Aldosterone assessment in patients with Menière's disease.

UNLABELLED: Since 1938 endolymphatic hydrops has generally been accepted as the basic histopathological substrate of Menière's disease. In animal studies it has been found that exogenous administration of aldosterone resulted in endolymphatic hydrops. Manifestations of Menière's disease are frequently observed in times of emotional stress. Mediated through the hypothalamus, stress leads to an increased secretion of adrenocorticotropic hormone from the anterior pituitary gland, followed by an increased adrenocortical production of glucocorticoids (cortisol and corticosterone) and mineralocortic…

Diagnosis of Menière's disease on MRI: feasibility at 1.5 Tesla.

BACKGROUND: Menière's disease (MD) is clinically characterized by the triad sensorineural hearing loss, tinnitus and/or aural fullness, and vertigo. Endolymphatic hydrops (EH) is the histopathological basis associated with MD, which can be demonstrated on magnetic resonance imaging (MRI). Currently, most studies are done on a 3-T MRI scanner and to date it is believed that EH can only be demonstrated on a 3-T magnet. We report the feasibility of demonstrating EH on a 1.5-T scanner using the standard 20-channel head and neck coil and the current standard 4-h delayed intravenous gadolinium-enhan…

[Diagnostic value and clinical application of inner ear imaging after an intratympanic injection of gadolinium for Meniere's disease].

OBJECTIVE: To explore the positive visualization rate of endolymph space in Meniere's disease (MD) by three-dimensional fluid-attenuated inv … ce was distinctly visualized on 3D-FLAIR MRI. Forty-nine (96.1%) patients demonstrated the signs of endolymphatic hydrops. Statistically significant differences existed between the results of 3D-FLAIR MRI and those of VEMP and glycerol tests (P < 0.05). No complication of tympanic membrane perforation or infection was observed. CONCLUSION: After an intratympanic injec …

[Effects of inner ear immune responses on auditory function in guinea pigs].

Following direct challenge with KLH antigen primary or secondary (after systemic immunization) to the ES (endolymphatic sac) in guinea pigs, … hat these ECoG findings are caused by immuno-injury to the ES and cochlea, as well as the resultant endolymphatic hydrops which develops acutely and gradually subsides after secondary challenge.

Identification of operculum and surgical results in endolymphatic sac drainage surgery.

To increase the accuracy of identification of the endolymphatic sac during endolymphatic sac drainage surgery, we focused on the intra-operative identification of the operculum as a structural marker of the endolymphatic sac and the vestibular aqueduct at the rugose portion. Then, we examined the relationship between identification of the operculum and surgical outcomes. We used 100 cases with intractable Meniere's disease who received endolymphatic sac drainage with local steroid administration, superior to traditional endolymphatic sac drainage surgery and non-surgical medical treatment. Author's reply to "Letter to the editor on the article: Saccular measurements in routine MRI can predict hydrops in Menière's disease by Simon F et al.".

This is an answer to the Letter to the Editor regarding our previously published article « Saccular measurements in routine MRI can predict … of the study was to present a possible radiological alternative to the more established methods of endolymphatic hydrops visualisation. If accessible, we agree that the latter techniques should be preferred, but we find that they are unfortunately too often unavailable in routine clinical practice to ENT physicians.

[Evaluation of the furosemide test using the air caloric stimulator].

Both the furosemide test, and the glycerol test have been reported as being effective for detection of endolymphatic hydrops. In the furosemide test, the caloric test is performed first, and then repeated 40 minutes after i.v. injection of furosemide, 20 mg. The maximum velocities of each caloric nystagmus are compared, so the caloric stimulus conditions must be constant. In the original method, the caloric test is performed by irrigation with 50 ml of water at 30 degrees C or 44 degrees C for 20 seconds. Since it is difficult to accurately maintain the water temperature at a constant level, h…

Magnetic resonance imaging of the inner ear after both intratympanic and intravenous gadolinium injections.

CONCLUSION: Using magnetic resonance imaging (MRI), endolymphatic hydrops could be visualized on both sides after intratympanic (IT) injection of gadolinium contrast agents (Gd) in one symptomatic ear and subsequent intravenous (IV) Gd injection. The MRI revealed a difference of intracochlear Gd distribution between the IT injection side and the contralateral IV side. OBJECTIVES: Although the IT method allows greater enhancement of the perilymph, many patients feel reluctance in receiving the IT injection in asymptomatic ears. We attempted to evaluate endolymphatic space size on both sides wi…

The long-term outcome of Meniere's disease patients following sac and shunt operation assessed by Hydrops MRI.

PURPOSE: The aim of this study was to evaluate the long-term outcome of Meniere's disease (MD) patients who had previously undergone sac and shunt operation using Hydrops MRI for assessment. METHODS: A total of 17 unilateral MD patients who had previously undergone sac and shunt operation underwent an inner ear test battery, followed by Hydrops MRI. The 34 ears were classified into two groups. Group A consisted of 17 operated ears, which were further subdivided into Group A1 (with endolymphatic hydrops, EH), and Group A2 (without EH). In contrast, Group B comprised 17 non-operated (opposite) …

Posterior Fossa Meningioma with Endolymphatic Sac Compression: Resolution of Dizziness after Surgery.

Dizziness is a prevalent clinical complaint, and determining the underlying etiology can be challenging. Posterior fossa masses have been observed to cause vertigo, with presentations very similar to Meniere's disease. In this report, we describe a case of a patient who presented with recurrent episodes of Meniere's-like vertigo in the setting of a posterior fossa meningioma, who underwent surgical resection of the tumor with significant improvement postoperatively. We postulate that these symptoms were caused by secondary endolymphatic hydrops and were able to demonstrate improvement in high-…

[Hearing loss after spinal anesthesia. An audiological controlled trial].

Audiological tests were conducted on 34 patients before and after undergoing an operation in spinal anaesthesia. One of these patients developed a considerable unilateral hearing loss in the low-frequency range, which persisted until an epidural blood-patch was given. Unexpectedly, we also found a general small but significant threshold shift at 500 Hz, which has never before been described in the literature. The biological mechanism is discussed, and the results suggest that the explanation of the hearing loss could be a condition similar to endolymphatic hydrops, resulting from perilymphatic…

Secondary Meniere's Syndrome Following Excessive Noise Exposure.

BACKGROUND: The effects of noise on the vestibular and balance function are under-studied. Studies have demonstrated that like the cochlea, the otolith organs are also sensitive to sound but the exact mechanism is unclear. Both impulse and continuous noise exposure may damage the cochlear and/or vestibular organ and lead to the formation of endolymphatic hydrops. We describe 9 patients who had Meniere's syndrome following excessive noise exposure. MATERIALS AND METHODS: Study design: retrospective study.Setting: Tertiary care centre in South India.Subject and methods: Medical records of patie…

Endolymphatic sac surgery: analysis of 100 operations.

One hundred endolymphatic mastoid shunt operations in 89 patients with classical Menière's disease were analysed. The disease was bilateral in 18 patients (20%). The patients were carefully pre-selected by a comprehensive protocol of audiovestibular and metabolic investigations. All patients had definite electrophysiological evidence of endolymphatic hydrops with an enhanced negative summating potential on transtympanic electrocochleography. The surgical results were analysed both by the original American Academy of Ophthalmology and Otolaryngology Guidelines (AA00, 1972) and the more recent m…

Enlarged vestibular aqueduct: Intraoperative electrocochleography findings during cochlear implantation.

Enlarged vestibular aqueduct (EVA) is the most frequent inner ear abnormality found on computed tomography in children with sensorineural hearing loss. The effects EVA abnormalities have on electrocochleography (ECochG) are unknown. Positive deflections in summation potential evoked by tone bursts were observed in 3/5 subjects, while a large negative deflection, similar to endolymphatic hydrops (EH), was observed for 2/5 subjects. The presence of an enlarged summation potential, with and without a compound action potential, was observed in response to a broadband click stimulus. Results sugges…

Contralateral Sudden Sensorineural Hearing Loss After Vestibular Schwannoma Surgery.

BACKGROUND: Contralateral sudden sensorineural hearing loss (SNHL) after vestibular schwannoma (VS) surgery is extremely rare, and the mechanism of hearing loss remains unclear. This report aims to provide details of a new case of contralateral SNHL after VS resection, discuss the possible causes of SNHL, and review the relevant literature. CASE DESCRIPTION: A 32-year-old woman developed sudden, profound SNHL after VS surgery. She gradually recovered partial hearing after 6 months of steroid, vasodilator, and neurotrophic therapy. CONCLUSIONS: Compensatory endolymphatic hydrops caused by the…

Case report: two cases of hearing impairment due to intracranial hypotension.

We report two cases of spontaneous intracranial hypotension (SIH) that presented with orthostatic hearing impairment. Pure tone audiometry in case 1 and distortion-produced otoacoustic emissions (DPOAEs) in case 2 clearly revealed orthostatic low-frequency hearing impairment. The symptoms resolved with conservative therapy in case 1, but they did not resolve completely after three treatments with epidural blood patch in case 2. The orthostatic changes at low-frequency that were present in both examinations indicated a relative endolymphatic hydrops due to SIH. Both otological examinations were…

Short-term outcome and prognosis of acute low-tone sensorineural hearing loss by administration of steroid.

Acute low-tone hearing loss (ALHL) is a typical type of hearing loss in Ménière's disease and thought to be caused by endolymphatic hydrops in the inner ear. We treated 40 patients with ALHL by administration of the steroid and the early outcome and prognosis of the hearing level was retrospectively evaluated. The prognosis was generally determined within 7-10 days after administration of steroid. High-dose steroid cured some patients who failed to recover with low-dose steroid therapy. Our results showed that steroid is one of the effective therapies for ALHL and supported that etiology of AL…
