BACKGROUND: Experimental studies have shown that creating a window in the bony cover of the cochlea and vestibular parts of the inner ear, with preservation of membranous and middle-ear functions, induces an air-bone gap (ABG). This study sought to determine if a similar mechanism explains the ABG frequently observed in audiograms of cochlear implant candidates.
METHOD: The study group included 47 candidates for a cochlear implant (94 ears) attending a university-affiliated tertiary medical center who had an ABG component in the audiogram in the absence of external or middle-ear abnormalities

Hox genes play a crucial role during embryonic patterning and organogenesis. Of the 39 Hox genes, Hoxa1 is the first to be expressed during embryogenesis and the only anterior Hox gene linked to a human syndrome. Hoxa1 is necessary for the proper development of the brainstem, inner ear and heart in humans and mice; however, almost nothing is known about the molecular downstream targets through which it exerts its function. To gain insight into the transcriptional network regulated by this protein, we performed microarray analysis on tissue microdissected from the prospective rhombomere 3-5 reg

BACKGROUND: Treatment of inner ear diseases remains a problem because of limited passage through the blood-inner ear barriers and lack of control with the delivery of treatment agents by intravenous or oral administration. As a minimally-invasive approach, intratympanic delivery of multifunctional nanoparticles (MFNPs) carrying genes or drugs to the inner ear is a future therapy for treating inner ear diseases, including sensorineural hearing loss (SNHL) and Meniere's disease. In an attempt to track the dynamics and distribution of nanoparticles in vivo, here we describe manufacturing MRI trac

Homozygous mutations in the fibroblast growth factor 3 (FGF3) gene have recently been discovered in an autosomal recessive form of syndromic deafness characterized by complete labyrinthine aplasia (Michel aplasia), microtia, and microdontia (OMIM 610706 - LAMM). In order to better characterize the phenotypic spectrum associated with FGF3 mutations, we sequenced the FGF3 gene in 10 unrelated families in which probands had congenital deafness associated with various inner ear anomalies, including Michel aplasia, with or without tooth or external ear anomalies. FGF3 sequence changes were not foun

Performing stapes surgery for otosclerosis is known to be potentially irreversibly harmful to the inner ear function in about 1% of the cases. An early postoperative transient depression of the bone conduction thresholds is frequently detected after stapes surgery. The purpose of this study was to compare the evolution of bone conduction thresholds after primary stapedotomy with two different techniques: skeeter versus CO(2) laser stapedotomy. Audiological data of 336 otosclerosis operations performed by 2 surgeons between 1997 and 2003 were subjected to analysis. The calibrated hole in the fo

A 9-year-old boy with inner ear malformation complained of slight deafness and unsteadiness. CT revealed a normal cochlea despite enlargement of the lateral semicircular canals. The vertical semicircular canals developed more or less normally. The caloric test showed complete canal paresis bilaterally; however, a horizontal rotational stimulus elicited a vestibulo-ocular response, which showed only rightward and downward nystagmus, and their maximal slow-phase velocities were low. In addition, the examination of the vertical semicircular canal function using the head-tilted rotation test revea

Guinea pigs systematically challenged with type II collagen were examined electrophysiologically, histologically, and immunologically for inner ear lesions. Some functional, morphologic, and immunologic changes could be demonstrated. Evidence of possible immunoglobulin deposition in the basilar membrane, the perivascular region, as well as in other inner ear tissue were present. Immunologic and histopathologic simulations to inner ear diseases in humans and to the inner ear antigen-induced animal model were found. The results are discussed in light of the original model introduced by Yoo et al

Active hearing implants have been developed to varying degrees for conductive hearing loss as well as for sensorineural hearing loss. Implants for conductive hearing loss match impedance transformation by the middle ear. They will be referred to as ITI (impedance transformation implants). Three partial ITIs have been developed for routine clinical use: the Swedish transcutaneous BAHA, the American subcutaneous AUDIANT, and the Japanese P-MEI. Of greater importance with respect to the number of patients are electronic implants for sensorineural hearing loss. These implants are designed to repla

BACKGROUND: Neural stem cell (NSC) transplantation exerts therapeutic effects on inner ear damage and pretreatment with quinazoline compounds may further enhance this efficacy.
OBJECTIVES: This study aimed to investigate whether quinazoline alleviates inner ear damage by enhancing the proliferation and differentiation of NSCs.
METHODS: Forty Hartley guinea pigs were randomly assigned to four groups. Hippocampal NSCs isolated from neonatal guinea pigs were pretreated with quinazoline and transplanted into animals with cisplatin-induced hearing loss. Evaluations included NSC proliferation and

OBJECTIVE: To determine the prevalence of anatomic obstructions, including pseudomembranes and tissue plugs, in the round window niche among human temporal bone specimens. To identify patient factors associated with these obstructions.
BACKGROUND: Effective drug delivery to the inner ear is critical for intratympanic and emerging intracochlear therapies for sensorineural hearing loss (SNHL). Both rely on access to the round window membrane (RWM), which may be obstructed by pseudomembranes or tissue plugs that impair therapeutic efficacy. This is the largest survey to date of RWM obstructions

The cochlear aqueduct, located within the temporal bone, forms a narrow connection between the cerebrospinal fluid (CSF) in the subarachnoid space and the perilymph of the scala tympani in the inner ear. The anatomical linkage provides a potential interface for pressure regulation, molecular exchange, and therapeutic access to the cochlea. Recent findings further indicate that the cochlear aqueduct is functionally coupled to the brain's glymphatic system, permitting CSF flow to the inner ear and raising the possibility of an inner ear glymphatic system. Morphological studies demonstrate substa

HYPOTHESIS: Intratympanic (IT) application of betahistine diffuses to the inner ear after 15 minutes of exposure and exhibits no ototoxicity.
BACKGROUND: Management of Menière disease involves oral treatment with betahistine; however, is mitigated by a delayed bioavailability (enteral resorption; first-pass effect). Topical IT administration might achieve higher intracochlear concentration.
METHODS: Animal study on wild-type rats with different IT-betahistine concentrations: 100 mg/mL, 50 mg/mL, 10 mg/mL, 1 mg/mL, and control (saline). For diffusion (n=18) perilymph was sampled postinjection

Gravitational changes have been shown to cause significant abnormalities in various body systems, including the cardiovascular, immune, vestibular, and musculoskeletal systems. While numerous studies have examined the response of the vestibular system to gravitational stimulation, research on functional changes in the peripheral inner ear remains limited. The inner ear comprises two closely related structures: the vestibule and cochlea. These components share similar structures and neural functions, highlighting the importance of investigating changes in auditory nerve cells in response to gra

Autoimmune inner ear disease (AIED) is a rare condition characterized by immune-mediated damage to the inner ear, leading to progressive sensorineural hearing loss (SNHL) and vestibular symptoms such as vertigo and tinnitus. This study investigates the pathogenesis and therapeutic strategies for AIED through the analysis of three cases with different underlying autoimmune disorders: rheumatoid arthritis, relapsing polychondritis, and IgG4-related disease. The etiology of AIED involves complex immunopathological mechanisms, including molecular mimicry and the "bystander effect," with specific a

Fibroblast growth factor 19 (FGF19), and its rodent ortholog FGF15, is a member of a FGF subfamily directly involved in metabolism, acting in an endocrine way. During embryonic development, FGF15/FGF19 also functions as a paracrine or autocrine factor, regulating key events in a large number of organs. In this sense, the Fgf15/Fgf19 genes control the correct development of the brain, eye, inner ear, heart, pharyngeal pouches, tail bud and limbs, among other organs, as well as muscle growth in adulthood. These growth factors show relevant differences according to molecular structures, signallin

The inner ear is the hub where hair cells (HCs) transduce sound, gravity, and head acceleration stimuli to the brain. Hearing and balance rely on mechanosensation, the fastest sensory signals transmitted to the brain. The mechanoelectrical transducer (MET) channel is the entryway for the sound-balance-brain interface, but the channel-complex composition is not entirely known. Here, we report that the mouse utilizes Piezo1 (Pz1) and Piezo2 (Pz2) isoforms as MET-complex components. The Pz channels, expressed in HC stereocilia, and cell lines are co-localized and co-assembled with MET complex par
