Bisphosphonate-related osteonecrosis of the jaw: a review of the literature.

Bisphosphonates (BPs) are a class of drugs used to treat osteoporosis and malignant bone metastasis. BPs show high binding capacity to the bone matrix, especially in sites of active bone metabolism. The American Society for Bone and Mineral Research defines BRONJ as "an area of exposed bone in the maxillofacial region that has not healed within 8 weeks after identification by a healthcare provider in a patient who is receiving or has been exposed to a bisphosphonate and has not had radiation therapy to the craniofacial region." Bisphosphonate-related osteonecrosis of the jaw (BRONJ) can advers

Innovative Biomaterials for the Treatment of Bone Cancer.

Bone cancer is a demanding challenge for contemporary medicine due to its high frequency of presentation and significant heterogeneity of malignant lesions developing within the bone. To date, available treatments are rarely curative and are primarily aimed at prolonging patients' survival and ameliorating their quality of life. Furthermore, both pharmacological and surgical therapies are aggravated by a consistent burden of adverse events and subsequent disability due to the loss of healthy bone structural and functional properties. Therefore, great research efforts are being made to develop

A Comprehensive Review of the Role of the Latest Minimally Invasive Neurosurgery Techniques and Outcomes for Brain and Spinal Surgeries.

Minimally invasive neurosurgical techniques offer reduced surgical trauma, lower complication rates, and faster recovery compared to traditional open procedures. Approaches such as keyhole craniotomy, endoscopic skull base surgery, endovascular neurosurgery, and percutaneous spinal techniques have advanced the management of cranial and spinal pathologies. These methods aim to minimize tissue disruption, blood loss, and hospital stay while maintaining surgical efficacy. This narrative review explores their evolution, applications, and advantages in modern neurosurgery.

Bone metastases in thyroid cancer.

Whereas preemptive screening for the presence of lymph node and lung metastases is standard-of-care in thyroid cancer patients, bone metastases are less well studied and are often neglected in thyroid cancer patient surveillance. Bone metastases in thyroid cancer are, however, independently associated with poor/worse prognosis with a median overall survival from detection of only 4 years despite an otherwise excellent prognosis for the vast majority of thyroid cancer patients. In this review we summarize the state of current knowledge as pertinent to bony metastatic disease in thyroid cancer,

A Tailored Approach for Appendicular Impending and Pathologic Fractures in Solid Cancer Metastases.

Advances in medical and surgical treatment have played a major role in increasing the survival rates of cancer patients with metastatic bone disease. The clinical course of patients with bone metastases is often impaired by bone complications, such as bone fractures, which have a substantial negative impact on clinical outcomes. To optimize clinical results and prevent a detrimental impact on patients' health, a tailored approach should be defined for any given patient. The optimal management of impending or pathologic fractures is unknown and relies on a multidisciplinary approach to tailor c

A multicenter case registry study on medication-related osteonecrosis of the jaw in patients with advanced cancer.

PURPOSE: This observational case registry study was designed to describe the natural history of cancer patients with medication-related osteonecrosis of the jaw (ONJ) and evaluate the ONJ resolution rate. METHODS: Adults with a diagnosis of cancer and with a new diagnosis of ONJ were enrolled and evaluated by a dental specialist at baseline and every 3 months for 2 years and then every 6 months for 3 years until death, consent withdrawal, or loss to follow-up. The primary endpoint was the rate and time course of ONJ resolution. Secondary endpoints included frequency of incident ONJ risk facto

Unlocking the potential of CD70 as a novel immunotherapeutic target for non-small cell lung cancer.

Although normally restricted to activated T and B cells and mature dendritic cells, constitutive expression of CD70, a member of the tumor necrosis family, has been described in both hematological and solid tumors, where it increases tumor cell and regulatory T cell survival by signaling through its receptor, CD27.We have assessed the co-expression of CD70 and CD27 in non-small cell lung cancer (NSCLC) by immunohistochemistry to explore a correlation between expression of the protein and tumor histologic subtype, genetic aberrations and prognosis. Furthermore, we tested the ability of ARGX-110

Multidisciplinary Management of Morbidities Associated with Chronic Graft-Versus-Host Disease.

Chronic graft-versus-host disease (cGVHD) represents a common long-term complication after allogeneic hematopoietic stem cell transplantation (HSCT). It imposes a significant morbidity burden and is the leading cause of non-relapse mortality among long-term HSCT survivors. cGVHD can manifest in nearly any organ, severely affecting the quality of life of a transplant survivor. While the mainstay of treatment has remained systemic immunosuppression with glucocorticoids, progress has been made within the last few years with approvals of three oral agents to treat steroid-refractory cGVHD: ibrutin

Management of Primary and Secondary Hyperparathyroidism in a Patient With Chronic Kidney Disease (CKD).

An 82-year-old female with stage 4 chronic kidney disease (CKD) presented with asymptomatic hypercalcemia and elevated parathyroid hormone (PTH) levels. Workup confirmed primary hyperparathyroidism due to a parathyroid adenoma, with secondary hyperparathyroidism due to CKD. Given her age and surgical risk, she was managed medically with cinacalcet, later switched to calcitriol due to hypocalcemia, then reverted to cinacalcet after recurrent hypercalcemia. This case illustrates the diagnostic and therapeutic challenges of managing coexisting primary and secondary hyperparathyroidism in CKD, hig

A diagnostic challenge of mysterious bone pain caused by tumor-induced osteomalacia with multiple tumors: a case report.

BACKGROUND: Tumor-induced osteomalacia (TIO) is a rare, tumor-induced, metabolic bone disorder, the exact incidence of which is unknown. The most common cause of TIO is hypersecretion of tumor-derived fibroblast growth factor 23 (FGF23). Surgical resection can cure TIO in most cases, while for patients with TIO who are ineligible for surgery, biologic antibodies targeting FGF23 can be used as treatment. However, the diagnosis of TIO is more difficult than its treatment as the initial presentation can be misleading or nonspecific; thus, diagnosing TIO remains a clinical challenge. CASE DESCRIP

Photobiomodulation in Medication-Related Osteonecrosis of the Jaw: Outcomes in Stage I and Its Adjunctive Role in Advanced Cases.

Background/Objectives: The development of pharmacotherapy, particularly in antiangiogenic drugs, has led to the emergence of MRONJ as a significant side effect. With the increasing incidence of cancer, the management of MRONJ poses a growing challenge for clinicians. The aim of the study is to evaluate the effectiveness of photobiomodulation (PBM) in treating patients with stage I, II, and III medication-related osteonecrosis of the jaw (MRONJ). Methods: A total of 31 patients were divided into two groups: Group 1 (n = 14 patients), with Stage 1 MRONJ; and Group 2 (n = 17 patients), with Stage

Tumor-Specific Success Probabilities and Factors Associated With Phase III Trials in Oncology Drug Development.

Despite advances in novel therapeutic modalities, such as molecularly targeted agents and immunotherapies, the probability of success in Phase III oncology trials remains low. This study quantitatively characterized success probabilities of Phase III trials in oncology drug development and evaluated the factors associated with trial success. Phase III interventional oncology drug trials registered at ClinicalTrials.gov between 2007 and 2023 with publicly available primary endpoint results were included. Trial success was defined as the achievement of at least one primary endpoint, and tumor-sp

The Story of Localized Amyloidosis: Something Old and Something New - A Case Series.

Localized amyloidosis is a rare benign disease characterized by extracellular deposition of misfolded proteins in a specific organ without systemic involvement, which might lead to organ dysfunction. Patients with localized amyloidosis do not develop systemic diseases such as cardiac, renal, hepatic, or nerve involvement. Typically, the localized disease is managed by surgical resection, while the systemic disease is treated with a combination chemotherapy and immunotherapy. However, some patients can have challenging clinical presentations, delaying diagnosis and organ dysfunction, and they m
