The changing use of disease-modifying anti-rheumatic drugs in individuals with rheumatoid arthritis from the United Kingdom General Practice Research Database.

OBJECTIVES: To describe the use of disease-modifying anti-rheumatic drugs (DMARDs) in the treatment of rheumatoid arthritis (RA) and changing trends in their use. METHODS: We used the General Practice Research Database (GPRD) to describe DMARD use by patients with RA identified using ICD-9 codes. The GPRD is a UK national database containing records of more than 7 million individuals from 683 general practices. Subjects were studied between 1987 and 2002. The prevalence and duration of individual DMARD use and changing trends in DMARD use were investigated. RESULTS: Thirty-four thousand thre

Stakeholders' experiences with a clinician-led access programme linking evidence generation and reimbursement for precision cancer treatments: the drug access protocol in the Netherlands.

BACKGROUND AND PURPOSE: In the current landscape of tumour-agnostic oncology drugs receiving European Medicines Agency (EMA) authorisation, Health Technology Assessment (HTA) bodies face challenges in assessing these innovative drugs. Due to these products' non-randomised, single-arm nature, uncertainty exists regarding their real-world benefit. In the Netherlands, the Drug Access Protocol (DAP), a programme developed by oncologists, insurers and the healthcare public institute, aims to provide an innovative solution to address this uncertainty. This study aims to investigate the key character

"Biophysical and colloidal properties govern anti-drug antibody generation: A case study using the tumoricidal murine-derived monoclonal antibody 4713".

We investigated how different stress conditions affect the biophysical and colloidal properties of a murine monoclonal antibody (mAb) 4713, whose humanized version is in clinical development, and how these stress-induced properties influence anti-drug antibody (ADA) induction in the Jcl:ICR mouse model. Biophysical analyses showed that non-stressed monomers remained native and non-immunogenic in mice. Stirring and freeze-thawing generated native-like aggregates that retained the tumoricidal activity, whereas heat-induced aggregates were denatured and lost functional activity. Mice immunization

Substance-general and substance-specific influences on adolescent vaping, smoking, alcohol consumption, and illicit drug use: context, inequalities, and putative determinants.

BACKGROUND: Using substances before the age of 15 is a key antecedent of substance use treatment later in life. Since the turn of the 21st century, adolescents' lives have changed considerably, presenting new risk and protective factors. Identifying factors and contexts associated with use of specific substances among contemporary adolescents is therefore a key public health priority. METHODS: We investigated vaping, smoking, alcohol consumption, and illicit drug use, in 30,920 adolescents aged 12-15 attending 100 schools and living across 1539 neighbourhoods in the south of England. Cross-cl

Cardio-Vessel Organoids (CVO) generated from pluripotent stem cell for modelling the cardiovascular system in vitro and revealing the cardiovascular toxicity of drugs.

Cardiovascular toxicity is a leading cause of drug attrition. Conventional animal models are constrained by cost, duration, and interspecies discrepancies. Organoids provide physiologically relevant alternatives for toxicity assessment. Here, we developed cardiovascular organoids (CVOs) by co-differentiating cardiomyocytes and vascular progenitor cells from human embryonic stem cells. CVOs demonstrated enhanced vascular and myocardial maturation relative to standalone cardiomyocyte or vascular organoids. In pharmacological assays, CVOs accurately recapitulated amiodarone bleomycin and cisplati

Next-generation anti-infective drugs in the post-antibiotic era: focusing on anti-virulence agents targeting the bacterial quorum sensing system.

Antimicrobial resistance is a critical global public health challenge, with drug-resistant infections contributing to more than one million deaths annually. The widespread dissemination of multidrug-resistant bacteria poses a severe threat to the management of infectious diseases. Bacterial evolution via genetic mutation and horizontal gene transfer diminishes antimicrobial efficacy, often leading to therapeutic failure, increased morbidity and mortality. However, the development of novel antibiotics lags far behind the rapid evolution of drug-resistant bacteria. Therefore, scientists worldwid

Temporal Trends in HIV-1 Subtypes and Antiretroviral Drug Resistance Mutations in Istanbul, Türkiye (2021-2024): A Next-Generation Sequencing Study.

HIV-1 genotyping and drug resistance tests are routinely performed in virology laboratories in some countries, aiding clinical management. In Istanbul, between January 2021 and March 2024, plasma samples from 1029 HIV-1-infected patients were analyzed using the NGS method, and mutation and drug resistance results were retrospectively evaluated alongside demographic data. Subtype B (54.4%) was most frequent in Turkish patients, while Subtype A1 (43.5%) was predominant among foreign nationals. The most common CRFs were CRF02_AG (3.8%) and CRF56_cpx (1.6%). According to the change in detection ra

Generation of an Induced Pluripotent Stem Cell-Derived Alveolar Type II In Vitro Model to Study Influenza A Virus Infection and Drug Treatments.

Influenza viruses (IVs) represent a significant global health issue, capable of causing seasonal epidemics and occasional pandemics with substantial morbidity and mortality. The emergence of viral resistance further complicates treatment strategies. In this study, induced pluripotent stem cell-derived human alveolar type II (iAT2) cells are used to model influenza A virus (IAV) infection and to assess antiviral responses. Cultured at an air-liquid interface (ALI) in transwell systems, iAT2 cells recapitulate key features of the alveolar epithelium and support productive IAV replication. Upon i

Concurrent Prescribing of Opioids and Sedative-Hypnotic Drugs for Long-Term Use in Australian General Practice: A Cross-Sectional Analysis Using MedicineInsight.

The number of unintentional deaths involving opioid and/or benzodiazepine use continues to increase in Australia. This study examined patterns of concurrent prescribing of opioids and benzodiazepines/Z-drugs (BZDs) for long-term use in Australian general practice. A cross-sectional analysis was undertaken using MedicineInsight, a national database of de-identified general practice electronic health records. We estimated the proportion of patients (per 1000, ‰) in 2017 receiving concurrent prescriptions for opioid and BZD medications for long-term use (≥ 3 prescriptions within 90 days). Poisson

Targeted next-generation sequencing for drug-resistant tuberculosis diagnosis: implementation considerations for bacterial load, regimen selection and diagnostic algorithm placement.

INTRODUCTION: Early and accurate diagnosis of drug-resistant tuberculosis (DR-TB) is essential for improving treatment outcomes. Phenotypic drug susceptibility testing (pDST) is comprehensive but slow, while rapid molecular assays provide resistance information for a limited number of drugs. Targeted next-generation sequencing (tNGS) offers the potential for broad and rapid resistance detection, but its integration into diagnostic algorithms has been hindered by uncertainty about its placement within existing workflows. METHODS: This study evaluated the extent to which two tNGS solutions-Deep

Synthetic cathinones in drug-facilitated sexual assault: A case report involving the novel generation substituted cathinone N-ethylpentedrone and a review of the literature.

The use of 3,4-methylenedioxymethamphetamine (MDMA) in drug-facilitated sexual assault (DFSA) is not uncommon. Indeed, the effects associated with the use of this substance may lead to disinhibition. Several synthetic cathinones, such as mephedrone or methylone, also possess marked entactogenic properties. This manuscript aims to (i) report a DFSA case involving a novel cathinone derivative, namely N-ethyl-pentedrone (NEPD) and (ii) review previously reported DFSA cases involving synthetic cathinones. Using liquid chromatography-high-resolution mass spectrometry (LC-HRMS), NEPD was detected in

Successful healing of aneurysmal false lumen using a second-generation drug-eluting stent in spontaneous coronary artery dissection: a case report.

BACKGROUND: According to 2023 ESC Guideline, conservative medical management is generally recommended for the treatment of spontaneous coronary artery dissection (SCAD) except for patients with signs of ongoing myocardial ischemia. However, in some cases, invasive treatment (coronary artery bypass graft surgery or percutaneous coronary intervention (PCI)) is performed because of the progression of aneurysm in SCAD. Although there is no established strategy for the management of coronary aneurysm in SCAD, we report a case of successful healing of aneurysmal false lumen (AFL) using a second-gene

Left main stem stenosis angioplasty with intravascular ultrasound optimisation criteria guidance using a new generation everolimus drug-eluting stent.

INTRODUCTION: Intravascular ultrasound (IVUS) is recommended in the use of left main stem (LMS) percutaneous coronary intervention (PCI). Since the LMS diameter is usually larger than other coronary arteries, a new generation everolimus drug-eluting stent (DES), Synergy Megatron DES (Boston Scientific) has better axial and radial strength allowing more post implant overexpansion and consequently better suited for LMS lesions. We performed a study to evaluate the clinical outcomes of PCI using 1) an improved IVUS protocol with optimisation targets and 2) the use of Megatron stents. MATERIALS A

An integrated microfluidics platform with high-throughput single-cell cloning array and concentration gradient generator for efficient cancer drug effect screening.

BACKGROUND: Tumor cell heterogeneity mediated drug resistance has been recognized as the stumbling block of cancer treatment. Elucidating the cytotoxicity of anticancer drugs at single-cell level in a high-throughput way is thus of great value for developing precision therapy. However, current techniques suffer from limitations in dynamically characterizing the responses of thousands of single cells or cell clones presented to multiple drug conditions. METHODS: We developed a new microfluidics-based "SMART" platform that is Simple to operate, able to generate a Massive single-cell array and M

As the virus evolves, so too must we: a drug developer's perspective : We need a new paradigm in searching for next-generation countermeasures.

The SARS-CoV-2 virus has been raging globally for over 2 years with no end in sight. It has become clear that this virus possesses enormous genetic plasticity, and it will not be eradicated. Under increasing selective pressure from population immunity, the evolution of SARS-CoV-2 has driven it towards greater infectivity, and evasion of humoral and cellular immunity. Omicron and its expanding army of subvariants and recombinants have impaired vaccine protection and made most antibody drugs obsolete. Antiviral drugs, though presently effective, may select for more resistant strains over time. I
