Stochastic Modelling of Respiratory System Elastance for Mechanically Ventilated Respiratory Failure Patients.

While lung protective mechanical ventilation (MV) guidelines have been developed to avoid ventilator-induced lung injury (VILI), a one-size-fits-all approach cannot benefit every individual patient. Hence, there is significant need for the ability to provide patient-specific MV settings to ensure safety, and optimise patient care. Model-based approaches enable patient-specific care by identifying time-varying patient-specific parameters, such as respiratory elastance, Ers, to capture inter- and intra-patient variability. However, patient-specific parameters evolve with time, as a function of d

Can titanium elastic nail be safely used for paediatric subtrochanteric femur fractures?

In this study, we aimed to show that subtrochanteric femur fractures, an uncommon type of fracture in the paediatric age group, can be treated with titanium elastic nailing (TEN). We reviewed the patients treated with TEN in the paediatric age group with subtrochanteric femur fractures who had been treated at the Orthopaedics and Traumatology Clinic of Izmir Tepecik Research and Training Hospital between January 2011 and December 2016 retrospectively. All fractures were fixed by retrograde nailing with supracondylar entry following reduction. Patients' demographics as well as data such as frac

Differential biomechanical responses of elastic and muscular arteries to angiotensin II-induced hypertension.

Elastic and muscular arteries are distinguished by their distinct microstructures, biomechanical properties, and smooth muscle cell contractile functions. They also exhibit differential remodeling in aging and hypertension. Although regional differences in biomechanical properties have been compared, few studies have quantified biaxial differences in response to hypertension. Here, we contrast passive and active changes in large elastic and medium- and small-sized muscular arteries in adult mice in response to chronic infusion of angiotensin over 14 days. We found a significant increase in wal

Early Aberrant Angiogenesis Due to Elastic Fiber Fragmentation in Aortic Valve Disease.

Elastic fiber fragmentation (EFF) is a hallmark of aortic valve disease (AVD), and neovascularization has been identified as a late finding related to inflammation. We sought to characterize the relationship between early EFF and aberrant angiogenesis. To examine disease progression, regional anatomy and pathology of aortic valve tissue were assessed using histochemistry, immunohistochemistry, and electron microscopy from early-onset (<40 yo) and late-onset (≥40 yo) non-syndromic AVD specimens. To assess the effects of EFF on early AVD processes, valve tissue from Williams and Marfan syndrome

Individual differences impacting skin deformation and tactile discrimination with compliant elastic surfaces.

Individual differences in tactile acuity are observed within and between age cohorts. Such differences in acuity may be attributed to various sources, including aspects of nervous system, skin mechanics, finger size, cognitive and behavioral factors, etc. This work considers individual differences, within a younger cohort of participants, in discriminating compliant surfaces. These participants exhibit a range of finger size and stiffness. Interestingly, both their finger size and stiffness well predict their discriminative performance, where softer/smaller fingers outperform stiffer/larger fi

Stem cell derived extracellular vesicles for vascular elastic matrix regenerative repair.

Abdominal aortic aneurysms (AAA) are localized expansions of the abdominal aorta that develop due to chronic proteolytic disruption of the structural extracellular matrix (ECM) components (elastin and collagen) within the aorta wall. Major limitations in arresting or reversing AAAs lie in naturally poor and aberrant regeneration and repair of elastic matrix structures in the aorta wall. Bone marrow derived mesenchymal stem cells (BM-MSCs) have emerged as a promising regenerative tool and their therapeutic effects are also known to be effected through their paracrine secretions. Extracellular v

The Effect of Glue Cohesive Stiffness on the Elastic Performance of Bent Wood-CFRP Beams.

This paper presents experimental, theoretical and numerical studies of wood-CFRP beams bonded with polyurethane (PUR) adhesive. The analyses include two types of CFRP (carbon fibre-reinforced polymer) strengthening configurations and pure glue laminated timber beams as a reference. Through detailed analyses of a double-lap connection on blocks with and without CFRP strips, the authors state that neglecting the cohesive stiffness of adhesive layers may lead to an overestimation of an overall beam's stiffness. This is significant with wood-CFRP connections, which showed values two times lower th

Does an Elastic Compression Bandage Provide Any Benefit After Primary TKA?

BACKGROUND: Compression bandages often are used after TKA to reduce swelling. However, the degree to which they are helpful has not been well characterized. QUESTIONS/PURPOSES: The purpose of this study was to determine whether use of a compression bandage after TKA was associated with (1) less leg swelling (our primary endpoint); or (2) secondary study endpoints, including improved ROM of flexion and extension, lower visual analog scale (VAS) pain scores for worst pain and pain during physical therapy just before surgery, postoperative day (POD) 1, POD 2, and POD 28, or fewer wound complicat

Force sharing and force generation by two teams of elastically coupled molecular motors.

Many active cellular processes such as long-distance cargo transport, spindle organization, as well as flagellar and ciliary beating are driven by molecular motors. These motor proteins act collectively and typically work in small teams. One particularly interesting example is two teams of antagonistic motors that pull a common cargo into opposite directions, thereby generating mutual interaction forces. Important issues regarding such multiple motor systems are whether or not motors from the same team share their load equally, and how the collectively generated forces depend on the single mot

Abnormal Pulmonary Arterial Elastance Is Associated With Reduced Exercise Capacity in Tetralogy of Fallot.

Background A previous study reported histologic abnormalities in the pulmonary artery ( PA ) of patients with tetralogy of Fallot ( TOF ). However, the potential effect of these anatomical findings on PA vascular function has not been studied. We hypothesized that TOF patients had abnormal PA vascular function, and that PA vascular function was associated with exercise capacity. Methods and Results This a study of adult TOF patients who had cardiac magnetic resonance imaging and echocardiogram (on the same day) at Mayo Clinic, 2002-2015. In order to test the study hypothesis, we compared PA el

Pushing and loss of elastic fibers are highly specific for melanoma and rare in melanocytic nevi.

The histopathological differentiation of melanocytic nevi from malignant melanoma (MM) is based on well-known criteria, and is straightforward in the vast majority of cases. However, there are few cases of melanocytic lesions (ML), the diagnosis of which is very challenging or even impossible. Here we have studied several morphological characteristics with particular focus on elastic fibers (EF) to identify features, helpful for the distinction between nevi and MM. In a monocentric retrospective study we have analyzed 14 morphological histological characteristics in 30 MMs and 90 nevi, encompa

The elastic capacity of a tendon-repair construct influences the force necessary to induce gapping.

PURPOSE: Most biomechanical investigations of tendon repairs were based on output measures from hydraulic loading machines, therefore, accounting for construct failure rather than true gapping within the rupture zone. It was hypothesized that the elastic capacity of a tendon-repair construct influences the force necessary to induce gapping. METHODS: A tendon-repair model was created in 48 porcine lower hind limbs, which were allocated to three fixation techniques: (1) Krackow, (2) transosseous and (3) anchor fixation. Loading was performed based on a standardized phased load-to-failure protoc

Elastic Stable Intramedullary Nailing of Femoral Shaft Fracture-Experience in 48 Children.

BACKGROUND: Femoral shaft fractures are an incapacitating pediatric injury accounting for 1.6% of all pediatric bony injuries. Management of these fractures is largely directed by age, fracture pattern, associated injuries, built of the child and socioeconomic status of the family. We retrospectively evaluated the use of elastic stable intramedullary nail (ESIN) in surgical management of femoral shaft fractures in children and its complications. METHODS: Fifty two children were treated with titanium elastic nails (TEN) from June 2009 to June 2014 at our institution. At the end of the study th

Intrinsic foot muscles contribute to elastic energy storage and return in the human foot.

The human foot is uniquely stiff to enable forward propulsion, yet also possesses sufficient elasticity to act as an energy store, recycling mechanical energy during locomotion. Historically, this dichotomous function has been attributed to the passive contribution of the plantar aponeurosis. However, recent evidence highlights the potential for muscles to modulate the energetic function of the foot actively. Here, we test the hypothesis that the central nervous system can actively control the foot's energetic function, via activation of the muscles within the foot's longitudinal arch. We used…

Plantarflexor passive-elastic properties related to BMI and walking performance in older women.

The objective of this study was to examine the influence of BMI on the passive-elastic properties of the ankle plantarflexors in older women. Twenty-three women, 65-80 yr, were separated into normal weight (NW, BMI <25.0kgm-2, n=11) and overweight-obese (OW, BMI≥25.0kgm-2, n=12) groups. Resistive torque of the ankle plantarflexors was recorded on an isokinetic dynamometer by passively moving the ankle into dorsiflexion. Stiffness, work absorption, and hysteresis were calculated across an ankle dorsiflexion angle of 10-15°. Maximal plantarflexor strength was assessed, then participants walked a

Study on electric energy consumed in intermittent series-parallel elastic actuators (iSPEA).

On compliant actuators, intermittent series-parallel elastic actuators (iSPEA) can reduce motor load by variable load cancellation through recruitment of parallel springs by a single motor. However, the potential to reduce electric energy consumed, compared to a traditional stiff driven joint has not yet been evaluated thoroughly both in simulations and experiments. We have developed a 1DOF MACCEPA-based iSPEA test bench with a self-closing intermittent mechanism. An iSPEA driven warehouse robot is used as a case study in simulation. A method to compare iSPEA and traditional actuators is propo

Elastic Modulus of Osteoporotic Mouse Femur Based on Femoral Head Compression Test.

A biomechanical test is a good evaluation method that describes the structural, functional, and pathological differences in the bones, such as osteoporosis and fracture. The tensile test, compression test, and bending test are generally performed to evaluate the elastic modulus of the bone using mice. In particular, the femoral head compression test is mainly used for verifying the osteoporosis change of the femoral neck. This study conducted bone mineral density analysis using in vivo microcomputed tomography (micro-CT) to observe changes in osteoporosis over time. It proposed a method of ide
