mass and side-to-side anastomosis. The final histopathological examination revea…
